Abstract
Description
The invention relates to medicine, namely to surgery, and can be used to treat adhesive disease.
Currently, the problem of adhesive disease of the peritoneum is the most urgent in surgery. Joints after surgical interventions in the abdominal cavity occur in about 80-90% of cases. Adhesive disease of the peritoneum in 50-75% of cases ends with the development of complications - acute adhesive intestinal obstruction, requiring repeated surgical treatment. Relapse of adhesive disease can reach 30-60%, exacerbating the condition of patients, leading to deaths in 13-55% of cases.
Currently, the means and methods for the treatment and prevention of adhesive disease are based on the local action of drugs by injecting them into the abdominal cavity after operations through established irrigators, creating hydroperitoneum, also using physiotherapeutic apparatuses through the abdominal wall, massage the abdominal wall, physiotherapy exercises, managing the adhesive process in order to preserve the passage of intestinal contents in the lumen.
A known method of treating adhesive disease (Patent RU No. 2177741, authors Bogomolov N.I. and Naydetskaya M.S.), where mobilization, probing of the intestine, laying in a horizontal position with fixation by microirrigators, followed by the introduction of medicinal substances. This method involves repeated mechanical damage to the peritoneum, leading to a relapse of adhesive disease. Also known is a method for the prevention of adhesion disease "Isonides" (Patent RU No. 97314), in which a 1% solution of isoniazid in hemodesis is introduced into the abdominal cavity after surgery. Isoniazid has a toxic effect on the body.
The prototype is taken (Patent RU No. 2250109, author Portenko Yu. G.) a method for treating adhesive disease of the abdominal cavity in children, where laparoscopic separation of adhesions, electrophoresis by proteolytic enzymes on the anterior abdominal wall, and the appointment of probiotics and sorbents before surgical treatment inside to correct homeostasis are performed .
The disadvantage of this method is the lack of effectiveness of probiotics and sorbents for the correction of disorders that occur in the human body with adhesive disease of the peritoneum.
An agent for the treatment of adhesive disease is aimed at eliminating the changes that occur with adhesive disease of the peritoneum. Water extraction from rhizomes of calamus swamp, St. John's wort perforated and licorice roots naked, obtained from rhizomes of calamus swamp, St. John's wort perforated, licorice roots, drinking water, taken at the following content, wt.%: Calamus rhizome bore 1.0, grass St. John's wort 1.0, licorice roots bare 1.0, drinking water - the rest. Water extraction is obtained by boiling for 5 minutes. The resulting extract is taken orally 1-2-3 times a day, depending on the condition of the patient. Surgical interventions are preceded by pathological changes that require surgical correction, the operation, in turn, is an additional traumatic factor for the peritoneum. It is known that the processes of inflammation, immunity and regeneration are closely related and for proper regeneration, damaged by inflammation, mechanical trauma to the peritoneum, it is necessary to affect all parts of the pathogenesis - inflammation, disorders in the immune system, regeneration. The components of the product contain a complex of biologically active substances that exhibit anti-inflammatory effects, eliminate intoxication, regulate regeneration, immunity, eliminate intestinal dysbiosis, hepatoprotective, nephroprotective, and regulating effect on connective tissue. Swamp air inhibits the processes of fermentation in the intestine, gas formation, normalizes the absorption of gases in the intestine, decreasing the intestinal pressure, since with an increase in pressure in the intestine more than 30 mm of water. Art. critical for microcirculation of the intestinal wall - ischemia occurs, impaired regeneration of connective tissue - adhesions. Air marsh stimulates intestinal motility, reducing the contact time of the inflamed intestinal walls. Hypericum perforatum has a strong anti-inflammatory effect, the regulator is a regeneration stimulator due to the content of tannins, forms a film on the intestinal wall, reducing the absorption of toxins. Naked licorice regulates changes in the endocrine system, also has a strong anti-inflammatory effect. When using this extract, the general anti-inflammatory effect, the complex effect on all links of the pathogenesis and causes of adhesions of the peritoneum are enhanced.
Example 1. The patient was admitted to the hospital after receiving an injury - a knife wound of the abdominal cavity, surgical treatment was carried out after preoperative preparation - revision of the abdominal organs, suturing of the postoperative wound tightly. The usual postoperative treatment is infusion, analgesic, antibiotic therapy, dressings. On the third day after the operation - abdominal pain, bloating, nausea, repeated vomiting, lack of gas discharge. Against the background of conservative treatment - no improvement. The operation in volume is relaparotomy along the suture, abundant serous effusion in the abdominal cavity, loops of the intestine are swollen, multiple peritoneal fusion between the loops of the intestine, compressing the lumen, viscerolysis, sanitation and drainage of the abdominal cavity, laparostomy. Remediation after 2 days, suturing of the postoperative wound on the fifth day as the size of the intestinal loops decreases. On the sixth day after surgery, treatment with this extraction was started, 2 hours after the start of treatment, increased peristalsis, flatus, less abdominal pain, on the seventh day decreased abdomen in volume, increased appetite, and improved general condition. On the third day after the start of treatment, the patient was transferred to a general ward. Discharged as wounds heal. During the outpatient observation for three years, there were no complaints characteristic of adhesive disease.
Example 2. The patient, born in 1945, was treated with abdominal pain, bloating, lack of appetite, severe general weakness. Surgery for a hernia of the anterior abdominal wall 1 month ago.
