Abstract
Description
The invention relates to medicine, namely to orthopedic dentistry, and can be used to determine the chewing effectiveness of patients.
2012.12.30).A known method of determining chewing effectiveness using a computer program for analyzing particles of chewed tablets of polymerized C-silicone impression material in chewing samples (patent No. a 20110391, IPC А61С 19/05, published. 2011.03.31, publ. 2012.12.30).
The disadvantages of this method are: the difficulty of disinfecting a chewed product; loss of material particles during chewing and disinfection.
4.10.2012) (прототип).The closest in technical level and the achieved result is a method for determining chewing effectiveness by colorimetric analysis (Ishikawa et al.) Of a chewed chewing gum that is sensitive to deviations in the acidity of the medium that changes color when chewing (Patent No. US 2012/0253232 A1, IPC A61B 5/11 , claimed. September 30, 2010, published on October 4, 2012) (prototype).
Недостатками данного способа являются: данная жевательная резинка отсутствует в открытом доступе; проведение манипуляции требует наличия дополнительного инструментария (колориметра).The disadvantages of this method are: this chewing gum is not available in the public domain; the manipulation requires the presence of additional tools (colorimeter).
The technical result of the claimed method is the ability to determine the masticatory effectiveness of the patient only taking into account the relief of the occlusal surfaces of the antagonizing teeth in the dental office on an outpatient basis for a minimum time with high accuracy and the ability to save sample results in order to control the patient's masticatory performance in dynamics.
The technical result is achieved by the fact that the method for determining chewing effectiveness includes the evaluation of mixing multi-colored contrasting tasteless unpolymerized masses of elastic impression material of high viscosity based on vinyl polysiloxane. As a result of the act of chewing, the lumps of test material are chewed and moved, resulting in the mutual penetration of vinyl polysiloxane masses of different colors, originally placed on the chewing surfaces of the teeth of the right and left side groups, into each other, which characterizes the process of mixing them. The higher the degree of mixing of the material, the higher the chewing efficiency of the patient.
The proposed method for determining chewing effectiveness is carried out in the following sequence.
1).The patient is invited to chew multicolored contrasting tasteless unpolymerized masses of highly viscous elastic impression material based on vinyl polysiloxane, polymerized by the type of addition reaction, formed in the form of 1.5 cm 3 balls and placed on the right and left groups of chewing teeth of the lower dentition for 20 chewing movements . Then, the two-color elastic mass mixed during chewing is crushed between two glass plates to a thickness of 3 mm to increase the area and eliminate traces of chewing, photographing from two sides with the lighting of the dental office installed and each photo examined using the Adobe Photoshop CS6 computer program on an RGB scale for determining the quality of mixing masses of various colors (Fig. 1).
A raster image of the mixed in the oral cavity polyvinylsiloxane bicolor mass in RGB mode is obtained. The obtained raster images of the test material characterize the degree of mutual penetration of multi-colored polyvinylsiloxane masses (white and sand) with respect to their areas on each surface of the mass of chewing material crushed between glass plates.
Pre-made standards of chewing material of various colors (white and sand), the mixing of which will be carried out during chewing, of vinyl polysiloxane impression material "Bisico S1". The standards should have the same size, and the photographic fixation of the standards should be carried out under the same cabinet lighting as the mixed test material.
To evaluate the mixing of materials of different colors in the RGB mode, the “color row” function of the computer program Adobe Photoshop CS6 is used (Schlicht G.Yu. Digital processing of color images.- M., Ekom Publishing House, 1997. S. 9-20). The color tone values of the raster image of each of the two standards are determined. Measurement of color tones of standards is carried out to obtain a sequence of color characteristics. The starting point of the diagnostic scale is the value of the color tone of the image of the standard of one color (white) on the RGB scale, the second point of the diagnostic color scale is the value of the RGB image of the standard of another color (sand). Using the “color series” function, on the diagnostic color scale, a section is selected that characterizes the specified color of the standard according to RGB values, according to which points are formed and color-labeled on the raster image of each of the two surfaces of the analyzed chewing material, forming the perimeters of mutually penetrating each other when mixing different in color vinyl polysiloxane masses of test material, according to which the quantitative characteristics of the areas of multi-colored masses mixed with vania. According to the percentage of these areas obtained, chewing effectiveness is estimated for each patient and for the aggregate group of subjects.
A high viscosity elastic impression material based on vinyl polysiloxane was chosen as a chewing material, which is due to the low initial hardness on the Shore scale, which allows us to take into account only the interaction of occlusal surfaces of antagonizing teeth when determining chewing efficiency without taking into account the chewing pressure force component. The need to take into account only the relief of the occlusal surfaces of antagonizing teeth is due to the circadian dynamics of the functional indicators of the chewing link of the dentoalveolar system associated with the chronotype and mood during the day, while the relief of the occlusal surfaces is constant.
The insignificant effort developed when chewing the silicone mass allows the use of this material in individuals with removable dentures. Due to the fact that the material is not polymerized and elastic, there is no loss of particles of the substance during chewing, and after chewing it by the patient, the material is easily disinfected for subsequent storage for the period of treatment of the patient.
The analysis of the prior art, including a search by patent and scientific and technical sources of information, allowed to establish that analogues, characterized by signs identical to all the essential features of the claimed invention, were not identified. The definition from the list of identified analogues of the prototype, as the closest solution for the totality of features, allowed to identify the set of essential distinguishing features in relation to the applicant’s technical result in the claimed object set forth in the claims.
Given the foregoing, the proposed method for determining chewing effectiveness meets the requirements of "novelty" and "inventive step".
Example. A method for determining chewing effectiveness is carried out using a dynamic chewing test using test material. To determine the chewing effectiveness, a vinyl polysiloxane silicone impression material (Bisico S1) was used, formed in the form of 1.5 cm 3 colored beads of various colors. Properties of the test material, such as low hardness on the Shore scale, allow only the relief of the occlusal surfaces of antagonizing teeth to be taken into account without regard to the component of the chewing pressure force. To take into account the effectiveness of chewing on both sides of the dentition, balls of test material of various colors are placed on the right (sand material) and left (white material) side sections of the lower dentition. Then the patient performs 20 chewing movements in the usual way. The number of chewing movements is not reported to the subject, their dentist calculates them. The resulting chewed mass is removed from the oral cavity, crushed between two glass plates to a thickness of 3 mm, after which they are photographed and analyzed in an Adobe Photoshope CS6 computer program on an RGB scale.
- М. ).To determine statistically reliable data, a study was conducted in the dynamics of masticatory effectiveness of 60 people aged 18-25 years, divided into two groups by gender, with intact dentition with an orthognathic bite with a predominantly right-handed type of chewing, which allowed us to identify the average value of mixing of the material maximum efficiency of chewing patients. Thus, the average value of the effectiveness of chewing, for both male and female, corresponds to 54.6% of the mixing of test material. Moreover, for male patients, the interquartile range is in the range from 36.1% to 73.3% of the mixing of chewing material, for female patients - from 41.4% to 67.8% of the mixing (Fig. 2), which is taken as reference values of the effectiveness of chewing (A. Kishkun, Manual of laboratory diagnostic methods. - M.: GEOTAR-Media, 2007).
