Abstract
Description
The invention relates to medicine, in particular to otorhinolaryngology, physiotherapy, and can be used in the treatment of patients with pathology of the auditory tube and acute otitis media.
Продувание и введение лекарственных средств в евстахиеву трубу производится через катетер (катетеризация), а если это невозможно, то путем резкого повышения воздушного давления в носовой полости (способ Полицера, Вальсальвы).In the implementation of many methods of treatment of acute otitis media, the Eustachian tube is used as an advantageous way for introducing the drug into individual inaccessible spaces formed in the middle ear during inflammation. The patency of the Eustachian tube can be tentatively established by the breakdown with blowing, swallowing and straining. Monitoring is carried out using special instruments - an otoscope and a manometer. The blowing and introduction of drugs into the Eustachian tube is carried out through a catheter (catheterization), and if this is not possible, then by a sharp increase in air pressure in the nasal cavity (method Politser, Valsalva).
A well-known ear catheter for the Eustachian tube according to Hartmann (TP Mchelidze, Otorhinolaryngological Dictionary. 2007, St. Petersburg, p. 123), which is a cylindrical tube 15-18 cm long, one end of which is expanded in the form of a bell, and the other is bent at an angle 140-150 ° and has the shape of a rounded beak. Catheters of various lumen widths from 1 to 3 mm and wall thicknesses from 0.5 to 1 mm are available. For pumping air serves the Policzer’s ear balloon, which holds 200-250 ml of air. This catheter is used as the closest analogue.
A known technique for catheterization of the auditory tube using a catheter for the Eustachian tube according to Hartmann (TP Mchelidze, Otorhinolaryngological Dictionary. 2007, St. Petersburg, p. 123). Caterization is carried out in a seated position of the patient. His head should be leaned against a solid stand in an upright position. Before catheterization, the patient should blow his nose. A preliminary anesthesia of the nasal mucosa is carried out with a solution of lidocaine 10% - 2 ml.
Catheterization technique. Under the control of anterior rhinoscopy, a catheter is inserted with its beak down into the lower nasal passage. Sliding along the bottom of the nasal cavity, the catheter should enter the nasopharynx. Then it is advanced until it contacts the back wall, rotated 90 ° so that its beak is directed to the ear of the opposite side, and pulled towards itself until its beak stumbles on the posterior edge of the septum, and the catheter is rotated 180 ° into the side of the studied ear so that its ring faces the outer-upper corner of the orbit of the catheterized side. Then the beak of the catheter enters the mouth of the Eustachian tube. Without turning the beak of the catheter, you can remove it in contact with the soft palate and then rotate the beak towards the mouth of the Eustachian tube. After that, the tip of the balloon is inserted into the socket of the catheter and air is blown several times, removing the balloon after each injection. The entry of air into the tympanic cavity is controlled through a rubber auditory tube, one end of which is inserted into the patient’s ear, and the other into the doctor’s ear.
However, the known catheter has several disadvantages:
1. The length of 15-18 cm and the angle of inclination of 140-150 ° do not allow the catheter to enter the auditory tube, but only make it possible to install a catheter near the mouth of the auditory tube, in connection with which most of the injected drug does not go into the tympanic cavity through the auditory tube, but enters the esophagus.
2. The material from which the catheter is made is metal, which in some cases can injure the side wall of the nasopharynx or the mouth of the auditory tube, and also cause submucosal emphysema.
In connection with the foregoing, the authors were tasked with developing a catheter devoid of these drawbacks.
Достигаемым при реализации разработанного устройства техническим результатом является оптимизация доставки лекарственного вещества в барабанную полость через слуховую трубу, возможность нагнетания лекарственных средств, а также эвакуации содержимого барабанной полости, проведения лечебных процедур (лекарственный электрофорез).Achieved during the implementation of the developed device, the technical result is the optimization of drug delivery to the tympanic cavity through the auditory tube, the possibility of pumping drugs, as well as evacuating the contents of the tympanic cavity, and conducting medical procedures (drug electrophoresis).
The specified technical result is achieved due to the fact that the catheter for catheterization of the auditory tube with the electrode has an optimal shape that allows you to enter the catheter directly into the auditory tube, and not to the area of the mouth and to inject the drug into the tympanic cavity, as well as to conduct drug electrophoresis, which allows to reduce swelling of the surrounding tissue.
In addition, the material made of soft plastic (silicone rubber) allows you to painlessly catheter the auditory tube without damaging the surrounding tissue and without causing complications such as emphysema of the nasopharynx wall.
В дистальной части катетера на расстоянии 170 мм имеется наклон на 30°, обращенный кверху, на расстоянии 180 мм имеется наклон на 20°, обращенный влево или вправо в зависимости от исследуемого уха (фигура 1).The catheter for catheterization of the auditory tube contains a basis of elastic material - silicone rubber with a shore hardness of A-60. The basis is made in the form of a tube with a thickness of 2-3 mm and a length of 200 mm. In the distal part of the catheter at a distance of 170 mm there is a tilt of 30 °, facing up, at a distance of 180 mm there is a tilt of 20 °, facing left or right, depending on the studied ear (figure 1).

Catheterization using the developed device is as follows.
After local anesthesia with a solution of lidocaine 10% 2 ml and treatment of the mucous membrane of the nasal cavity with adrenaline under the control of an 0 ° endoscope, a catheter is inserted into the nasal cavity for catheterization of the auditory tube. The catheter is advanced until it comes into contact with the posterior wall of the nasopharynx and rotated 90 ° to the side of the studied ear. By pulling the catheter toward itself, the tip of the catheter enters the mouth of the Eustachian tube. After that, the tip of the balloon is inserted into the catheter and air is blown in several times. The entry of air into the tympanic cavity is controlled through a rubber auditory tube, one end of which is inserted into the patient’s ear, and the other into the doctor’s ear. After making sure that the air passes into the tympanic cavity, the doctor can inject the necessary drugs into the catheter and carry out drug electrophoresis.
The use of a catheter was tested in 20 patients with acute otitis media aged 18 to 57 years. At the same time, in order to confirm the passage of the drug substance into the tympanic cavity in 4 patients, the auditory tube was catheterized with the introduction of X-ray contrast material Trazograf in an amount of 2 ml and computed tomography of the temporal bones before and after catheterization. In CT images, after cationization, the presence of an X-ray contrast substance in the tympanic cavity was recorded.
