Abstract
Description
The invention relates to medicine, namely to gastroenterology and endocrinology, and can be used for the treatment of patients with type 2 diabetes mellitus in combination with excess bacterial growth syndrome in the small intestine and Helicobacter pylori chronic gastroduodenal diseases.
2010. Т. 13. №1, С. 77-79).Numerous studies on the problems of diabetes mellitus (DM) and gastroenterology indicate that with diabetes, the digestive tract is affected to one degree or another, and primarily its upper sections (Smolyaninov AB, Novitsky VA, Lebedev NN State of acid-forming function of the stomach in patients with duodenal ulcer combined with diabetes. Clin. Honey. 2001. No. 8, P. 44-47). Pathology of the stomach and 12-intestine was observed in 66% of cases in individuals with type 1 and type 2 diabetes, as well as with metabolic syndrome (Egorova E.G., Zvenigorodskaya L.A., Lazebnik L.B. Metabolic syndrome from the perspective of a gastroenterologist / / Breast cancer. 2005.V. 13, No. 26, S. 1706-1712). An important role in the development of the pathology of the gastroduodenal zone is assigned to the infection of patients with diabetes, as well as the general population, with helicobacteriosis. Patients with type 2 diabetes are at risk for infection with Helicobacter pylori (H. pylori). This is associated with impaired motor-evacuation, secretory function of the gastroduodenal zone, poor compensation of carbohydrate metabolism, depression of immunity (Dedov I.I., Suntsov Yu.I., Kudryakova S.V. Economic problems of diabetes in Russia // Diabetes. 2000 , No. 3, S. 56-58.). The degree of HP infection in patients with type 2 diabetes reaches 40-70% (Mkrtumyan AM, Kazyulin AN, Bairova KI. Frequency and degree of infection with Helicobacter in type 2 diabetes. Diabetes mellitus. 2010. V. 13. No. 1 , S. 77-79).
2013, №1, С. 56-60).With the above changes in the gastrointestinal tract and immunity, frequent development in patients with type 2 diabetes with the syndrome of excessive bacterial growth (SIBR) in the small intestine is also associated. The frequency of this pathology in them, according to various sources, can reach 65% (E.Yu. Plotnikova, M.V. Krasnova, E.N. Baranova, M.V. Borsch. Uninvited guests: excessive bacterial growth in the small intestine. What to do? Gastroenterology. 2013, No. 1, S. 56-60).
2011. №3, С. 125-30).There are various ways to treat SIBR, they are often not standardized, based on a combination of various intestinal antiseptics, probiotics and antispasmodics. One of them includes the use of the antibiotic rifaximin at a dose of 400 mg 2 times a day in combination with a prebiotic - Khilak forte 20-40 drops 3 times a day (E.Yu. Plotnikova, M.V. Borsch, M.V. Krasnova, E. N. Baranova, Some aspects of the diagnosis and treatment of excessive bacterial contamination of the small intestine in clinical practice. Attending physician. 2013. No. 2, P. 76-81). Other authors propose a treatment technique for SIBR based on the use of tetracycline 250 mg 4 times a day, metronidazole 500 mg 3 times a day in combination with lactobacilli (Mayev I.V., Ivashkina I.Yu., Kucheryavy Yu.A., Oganesyan T . C. Diagnosis and treatment of the syndrome of excessive bacterial growth in the small intestine. Experimental and clinical gastroenterology. 2011. No. 3, P. 125-30).
However, these methods have the following disadvantages: they were used in patients who do not suffer from chronic gastroduodenal diseases in combination with type 2 diabetes; these methods did not take into account the infection of patients with type 2 diabetes with gastroduodenal helicobacteriosis. We have not seen any methods of treating SIBR in patients with type 2 diabetes in combination with chronic Helicobacter positive gastroduodenal diseases in domestic and foreign literature.
Н. Инфекция Helicobacter pylori. Known methods for eradicating H. pylori, developed by the International Community of Gastroenterologists and the Russian Gastroenterological Association, are standardized in numerous schemes (Maastricht Consensus 2010, 2015; Recommendations of the Russian Gastroenterological Association for the Treatment of Helicobacteriosis in Adults, Moscow 2012). One of the first-line regimens (initial therapy) is a 10-day classic triple regimen consisting of omeprazole, amoxicillin and clarithromycin. It is the most popular in the world and its effectiveness reaches 80% (Lapina T.L., Mutigulina E.R., Ivashkin V.T. Rational choice of eradication therapy for Helicobacter pylori infection. Russian journal of gastroenterol. Hepatol. Coloproctol. 2013. No. 5, S. 74-80). One alternative to the triple scheme of treatment methods for H. pylori infection is quadrotherapy without bismuth preparations, it is also called a combined scheme. Its composition, taking into account the latest recommendations of Maastricht V (2015) and the resolution of the Expert Council of the Russian Association of Gastroenterologists (2017), is as follows: rabeprazole, amoxicillin, clarithromycin and metronidazole. The duration of treatment is 14 days. To increase the effectiveness of first-line regimens, it is proposed to include in the treatment a probiotic preparation, in particular, the bacterial preparation Saccharomyces boulardii (Mayev I.V., Samsonov A.A., Andreev D.N. Helicobacter pylori infection. - M.: GEOTAR- Media, 2016, 256 pp.).
H.pylori.The influence of anti-Helicobacter therapy regimens on the microbiota of the small intestine is not well understood, the data are contradictory: from negative to neutral (Uspensky Yu.P., Baryshnikova N.V. Use of nitrofuran series drugs in first-line eradication therapy regimens. Breast cancer. 2012. No. 35, C. 1694). There are no data in the literature on the effect of the classical anti-Helicobacter triple regimen, as well as the combined regimen for SIBR in the small intestine in patients with type 2 diabetes mellitus infected with H. pylori.
Close to the proposed method is a method of treating intestinal dysbiosis (Patent for the invention No. 2602696, authors Denisov D.E., Kuznetsova T.N.). The authors suggest that for the treatment of intestinal infections in general and dysbacterioses, in particular, use a three-stage appointment of probiotics from the genus Bacillus, as well as bifidumbacteria and lactobacilli, to suppress pathogenic microflora and improve intestinal mucosa. The method improves the effectiveness of treatment.
Another prototype of the proposed method is a method for the treatment of inflammatory diseases of the gastrointestinal tract associated with H. pylori (Patent for invention No. 2278682, authors Melnikova I.Yu., Petrov L.N., Verbitskaya NB, Dobrolezh O.V. , Kobatov A.I., Dobritsa V.P.). Using this method, the authors conduct monotherapy with the biological product "Vitaflor" for at least four weeks. The dose of the drug is determined by the age of the patients. The use of this biological product contributed to the suppression of local hyperergic reactions, the rapid relief of upper dyspepsia syndrome, the accelerated healing of stomach ulcers and duodenal ulcers without cicatricial defects and deformations, and the prevention of relapse of Helicobacter pylori infection during the year.
However, with all the advantages of these methods of treating intestinal dysbiosis and inflammatory Helicobacter-associated diseases of the gastrointestinal tract, the treatment of patients suffering from type 2 diabetes in combination with chronic gastroduodenal diseases was not carried out. In addition, the concept of inflammatory diseases of the gastrointestinal tract and dysbiosis are not identical to SIBR, and therefore the question of the treatment of the latter has remained open. In these methods, modern anti-Helicobacter therapy was not used in full, therefore it is also impossible to judge its effect on SIBR. All of the above identified the need to create this method of treatment of SIBR.
The purpose of the proposed method is to increase the effectiveness of treatment of SIBR in the small intestine in patients with type 2 diabetes who are infected with helicobacteriosis.
The technical result consists in the normalization of the microbiota of the small intestine, as well as in the high eradication of H. pylori, 1 month after the end of therapy with the proposed scheme in patients with type 2 diabetes and chronic gastroduodenal diseases.
The technical result of the method is achieved as follows. Type 2 diabetes patients who are infected with H. pylori and who are diagnosed with chronic gastroduodenal diseases (chronic erosive and non-erosive gastroduodenitis, peptic ulcer disease), as well as those with SIBR, are offered a combination treatment regimen enhanced with probiotic as anti-Helicobacter pylori. The recipe for this regimen includes: a proton pump inhibitor (rabeprazole at a dose of 20 mg 2 times / day), antibacterial drugs: (amoxicillin 1000 mg 2 times / day, clarithromycin 500 mg 2 times / day, metronidazole 500 mg 3 times / day) and probiotic (bacterial preparation Saccharomyces boulardii 500 mg 2 times / day). All presented preparations are official, approved for use in the conditions of the Russian Federation. All drugs are prescribed simultaneously and the duration of treatment is 14 days.
As a result of this therapy, there is an effective treatment of SIBR, which consists in normalizing the small intestinal microflora, and there is an effective eradication (disappearance) of helicobacteriosis with an improvement in the clinical picture of chronic gastroduodenal diseases in patients with type 2 diabetes.
To confirm the objectivity of the method, we conducted an open, randomized, comparative clinical study. By the principle of random sampling, 68 patients with various chronic gastroduodenal diseases (chronic erosive and non-erosive gastroduodenitis, peptic ulcer) and H. pylori infected were selected. Chronic gastroduodenal diseases were diagnosed according to available examination standards using special methods: esophagogastroduodenoscopy (EGDS), biopsy of the gastric mucosa and 12-intestine with histological examination. H. pylori infection was determined by a urease test (in biopsy samples from at least two places - the body and the stomach antrum) and an ELISA with monoclonal antibodies to detect pathogen antigens in the feces. Moreover, the latter method was required to control eradication, which was carried out 1 month after the end of treatment. All patients had an exacerbation of chronic gastroduodenal diseases clinically and endoscopically.
All patients suffered from type 2 diabetes, they were compensated for by the level of blood glucose and glycated hemoglobin.
Persons suffering from severe decompensated diseases of the heart, lungs, liver, pancreas, and intestines were excluded from the examination. Written consent was obtained from all patients for examination and treatment, according to the order of the Ministry of Health of the Russian Federation No. 173/1 of July 25, 2012 “On informed voluntary consent to medical care”.
I.Wiklund (Wiklund I., Bardhan K.D., Muller-Lissner S., et al. Ital. J. Gastroenterology Hepatol. 1998; 30 (1):19 -27). To determine the clinical effectiveness of the treatment, a methodology for assessing gastroenterological complaints using the GSRS system (Gastrointestinal Simptom Rating Scale) developed by I. Wiklund (Wiklund I., Bardhan KD, Muller-Lissner S., et al. Ital. J. Gastroenterology Hepatol. 1998; 30 (1): 19 -27). The Russian-language version of the GSRS questionnaire was created by researchers of the Interethnic Center for the Study of Quality of Life (Novik A.A., Ionova T.I. Guidelines for the Study of the Quality of Life in Medicine (3rd edition, revised and supplemented). Edited by Yu.L. Shevchenko. M .: Publishing house of the Russian Academy of Natural Sciences, 2012.528 s.). The essence of the technique consisted in a survey of patients on 5 scales: DS - diarrheal syndrome, IS - dyspeptic, CS - constipation, AR - abdominal pain, RS - reflux. For each syndrome, you can get a maximum of 14 to 28 points, depending on the frequency of symptoms. For a general assessment of complaints, scores of different scales were summarized.
To identify SIBR, we used the results of a hydrogen test with lactulose, which was carried out initially and 1 month after the end of treatment. The hydrogen test is recognized as the most informative for assessing the presence of SIBR in the intestine. A gastrolizer (Great Britain) was used as a device for conducting a hydrogen test, and the technique itself did not differ from the standard one (Plotnikova E.Yu., Zakharova Yu.V. Diagnosis and treatment of the syndrome of excessive bacterial growth. Russian Medical Journal. 2015. No. 13 S. 767.). According to the recommendations, the criterion for the presence of SIBR was considered to be an increase in the level of exhaled hydrogen by more than 12 ppm compared to the initial one during the first 30-60 minutes of the study, as well as a two-stage increase in the level of this indicator in the first 120 minutes of the test.
All patients with type 2 diabetes and chronic gastroduodenal diseases were divided into 2 groups: group 1 included 32 people who received a standard triple H. pylori eradication regimen (omeprazole 20 mg 2 times / day, amoxicillin 1000 mg 2 times / day ., clarithromycin 500 mg 2 times / day.) 10 days. Group 2 included patients with type 2 diabetes and chronic hepatitis D who received a combination therapy regimen enhanced with a probiotic: rabeprazole 20 mg 2 times / day, amoxicillin 1000 mg 2 times / day, clarithromycin 500 mg 2 times / day, metronidazole 500 mg 3 times / day., Bacterial preparation Saccharomyces boulardii 500 mg 2 times / day.
0,05.The research results were processed using the Statistic 10 application software package, the absolute values were compared with the calculation of average values, average errors, using Student's criterion. For relative values, Fisher's exact test was used. Differences were considered statistically significant when the p level reached was less than 0.05.
1.Patients of the 1st and 2nd groups were randomized according to a number of demographic and clinical indicators. The results of randomization are presented in table 1.
As can be seen from table 1, patients of the 2 groups did not significantly differ among themselves in such indicators as age, sex distribution, body mass index, disease length, frequency distribution of chronic gastroduodenal diseases, diabetes characteristics, and associated pathology. This allowed them to be compared only by treatment results with various anti-Helicobacter pylori regimens.
2.Initially and 1 month after the end of therapy, the indicated schemes recorded clinical parameters and the results of instrumental examination. The results of the treatment of patients with diabetes and chronic gastroduodenal diseases with various regimens of anti-helicobacter therapy are presented in table 2.
Исходно в данной группе число больных с СИБР составило 68,8%, и не произошло достоверного снижения этого показателя после проведенного лечения (43,8%, р больше 0,05).According to the data in Table 2, in the 1st group of patients receiving classical triple therapy, the number of patients infected with H. pylori significantly decreased by 68.7%. But this percentage did not reach the effective indicator recommended by the International Consensus of 80% or more (Maastricht V). During treatment with a triple regimen, a reliable overall clinical efficacy was noted (a decrease in the overall GSPS index), as well as a decrease in the manifestations of dyspeptic, abdominal and reflux syndromes. Endoscopic efficacy reached 71.8%. Initially, in this group the number of patients with SIBR was 68.8%, and there was no significant decrease in this indicator after treatment (43.8%, p> 0.05).
0,05).When treating patients with an enhanced combined regimen, the percentage of H. pylori eradication was 96.9, which is significantly higher than with triple therapy (68.7%, p less than 0.05). A month after the end of treatment with this regimen, the clinical efficacy was significantly higher than the initial and significantly higher that with triple therapy (GSRS: 13.4 plus / minus 1.3 and 28.3 plus / minus 2.3, respectively, p is less than 0, 05). Significantly decreased the manifestations of all those studied by the syndrome. Endoscopic remission with this type of therapy reached 100%. The number of patients with SIBR significantly decreased after treatment in this group (13.9%, compared with the initial -63.9, p less than 0.05).
Thus, the use of an enhanced combined regimen as anti-Helicobacter pylori therapy, compared to the classical triple therapy, led not only to effective eradication of HP and to a greater clinical and endoscopic effect of the treatment of chronic gastroduodenal diseases in patients with type 2 diabetes, but also to effective treatment of SIBR in these patients.
