Abstract
Description
The invention relates to medicine, namely to psychiatry, in the treatment of moderate cognitive decline.
A moderate cognitive decline is a symptom complex observed in lesions of the central nervous system that are different in etiology. In the Russian Federation, the leading causes of its development are cerebrovascular disease, a combination of cerebrovascular disease and Alzheimer's type neurodegeneration, Alzheimer's type neurodegeneration, a combination of chronic alcohol intoxication and cerebrovascular disease or traumatic brain disease. It is often difficult to determine the etiology of cognitive decline in a particular patient due to the combined nature of brain damage and the relative nosological non-specificity of both clinical and neuroimaging data. Accordingly, the appointment of etiotropic therapy focused on a specific damaging factor is also difficult.
Most often, a moderate cognitive decline occurs in people over 65 years of age, making up 30-90% of the corresponding population groups, with increasing frequency depending on age. As a rule, in individuals with this condition, multiple background somatic pathology is noted, requiring active drug therapy. The medical and social significance of a moderate cognitive decline is determined by the fact that it with a probability of 10-15% per year passes into dementia, a severe progressive mental disorder that causes irreparable losses for the patient and his relatives.
According to modern concepts, regardless of the etiology of damage to the central nervous system in a particular patient, the main mechanisms for the development of cognitive disorders are impaired neuronal metabolism, neurotoxicity of various exogenous and endogenous agents, oxidative stress, impaired cerebral circulation and microcirculation. In particular, systemic metabolism disorders, such as hyperhomocysteinemia, insulin resistance, hyperglycemia, hypercholesterolemia, hyperlipidemia, make a significant contribution to the development of cognitive decline.
A method is known from the prior art for treating moderate cognitive decline with ipidacrine, which belongs to the group of acetylcholinesterase inhibitors (Instructions for use of the drug for medical use P N014238 / 01-160311 from the website http://grls.rosminzdrav.ru/). The disadvantage of this method is a significant number of contraindications for the use of ipidacrine, which include widespread diseases: epilepsy, extrapyramidal diseases with hyperkinesis, angina pectoris, severe bradycardia, bronchial asthma, obstruction of the intestine or urinary tract, vestibular disorders, gastric ulcer and duodenal ulcer . Up to 10% of patients can have side effects in the form of nausea, salivation, palpitations, diarrhea, bradycardia, bronchospasm, bronchial hypersecretion, convulsive conditions. In addition, ipidacrine does not have any additional beneficial effect on the body, in addition to the main therapeutic effect. Since a moderate cognitive decline is most often found in elderly and senile patients with multiple somatic pathologies, the use of the method increases the number of drugs taken simultaneously by patients, which is associated with the risk of unpredictable drug interactions.
There is a method of treating moderate cognitive decline with memantine, belonging to the group of N-methyl-D-aspartate receptor antagonists (V.V. Zakharov. Evolution of cognitive deficit: mild and moderate cognitive impairment. Neurology, neuropsychiatry, psychosomatics. 2012, No. 2, p. . 16-21). The disadvantage of this method is the small evidence base for the effectiveness of the drug with moderate cognitive decline, since in randomized controlled trials, the effectiveness of memantine is shown only for moderate or severe Alzheimer's type dementia, which is the official indication for its use. As with the previous method, in this case, the number of drugs simultaneously taken by the patient increases, which is associated with the risk of unpredictable drug interactions.
A known method of enhancing cognitive function and the treatment of cognitive decline in humans and companion animals (RU 2507742 C2), based on the administration of pharmaceutical compositions comprising one or more long chain polyunsaturated fatty acids, one or more compounds releasing nitrogen monoxide, and one or more medium chain triglycerides. The disadvantage of this method is the lack of empirical evidence of its effectiveness. In addition, the proposed pharmaceutical composition will even theoretically be effective only with a moderate vascular cognitive decline, but not with a moderate cognitive decline of a dismetabolic or degenerative nature, since it is a combination of an antiatherosclerotic agent and a vasodilator.
A known method of treating cognitive disorders based on the administration of a pharmaceutical composition comprising a partial nicotinic receptor agonist and a centrally acting acetylcholinesterase inhibitor rivastigmine or donepezil (RU 2493851). The disadvantages of the method are a significant number of restrictions on the use of acetycholinesterase inhibitors (sinus node syndrome, atrioventricular and sinoatrial blockade, exacerbation of gastric ulcer and duodenal ulcer, urinary tract obstruction, convulsive syndrome, bronchial asthma, bronchial obstruction), high incidence of side effects 50% of patients), the possibility of their potentiation while prescribing a nicotinic receptor agonist. In this case, a patient with a moderate cognitive decline, who most often has multiple background somatic pathology, is additionally prescribed not one but two drugs, which further increases the risk of unpredictable drug interactions.
A known method of treating cognitive disorders based on the first compound, a selective 5-HT6 receptor antagonist and the second compound, a 5-HT2A receptor selective antagonist, where the first compound has an affinity for 5-HT6 receptor with a pKi value greater than or equal to 8 , and the second compound has an affinity for the 5-HT2A receptor with a pKi value greater than or equal to 8 (RU 2420318 C2). The disadvantage of this method is the lack of empirical evidence of its effectiveness. As in the previous case, a patient with a moderate cognitive decline, most often having multiple background somatic pathology, is additionally prescribed not one, but two drugs, which further increases the risk of unpredictable drug interactions.
A known method for the treatment of cognitive impairment, based on the introduction of human albumin in the form of a solution for plasma filtration (plasmapheresis) - RU 2414925 C2. The disadvantage of this method is the use of a drug obtained from human blood, which is associated with a risk of infection with parenteral infections (HIV, viral hepatitis B, viral hepatitis C). Given that cognitive decline alone is not a threat to the patient’s life, the risk of using this method is likely to exceed its potential benefit.
A known method of treating cognitive disorders based on the intake of one or more chemical compounds that block the neurotoxicity of ADDL protein (RU 2012120904 A). The disadvantages of the method are: 1) a narrowly targeted effect on one of the possible mechanisms for the development of cognitive disorders, the clinical significance of which is not clear at the current level of knowledge; 2) lack of clinical evidence of effectiveness; 3) as in a number of previous cases - an increase in the number of drugs taken at the same time by patients, which is associated with the risk of unpredictable drug interactions.
There is a method of treating cognitive dysfunction in patients with arterial hypertension with metabolic syndrome, which consists in taking a combination of carvedilol 50 mg / day and amlodipine 5-10 mg / day for 6 months (RU 2012116674 A). The disadvantage of this method is the impossibility of its use in patients with cognitive dysfunction with normal or low blood pressure, because the proposed drugs cause a sufficiently pronounced decrease in blood pressure, which is favorable only for patients with arterial hypertension.
Known methods for the treatment of cognitive impairment are the introduction of a patient (including intracranially) one or more inflammatory mediators or their functionally active fragments or polynucleotides encoding one or more inflammatory mediators and able to be expressed in human nerve cells (RU 2011108560 A). The disadvantages of these methods are: 1) a narrow effect on one of the possible mechanisms for the development of cognitive disorders (cerebral amyloidosis), the clinical significance of which is not clear at the current level of knowledge; 2) lack of clinical evidence of effectiveness; 3) the highly invasive nature of part of the proposed interventions (modification of the genome of functioning neurons, intracranial infusion); 4) low safety of the inflammatory mediators themselves, when used according to other indications, such side effects as influenza-like condition, seizures, depression, suicidal attempts, exacerbation or occurrence of psychotic symptoms are noted.
Thus, the following disadvantages are characteristic of most known methods of treating moderate cognitive decline: 1) an increase in the number of drugs taken by the patient at the same time, which is associated with the risk of unpredictable drug interactions; 2) a narrowly targeted effect on one of the possible mechanisms for the development of cognitive decline or on cognitive decline only in a certain group of patients; 3) an unknown or knowingly low level of safety of the proposed medicines and treatment methods.
Известно применение Тиоктовой кислоты при диабетической и алкогольной полинейропатии, жировой дистрофии, циррозе печени, хроническом гепатите, интоксикации (Инструкция по применению лекарственного препарата для медицинского применения Октолипен®, № ЛП 001863-280912, c сайта http://grls.rosminzdrav.ru/).The use of thioctic acid in cases of diabetic and alcoholic polyneuropathy, fatty degeneration, cirrhosis of the liver, chronic hepatitis, intoxication is known (Instructions for the use of the drug for medical use Oktolipen®, No. ЛП 001863-280912, from the website http://grls.rosminzdrav.ru/ )
Information on the effectiveness of the use of thioctic acid for the treatment of moderate cognitive decline in the prior art is not found.
Effect: improvement of cognitive functions of patients who initially had symptoms of moderate cognitive decline.
The technical result is achieved as follows. A patient with a detected moderate cognitive decline is prescribed thioctic acid, synonyms: lipoic acid, alpha-lipoic acid, thioctacid - at a dose of 600 mg / day orally on an empty stomach 30 minutes before meals, in 1-3 doses for at least 3 months.
The proposed method for the treatment of moderate cognitive decline was used in 30 patients aged 65-74 years, including 22 women, 8 men. Moderate cognitive decline in 8 patients was due to cerebrovascular disease, in 9 patients to a combination of cerebrovascular disease and Alzheimer's type neurodegeneration, in 8 patients to a combination of cerebrovascular disease and dysmetabolic encephalopathy due to type 2 diabetes, in 5 patients to a combination of cerebrovascular disease and chronic in the anamnesis. The examined patients lacked clinical and anamnestic data on any mental and behavioral disorders, except for moderate cognitive decline. Acute CNS lesions, in particular acute cerebrovascular accident, traumatic brain injury, hypo- or hyperglycemic coma, with a possible spontaneous improvement in cognitive functions in the recovery period, have not been recorded over the past 12 months.
A moderate cognitive decline was identified on the basis of: 1) patient complaints about memory impairment for current events, difficulties in acquiring new skills or using recently acquired skills, in some cases - difficulties in orientation in space; 2) the data of the clinical examination by a psychiatrist: memory loss for recent events revealed in the conversation, confusion in the dating of life events while maintaining the ability to present them in the correct sequence, inaccurate orientation in time with a safe orientation in place and personality, thoroughness and rigidity of thinking, difficulties in establishing causal relationships and understanding more complex logical and grammatical relationships; 3) neuropsychological testing on the Short Scale for assessing the mental status of MMSE. The average score for cognitive function assessment on the MMSE scale before treatment in patients was 25.2 with fluctuations within 0.2 points, which meets international criteria for moderate, pre-dementia, cognitive impairment. According to clinical and anamnestic data, not a single patient had data for dementia, as evidenced by the safety of home adaptation, social adaptation in a familiar environment, the ability to live independently, prudently manage their finances, and most of the previously acquired professional skills.
It should be noted that all examined patients received from 2 to 5 drugs for the treatment of various concomitant diseases. Most often these were antihypertensive drugs, antiplatelet agents, antianginal drugs, oral hypoglycemic drugs, diuretics.
Patients received thioctic acid in the form of a prolonged oral dosage form of 600 mg once a day in the morning on an empty stomach 30 minutes before meals for 3 months.
Upon repeated examination after 3 months, subjectively, patients noted an improvement in memory for current events, an improvement in concentration, and "clarification" in the head. The average score for repeated testing on the MMSE scale was 26.3 with fluctuations within 0.3 points, the difference with the initial level was statistically significant (p <0.01). Side effects of thioctic acid, undesirable drug interactions were noted.
It should be noted that at present, none of the preparations of thioctic acid, according to official instructions from the website http://grls.rosminzdrav.ru/, is intended for use with moderate cognitive decline or any subtype of it. Indications for the use of these drugs are diabetic polyneuropathy and alcoholic polyneuropathy. Studies on the use of thioctic acid with moderate cognitive decline have not been found in the available literature. This indicates the novelty of the proposed method for the treatment of moderate cognitive decline.
