Abstract
Description
The invention relates to medicine, in particular to neurology, neurophysiology, psychiatry, and in particular to functional diagnostics and can be used to objectify the diagnosis of cognitive impairment by the time of the response of CBP neural networks to a sound stimulus.
The prevalence of cognitive impairment in a population increases with age, including an increase in the number of undifferentiated cognitive impairment, including Alzheimer's disease, Parkinson's disease, senile dementia, which are observed in 50 million people in the modern world. The search for neuromarkers for early diagnosis in most mental illnesses is relevant to ensure not only early diagnosis and prevention, but also personalized treatment and prevention using various non-pharmacological methods of treatment and prevention, including using physical and cognitive exercises, social contacts and lifestyle . This will extend the period of compensation for impaired brain function and give the patient about ten additional years of conscious life.
Currently, the diagnosis of cognitive impairment is established on the basis of clinical interviewing and various psychometric tests. The result depends on the classifications used (International Classification of Diseases 9 (ICD-9), and its revisions (ICD-10, ICD-11), etc.), using subjective criteria and psychometric methods.
A dysfunction of the cortical neural networks, manifested by cognitive impairment and memory impairment, is characterized by a delay in the response of the brain to various stimuli (sounds, flashes of light). The brain reaction can be recorded by EEG, MEG, PET, MRI in combination with functional tests. The most widely used method is EEG, which allows recording changes in the wave activity of the brain at rest (the appearance of slow-wave activity in the frontal lobes, the disappearance of the alpha rhythm) and the response of the brain to a sound stimulus in the form of cognitive evoked potential - P300. The delay (latent period) of the P300 response and its amplitude are recorded on the EEG. An increase in the delay time and a decrease in the response are considered as early manifestations of cognitive impairment, i.e. their neuromarkers. The prototype was the method for detecting the cognitive evoked potential of the P300 EEG (Kropotov Yu.D. Quantitative EEG, cognitive evoked potentials of the human brain and neurotherapy, 2010. Donetsk. Zaslavsky. - 512 s) as an indicator that allows detecting early impairment of neural network function to irritation . The method is carried out by recording an electroencephalogram in combination with a series of sound stimuli that are given binaurally. Against the background of insignificant sound stimuli, the patient is given a random stimulus of a certain frequency in the mode of random events, to which the cognitive evoked potential of P300 should arise. The latent period between a significant stimulus and P300 is determined, as well as the amplitude of the evoked potential of P300 and its components. The disadvantage of this method are:
- сложность метода (недоступность метода для самостоятельного использования практикующими врачами во время амбулаторного приема),- the complexity of the method (inaccessibility of the method for independent use by practicing doctors during outpatient visits),
- необходимость специализированного оборудования (электроэнцефалограф) и компьютерных программ для регистрации и анализа результатов.- the need for specialized equipment (electroencephalograph) and computer programs for recording and analyzing the results.
Effect: simplification and accessibility of the method for the diagnosis of cognitive impairment with high accuracy. This technical result is achieved by five times determining the time of the conditioned reflex movement of the hand to the sound signal, followed by the calculation of the average value of time and the range of its variability.
This technical result is achieved using an EMR-01 electromyoreflexometer (Neurosoft, Ivanovo, RF), which records the time of the conditional movement of the hand to the sound signal supplied by the doctor five times at different time intervals. The average travel time and its variability are determined as the difference between the maximum and minimum values.
The electromyoreflexometer is intended for general and professional psychodiagnostics; it was used for the first time to assess cognitive impairment.
The method is as follows:
The time of the conditional movement of the arm is determined five times after instructing the patient, recording the results of each determination of the time of movement. The conditional movement is set by the doctor, giving a sound signal at different time intervals (3-7 s), the patient responds to the sound by pressing the device key. The average travel time and the travel time variability are calculated as the difference between the maximum and minimum travel times.
Cognitive impairment (delay in the response time of neural networks to an audio signal) is diagnosed with a combination of an increased time of conditioned reflex movement of a hand to an audio signal (more than 0.330 s) and its variability (more than 0.150 s), i.e. differences between the maximum and minimum values of the conditional movement time.
Verified conditions for the applicability of the method: men and women aged 18 to 80 years with undisturbed hand function as a result of injuries and consequences of a stroke, not suffering from severe somatic diseases, not having a history of TBI, not abusing psychoactive substances, not receiving psychopharmacology.
The EMR-01 electromyoreflexometer (Neurosoft, Ivanovo, RF) allows you to record the motor reaction time with an accuracy of 0.001 fractions of a second, including the sound signal that the doctor gives, starting the time counter. The patient is instructed to “press the button of the device after the sound signal” to stop the time counter. The patient sits so that he does not see the scale of the device to exclude the reaction from the visual analyzer. The time of the conditional movement of the patient’s hand, pressing the button in response to the sound signal, is recorded. Practically healthy individuals aged 18 to 73 years were examined: students aged 19 ± 0.3 years (12 people), employees engaged in fitness, aged 39.1 ± 2.7 years (10 people), working pensioners at the age of 66.3 ± 1.6 years (9 people). Also examined were patients with cognitive impairment who sought medical advice from the Center for Cognitive Impairment - 10 people, aged 74.2 ± 1.5 years. The results are presented in the table:
The results obtained have a normal distribution, so sigmal deviations can be used to calculate the standards. M ± 2σ values include 95.5% of all cases for healthy individuals aged 18 to 73 years. Cases with characterization greater than M + 2.0 s (this is 2.3% of all cases) are considered very high, we have identified them as markers indicating initial impairment of cognitive functions. Based on this, the upper limit of the norm of time of the conditioned reflex movement of the arm is M + 2σ, i.e. 0.330 s, the upper limit of the norm of the time of its variability (M + 2σ) is 0.150 s. Exceeding these values indicates impaired cognitive function.
