Проводился сбор жалоб и анамнеза, общеофтальмологическое обследование: визометрия, периметрия, биомикроскопия переднего отрезка глаза, обратная бинокулярная офтальмоскопия со склерокомпрессией, измерение внутриглазного давления, фоторегистрация. При офтальмоскопии у пациентов с хроническим периферическим увеитом в центральных отделах глазного дна наиболее часто определялся перипапиллярный отек сетчатки, умеренный отек и гиперемия диска зрительного нерва, макулопатия (табл. 3).During ophthalmoscopy in patients with chronic peripheral uveitis in the central parts of the fundus, peripapillary edema of the retina, moderate edema and hyperemia of the optic nerve head, and maculopathy were most often determined (Table 3).
Анализ и сопоставление жалоб, биомикроскопических данных переднего отрезка глаз пациентов с хроническим периферическим увеитом с офтальмоскопической картиной хориоретинальных структур на крайней периферии глазного дна позволили выделить наиболее характерные для данной патологии диагностические признаки, на основании которых составлен способ скрининговой диагностики хронического периферического увеита в виде анкеты-опросника, заполняемой врачом-офтальмологом (табл. 4).Analysis and comparison of complaints, biomicroscopic data of the anterior eye segment of patients with chronic peripheral uveitis with an ophthalmoscopic picture of chorioretinal structures on the extreme periphery of the fundus made it possible to identify the most characteristic diagnostic signs for this pathology, based on which a method for screening diagnosis of chronic peripheral uveitis in the form of a questionnaire was compiled filled by an ophthalmologist (table. 4).
Клинические исследования проведены на базе офтальмологической клиники ФГБОУ ВО СибГМУ Минздрава России (г. Томск) и офтальмологического амбулаторно-поликлинического отделения ООО «Гранд Ретина» среди 208 пациентов (98 женщин и 110 мужчин) в возрасте от 18 до 40 лет с установленным диагнозом хронического периферического увеита. Подтверждение диагноза пациентам проводилось на основании данных офтальмоскопии крайней периферии сетчатки при помощи налобного бинокулярного офтальмоскопа с проведением склерокомпрессии. Критериями исключения из исследования являлись: сопутствующая тяжелая офтальмологическая патология, наличие иммуннокомпрометирующих состояний у пациентов, наличие соматических заболеваний в стадии декомпенсации.Clinical studies were carried out on the basis of the ophthalmology clinic of the Federal State Budget Educational Institution of Higher Education at the Siberian State Medical University of the Ministry of Health of Russia (Tomsk) and the ophthalmological outpatient department of Grand Retina LLC among 208 patients (98 women and 110 men) aged 18 to 40 with a diagnosis of chronic peripheral uveitis. The diagnosis was confirmed on the basis of ophthalmoscopy of the extreme periphery of the retina using a forehead binocular ophthalmoscope with sclerocompression. The exclusion criteria from the study were: concomitant severe ophthalmic pathology, the presence of immunocompromising conditions in patients, the presence of somatic diseases in the stage of decompensation.
Разработанный способ скрининговой диагностики хронического периферического увеита опробован на 42 пациентах, обратившихся на первичную консультацию к врачу-офтальмологу.The developed method for the screening diagnosis of chronic peripheral uveitis was tested on 42 patients who applied for an initial consultation with an ophthalmologist.
Клинический пример 1.Clinical example 1.
Пациентка М., 34 года. Острота зрения обоих глаз 1,0. Рефракция эмметропическая. При обследовании по предложенному способу выявлено:Patient M., 34 years old. The visual acuity of both eyes is 1.0. Emmetropic refraction. When examining the proposed method revealed:
- в структуре жалоб - чувство дискомфорта, нестабильность зрения, покраснение глаз, затруднение фокусировки при длительной зрительной нагрузке;- in the structure of complaints - a feeling of discomfort, instability of vision, redness of the eyes, difficulty focusing with prolonged visual load;
- биомикроскопические данные - пастозность переходной складки, расширение сосудов конъюнктивы (фиг. 1), неравномерный мидриаз, помутнение периферических отделов хрусталика (фиг. 2);- biomicroscopic data - pasty of the transitional fold, dilatation of the vessels of the conjunctiva (Fig. 1), uneven mydriasis, clouding of the peripheral parts of the lens (Fig. 2);
- со стороны центральных отделов глазного дна без грубой патологии.- from the central parts of the fundus without gross pathology.
На основании полученных данных у пациентки был диагностирован хронический периферический увеит и проведена офтальмоскопия со склерокомпрессией. В ходе данного исследования выявлен диффузный отек сетчатки вдоль зубчатой линии во всех квадрантах глазного дна и преретинальные экссудаты (фиг. 3), что позволило подтвердить правильность установления диагноза заболевания и назначить соответствующее патогенетическое и симптоматическое лечение.Based on the data obtained, the patient was diagnosed with chronic peripheral uveitis and underwent ophthalmoscopy with sclerocompression. Пациент В., 27 лет. В анамнезе миопия средней степени обоих глаз, в 2012 г. - барьерная периферическая лазеркоагуляция сетчатки обоих глаз, кераторефракционная операция (LASIK) обоих глаз. Острота зрения обоих глаз на момент обращения 1,0. При обследовании по разработанному способу выявлено:Patient B., 27 years old. A history of moderate degree myopia of both eyes, in 2012 - barrier peripheral laser coagulation of the retina of both eyes, keratorefractive surgery (LASIK) of both eyes. The visual acuity of both eyes at the time of treatment is 1.0. When examining the developed method revealed:
- в структуре жалоб - «затуманивание» зрения, покраснение глаз, чувство жжения, «мушки» в поле зрения;- in the structure of complaints - "blurring" of vision, redness of the eyes, a burning sensation, "flies" in the field of vision;
- биомикроскопические данные - помутнение периферических отделов хрусталика, неравномерный мидриаз (фиг. 4), пастозность нижней переходной складки конъюнктивы, мелкие кисты и кальцинаты тарзальной конъюнктивы (фиг. 5);- biomicroscopic data - clouding of the peripheral parts of the lens, uneven mydriasis (Fig. 4), pastiness of the lower transitional fold of the conjunctiva, small cysts and calcifications of the tarsal conjunctiva (Fig. 5);
- со стороны центральных отделов глазного дна диск зрительного нерва несколько гипермирован, границы четкие, вдоль экватора - сформированные лазеркоагуляты I-II степени.- from the side of the central parts of the fundus of the optic nerve, the disk of the optic nerve is somewhat hypermediated, the borders are clear, along the equator there are formed laser coagulates of the I-II degree.
На основании полученных данных у пациента был диагностирован хронический периферический увеит и проведена офтальмоскопия со склерокомпрессией. В ходе данного исследования выявлен выраженный диффузный отек сетчатки вдоль зубчатой линии во всех квадрантах глазного дна и обширные преретинальные экссудаты, что позволило подтвердить правильность выставленного диагноза хронического периферического увеита и назначить соответствующее противовоспалительное лечение.Based on the obtained data, the patient was diagnosed with chronic peripheral uveitis and underwent ophthalmoscopy with sclerocompression. Таким образом, результаты проведенного клинического исследования свидетельствуют об эффективности применения способа скрининговой диагностики хронического периферического увеита в ходе обследования пациентов на амбулаторном этапе оказания офтальмологической помощи. Thus, the results of the clinical study indicate the effectiveness of the method of screening diagnosis of chronic peripheral uveitis during the examination of patients at the outpatient stage of ophthalmic care.
