Сущность предлагаемого изобретения заключается в следующем.The essence of the invention is as follows.
С помощью микрохирургической техники выделяются из окружающих тканей поврежденные концы нерва. После доступа к нерву производится щадящий эпиневролиз. Затем участок поврежденного нерва (1, 2) размещают в нейротрубочку с продольным разрезом (7, 4), которая может быть выполнена из биополимера, коллагена или других биоинертных компонентов. Диаметр нейротрубочки предпочтительно составляет от 0,4 до 5,0 см, длина - от 1,0 см до 15,0 см. Концы нейротрубочки подшивают к эпиневрию (8) таким образом, чтобы максимальное натяжение нерва приходилось на неповрежденный участок нерва, т.е. на расстоянии от 0,5 до 5 см от места повреждения. В случае если наложить шов ближе чем 0,5 см, натяжение в области шва нерва не снимется, а в случае если подшить нейротрубочку дальше чем 5 см, то поврежденные участки нерва приобретают излишнюю подвижность относительно друг друга, тем самым, замедляют процесс восстановление нерва, при этом процент послеоперационных осложнений увеличивается. Кроме того, хирургу будет сложнее сопоставить пучки ввиду отсутствия надежной иммобилизации нерва в нейротрубочке. После фиксации нерва к нейротрубочке соединяют поврежденные участки нерва (1, 2) эпиневральным швом (9). Для удобства идентификации и соединения поврежденных участков нерва между собой в продольный разрез (4) нейротрубочки устанавливают расширители (на фиг. 1 они не показаны), предпочтительно имеющие вид полукруглых скоб, толщиной от 0,01 до 0,5 см, диаметром от 0,5 до 4,0 см, которые разводят стенки нейротрубочки в противоположные стороны, обеспечивая свободный доступ к нерву, расположенному в полости нейротрубочки.With the help of microsurgical technique, damaged ends of a nerve are isolated from surrounding tissues. If the suture is closer than 0.5 cm, the tension in the area of the nerve suture will not be removed, and if the neurotube is hemmed further than 5 cm, the damaged sections of the nerve acquire excessive mobility relative to each other, thereby slowing down the process of nerve repair, while the percentage of postoperative complications increases. In addition, it will be more difficult for the surgeon to match the bundles due to the lack of reliable immobilization of the nerve in the neurotubule. After fixation of the nerve, the damaged sections of the nerve (1, 2) are connected to the neurotubule with an epineural suture (9). For ease of identification and connection of the damaged sections of the nerve with each other, extenders are installed in the longitudinal section (4) of the neurotubule (they are not shown in Fig. 1), preferably having the form of semicircular staples, with a thickness of 0.01 to 0.5 cm, a diameter of 0, 5 to 4.0 cm, which dilute the walls of the neurotubule in opposite directions, providing free access to the nerve located in the cavity of the neurotubule.
