Проводилась комплексная медикаментозная стандартная терапия, включая антигипертензивные, антитромботические, гиполипидемические и нейропротективные средства. Осуществлялся комплекс активных реабилитационных мероприятий. Больной С., 69 лет, перенес ишемический инсульт с образованием инфаркта мозга в бассейне правой средней мозговой артерии. При поступлении имелось выраженное ограничение повседневной активности и умеренно выраженный неврологический дефицит (mRS 5 баллов, NIHSS 8 баллов). По данным кардиореспираторного мониторирования, выполненного на вторые сутки инсульта, во время ночного сна ИАГ составил 24 события/час, что свидетельствовало в пользу наличия у больного НДС тяжелой степени. Проводилась комплексная медикаментозная стандартная терапия, включая антигипертензивные, антитромботические, гиполипидемические и нейропротективные средства. Наряду с лекарственной терапией больному в течение последующих 7 дней в период ночного сна с 22.00 до 7.00 придавали возвышенное положение голове. То есть поднимали головной конец кровати на 30° и одновременно проводили непрерывную инсуффляцию кислорода через носовую канюлю со скоростью 2-4 л/мин с поддержанием уровня тканевой сатурации не менее 95%. Тканевую сатурацию контролировали с помощью пальцевого пульсоксиметра.
