Abstract
Description
The invention relates to medical equipment and is intended for orthosis of children and adolescents with various pathologies of the ankle joints with a predominant increase in muscle tone, including those arising from cerebral palsy (cerebral palsy), the consequences of traumatic brain injury (TBI), encephalitis, early organic damage to the nervous system system (CNS), with congenital malformations of the brain, extrapyramidal disorders of various origins.
Known apparatus orthopedic on the ankle joint for children with increased muscle tone (RF patent No. 2320299), in which the sleeve of the leg and sleeve of the foot are interconnected using polymer elastic ankle joints on the inside or (and) the outside. However, this unit orthopedic to the ankle joint under sharp dynamic loads often loses the stability of elastic ankle joints (of the design presented) at the time of the front push when the heel of the affected leg approaches the supporting surface. In this case, the sleeve of the leg tends to move forward relative to the sleeve of the foot, which causes pain and additional trauma in the ankle joint. In addition, in this unit orthopedic to the ankle joint, due to the selected configuration of the elastic joints and their small volume, the moment of elastic forces is insufficient and often not able to affect both the equinus installation of the foot during its transfer and the prevention of a hard impact on the joint in phase of the front push, that is, when placing the foot with the heel on the supporting surface. The shin shells in the known orthopedic apparatus for the ankle joint are not rigid enough in the region of the posterior wall of the sleeve, which creates insufficient discharge of the affected limb and does not help to prevent a hard impact on the joint in the forward push phase.
In the known apparatus orthopedic on the ankle joint, the sleeve of the lower leg and foot are made of polymer sheet thermoplastics. This creates a thick-walled shell, a sufficiently large weight of the apparatus, insufficient strength in the places of installation of the ankle joints and fasteners, contributes to the greenhouse effect. In addition, the sleeve of the foot does not recover energy and does not contribute to an increase in repulsion when walking in the back shock phase. The uniformity of the tibia sleeve in the known orthopedic apparatus on the ankle joint, if necessary, to ensure its certain stiffness, makes it difficult to remove and put the orthopedic apparatus on the ankle joint on the patient's limb.
The technical solution is to create an orthopedic apparatus for the ankle joint, which allows to increase the reliability and comfort of its socks by enhancing the strength characteristics of the foot sleeve and lower leg sleeve in the area of its back wall and the joints with hinges and fastening elements of the sleeves, to create a multi-strength design that does not interfere with the removal and putting on the device; ensuring the soreness of the front section of the foot sleeve, which increases energy recovery and enhances the back push in the phase of repulsion of the affected limb during walking, reduce heat exposure and reduce perspiration - the so-called greenhouse effect - by rejecting sheet plastic sleeves, strengthen the strength and multi-cycle characteristics of elastic polymer hinge due to the use of an ankle hinge in the form of a four-petalled spatial figure (“butterfly hinge”), which will allow for the study of pathologies of the ankle joint in a wide range and in a short time, as well as to shorten the postoperative, recovery and rehabilitation periods.
The task is achieved by performing shin and foot sleeves from composite material, in which a rational ratio of the physicomechanical properties of different sections of the sleeves is achieved by enhancing the rigidity of the back wall of the shin and foot sleeves in the metatarsophalangeal roll due to the additional laying of carbon tapes and fabric from synthetic high-strength thread for example, "Rusar", which will prevent shock on the joint in the phase of the front push, increase the stability of the legs in the apparatus, t to increase comfort in the process of wearing the device, will provide ease of putting on and removing the device from the limb, will provide energy recovery and amplification of the back push when walking, the reliability of the installation of fasteners for sleeves and hinges; composite material will reduce the effect of the greenhouse effect, thereby preventing possible allergic reactions; the stability of the legs in such an apparatus and its reliability in the toe is achieved by using a polymer elastic hinge in the form of a four-petalled spatial original figure with sections of different thicknesses, increased tensile strength and multi-cycle loads. An increase in the strength of the polymer elastic joint in the region of the lobes is achieved by interspersing nanosized particles, for example, aluminum oxide, or by adding pulp from a high-strength synthetic thread to the polymer during molding. Such additives, on the one hand, increase the reliability of fastening the shin and foot sleeves with a polymer elastic hinge, on the other hand, maintain the elasticity of the material in the ankle region, providing movement in the sagittal plane and some rotational movements in the horizontal plane, which will expand the indications for the appointment orthopedic apparatus for ankle joint, the use of a new apparatus with a wider range of diseases of the ankle joint, reduction of postoperative, recovery of rehabilitation and treatment periods.
1).The orthopedic apparatus for the ankle joint consists of the lower leg sleeve pos. 1, foot sleeves pos. 2, connected by internal and external polymer elastic ankle joints located on both sides of the apparatus pos. 3, fasteners of the elastic hinge to the legs of the lower leg and foot pos. 4, back stop pos. 5, 6, elements for fastening sleeves made of textile fastener “Contact” pos. 7 and rivet connection pos. 8 (Fig. 1).
Sleeves of the lower leg and foot on both sides, both on the outer and inner sides, are connected by elastic ankle joints of the same design.
Figure 1 shows only the external elastic ankle joint.
Sleeves of the lower leg and foot of the orthopedic ankle joint are made according to individual casts.
In the manufacturing process of the composite material for the liner of the foot and lower leg, a carbon tape was used as a matrix. The use of carbon tape, instead of carbon fabric, contributed to an increase in labor productivity by 1.5-1.8 times, since in this case operations such as cutting the fabric into strips, treating the edges of the fabric with an adhesive to avoid shedding of threads, and drying the strips of fabric are excluded after applying glue.
In the sleeve of the foot pos. 2 (Fig. 2a) carbon tape pos. 9 and pos. 10 and an insert made of fabric based on synthetic high-strength yarn pos. 11 are laid along the longitudinal axis of the foot sleeve to the axis of the metatarsophalangeal roll and in the areas of connection with the petals of the ankle joints. To reinforce the posterior wall of the lower leg sleeve, pos. 1 (Fig. 2b), two carbon tapes pos. 12 and pos. 13, with one carbon tape pos. 13 is laid from the upper edge of the calf sleeve on the right side, envelops the rear region of the sleeve to the lower left side of the calf sleeve, and the other carbon tape pos. 12 is laid from the upper left side of the tibia sleeve, envelops the posterior region of the sleeve, overlaps the first tape behind (lap) to the lower right side of the tibia sleeve.
In the places of attachment pos. 8 textile pile fasteners "Contact" pos. 7 (Fig. 1) with a sleeve of the lower leg and fasteners of the polymer elastic hinge to the lower leg sleeve pos. 4 fabric inserts based on synthetic high-strength yarn pos. 11, as shown in FIG. 2b, which ensures the reliability of the fastening of polymer elastic joints and fastening elements of sleeves during operation of the apparatus.
The manufacture of the tibia and foot sleeves from a composite material of a new structure with an additional arrangement of carbon ribbons and fabric from synthetic high-strength yarn made it possible to increase the hardness of the back wall of the liner and the sole of the foot, and to ensure the reliability of the hinges with the sleeves of the foot and lower leg of the orthopedic apparatus.
4.In order to increase the strength characteristics of polymer elastic ankle joints, increase reliability and comfort, provide an increase in the moment of elastic forces in the process of wearing an orthopedic apparatus on the ankle joint, a polymer elastic ankle joint, which is made in the form of a four-petalled spatial figure, is invented. 3 (Fig. 3), the inner surface of which is flat, and the outer one is made in the form of a convex ellipsoidal base, pos. 15 in the middle of the figure, with the upper poses. 16 and lower pos. 17 with rounded “petals” pads located at angles α = 45 ° and β = 110 ° to the axis of symmetry of the figure in the coordinate system X, Y, respectively, as shown in FIG. 3, and located symmetrically relative to the ordinate axis, with each of the petal pads has an opening pos. 14 for fastening the hinge to the legs of the lower leg and foot using the fasteners pos. four.
1.The hinge is attached by one (upper and lower) petals to the lower part of the shin sleeve, and other (upper and lower) petals to the upper part of the foot sleeve, as shown in FIG. one.
In order to increase the strength of the polymer product, in the process of manufacturing the hinges, nanosized particles, for example, aluminum oxide in the amount of 2-3% by weight of the polymer, were introduced into the composition of the polymer composition. The strength characteristics of the hinge increased by 1.5-2.0 times. Studies have shown that the distribution of alumina particles in the polymer composition plays a significant role. With an increased content of nanosized particles in the petals (2 times more) compared with the central elliptical part of the hinge, its strength characteristics increase. For example, in the process of testing for multi-cycle characteristics, the hinge withstood 1,500 thousand cycles until cracks appeared. During wear, the hinge must withstand an average of 250 thousand cycles, given that the child wears the device on average only 3-4 hours a day and takes 10,000 steps per day, and the service life of the orthopedic product should be at least 6 months.
Thus, the margin of safety of the ankle joint ensures the reliability of the orthopedic apparatus during its operation.
A similar effect is achieved if, during the manufacturing of the polymer elastic hinge, pulp from a synthetic high-strength thread is added to the polymer composition. The pulp is distributed evenly throughout the entire volume of equipment by thorough mixing. The quantitative ratio of pulp and polymer composition is 0.5: 10. With this ratio, the addition of pulp does not separate the polymer, but hardens, increasing the strength characteristics of the polymer ankle joint by 1.5-2.0 times.
The invented design of the elastic ankle joint provides sufficient elasticity and allows the foot to be rotated to the required angle, while the apparatus fits the leg tightly, which makes the patient comfortable to wear the apparatus. Fastening the hinge with the petals to the walls of the shin and foot sleeves ensures the stability and reliability of the orthopedic product and guarantees the shelf life of the device even under heavy loads that can be created in connection with the use of the device on the ankle joint by children with consequences of cerebral palsy from 12 to 14 years.
In order to reduce shock loads on the affected limb when walking in the apparatus while resting on the heel, accumulating energy and creating an additional moment of bending force in the ankle joint in the sagittal plane, an emphasis was made in the apparatus on the back wall of the calf sleeve in the form of a cone made of a polymer composition, which enters the support platform located on the sleeve of the foot. In this case, the arising moments of strength in a person are normal, that is, without pathologies in the ankle joint.
Thus, the use in medical practice of the apparatus presented on the ankle joint, the design of which includes sleeves made of composite material with additional laying both in the sleeve of the foot, and in the sleeve of the lower leg of carbon years and high-strength fabric, as well as the use of an emphasis in the form of a cone from a polymer composition included in the support platform, elastic ankle hinges of the original design in the form of a four-petalled spatial figure with an increase in strength characteristics due to the introduction of nano measuring particles or pulp made of synthetic high-strength yarn, the technical task is achieved as much as possible - increasing the reliability and comfort of the apparatus during wear, ensuring the spar of the front part of the foot sleeve that increases energy recovery and enhances the back push in the phase of repulsion of the affected limb during walking, reducing heat exposure and decreased sweating, the so-called greenhouse effect. The listed advantages of the present invention allow the treatment of pathologies of the ankle joint in a wide range and in a short time, as well as to shorten the postoperative, recovery and rehabilitation periods and thereby increase the socio-economic effect of reducing the patient's stay in hospitals, rehabilitation centers and other medical institutions.
To verify the reliability of the advantages of the new invention indicated in the text - an orthopedic apparatus for the ankle joint, prototypes of devices in the amount of 15 pieces were made. Polymer hinges were made using nanosized alumina particles. Field tests took 6 months. As a result of tests and a survey of patients and their parents (children from 6 to 14 years old), it was found that the children were comfortable in the devices. They noted a reduced mass of the apparatus. The leg was in the required position recommended by the doctor, the arising moment of elastic forces influenced the equine position of the foot during its transfer and in the phase of the back push. Patients noted the “lightness” of pushing off the sore leg from the support. In the process of wearing the apparatus, a hard blow to the joint was not observed in the phase of the front push, that is, when the foot was placed with the heel on the supporting surface. The patient's leg in the apparatus was in a stable position; naminov, abrasions, redness on the skin of patients was not observed. Throughout the test period (6 months), no noticeable defects, cracks, delaminations, chips, etc. were observed either on the sleeves or on the polymer elastic ankle joints. The tests were satisfactory: the temporal structure of the step, the cosmeticity of the gait and other biomechanical characteristics of the walk improved; 12 patients reduced recovery time.
