Социальная значимость дисциркуляторной энцефалопатии (ДЭ) определяется выраженностью и степенью прогрессирования когнитивных нарушений (КН), которые она вызывает. Основной проблемой поздней диагностики когнитивной дисфункции, обусловленной ДЭ, является выявление данного состояния уже на необратимой стадии деменции [Локшина А. Способ осуществляется следующим образом.The method is as follows.
Проводится исследование в режиме диффузионно-тензорной МРТ на MP-сканере «Magnetom Verio» с напряженностью магнитного поля 3 Тл (Siemens, Германия), с использованием импульсной последовательности DTI с измерением диффузии в 12 направлениях и параметрами: TR=3600 мс, ТЕ=95 мс, FOV=230×230 мм, толщина среза - 4 мм, количество срезов - 25, различные значения фактора взвешенности для каждого из 12 направлений измеряемой диффузии, длительность исследования - 3 мин 59 с. По результатам нейропсихологического тестирования происходило разделение пациентов на две подгруппы. В 1-ю подгруппу (51 человек) вошли пациенты без нарушений когнитивных функций, у которых при нейропсихологическом тестировании результаты теста MMSE составили от 28 до 30 баллов, результаты теста FAB - от 17 до 18 баллов. Во 2-ю подгруппу (80 чел.) были включены пациенты с умеренными когнитивными нарушениями и наличием деменции легкой степени выраженности. Результаты теста MMSE у пациентов составили от 20 до 27 баллов, FAB - от 12 до 16 баллов. В группу здоровых добровольцев вошли 50 человек без проявлений ДЭ и других сопутствующих заболеваний, в результате исследования сформировавшие базу референсных значений диффузионного тензора. Выбирались пациенты, не имеющие в анамнезе сотрясений и ушибов головного мозга, эндокринной и соматической патологии, токсических поражений, метаболических расстройств, психических отклонений, эпилепсии, когнитивных нарушений. Допускалось наличие в анамнезе испытуемых головных болей напряжения, умеренных проявлений вегетососудистой дистонии. При рутинном MP-исследовании у пациентов референсной группы не визуализировалось патологических изменений головного мозга. Средний возраст референсной группы составил 40,6±1,2 года.According to the results of neuropsychological testing, patients were divided into two subgroups. Patients were selected who did not have a history of concussions and bruises of the brain, endocrine and somatic pathology, toxic lesions, metabolic disorders, mental disorders, epilepsy, cognitive impairment. The history of the tested headaches of tension, moderate manifestations of vegetovascular dystonia was allowed. In a routine MP study, no pathological changes in the brain were visualized in patients of the reference group. The average age of the reference group was 40.6 ± 1.2 years.

Для статистической обработки применялась программа Statistica 6.0 for Windows. Уровень статистической значимости р<0,05. Уточнение взаимосвязи КФА (индекс, %) во всех четырех исследуемых областях с наличием когнитивных нарушений в баллах проводилось с помощью корреляционного анализа Spearman. При сопоставлении значений КФА (индекс, %) между группами пациентов с наличием и отсутствием когнитивных нарушений использовался t-Student-тест. Пороговые значения области риска КФА (индекс, %) рассчитывались как отношение среднего значения КФА в группе без наличия когнитивных нарушений (но со сниженным функционалом) к среднему значению КФА в референсной группе норм, с учетом среднеквадратичного отклонения.Statistica 6.0 for Windows was used for statistical processing. Значения КФА (индекс, %) у пациента М. находились выше зоны риска, несмотря на выраженные дисциркуляторные изменения головного мозга. Значения КФА (индекс, %) у пациента Ю. были ниже, чем у пациента М. и по трем трактам (передние отделы лучистого венца, верхний продольный пучок и переднее бедро внутренней капсулы) попадали в диапазон риска когнитивного снижения. Динамическое наблюдение пациента Ю. в течение года отметило ухудшение его состояние в виде нарастания когнитивного дефицита.
