Изобретение относится к медицине, а именно к хирургии и анестезиологии-реаниматологии, и может быть использовано для развития полиорганной недостаточности при первой стадии нарушения функции органов у больных распространенным перитонитом в зависимости от недостаточности энергообмена.The invention relates to medicine, namely to surgery and anesthesiology-resuscitation, and can be used for the development of multiple organ failure in the first stage of organ dysfunction in patients with advanced peritonitis depending on the lack of energy exchange.
В настоящее время существует способ прогнозирования развития полиорганной недостаточности при перитоните, основанный на показателях функционального состояния компонентов системы гемостаза и фибринолиза (Бюллетень сибирской медицины №3, 2010 г., стр. 27-31. Недостатком данного способа является затратность - требуются дополнительная аппаратура и обученные сотрудники для работы на ней (анализатор реологических свойств крови портативного АРП-01 «Меднорд», г. Технический результат: упрощение прогнозирования развития полиорганной недостаточности у больных распространенным перитонитом при высокой точности способа.Effect: simplified prediction of the development of multiple organ failure in patients with advanced peritonitis with high accuracy of the method.
Сущность способа заключается в определении динамики баллов шкалы SOFA (http://www.rusnauka.com/25_PNR_2012/Medecine/7_11) при распространенном перитоните за любые смежные трое суток послеоперационного периода. При наличии первой стадии нарушения функции органов (диссертация Стариковой А.И «Обоснование показаний к релапаротомии и прогнозирование исхода при распространенном перитоните» стр. 60, автореферат - стр. 14) и 4-х и более баллов по шкале SOFA на 1-е сутки, 5 и более - на 2-е сутки, 7 и более баллов - на 3-й сутки исследуемого периода прогнозируют развитие полиорганной недостаточности.The essence of the method is to determine the dynamics of the points of the SOFA scale (http://www.rusnauka.com/25_PNR_2012/Medecine/7_11) with widespread peritonitis for any adjacent three days of the postoperative period. Starikova’s dissertation “Justification of indications for relaparotomy and prediction of outcome with peritonitis widespread” p. 60, abstract - p. 14) and 4 or more points on the SOFA scale on the 1st day , 5 or more - on the 2nd day, 7 or more points - on the 3rd day of the study period, the development of multiple organ failure is predicted.
Способ осуществляется следующим образом. У больного распространенным перитонитом с первой стадией нарушения функции органов (абсолютный прирост баллов шкалы SOFA третьих суток выбранного периода более чем на 100% превышает абсолютный прирост баллов вторых суток) оценивают динамику баллов шкалы SOFA. При наличии 4 и более баллов по шкале SOFA на 1-е сутки, 5 и более - на 2-е сутки, 7 и более баллов - на 3-й сутки исследуемого периода прогнозируют развитие полиорганной недостаточности.The method is as follows. Пример 1. Больной, 53 года. Диагноз: Закрытая травма живота. Разрыв ректо-сигмоидного отдела толстой кишки. Распространенный фибринозно-гнойный перитонит. Нагноение послеоперационной раны. Полная эвентрация кишечника. Межпетлевые абсцессы. Частичная спаечная тонкокишечная непроходимость. Абдоминальный сепсис, тяжелое течение. Сахарный диабет 2 тип. Язвенная болезнь двенадцатиперстной кишки.Example 1. Patient, 53 years old. Diagnosis: Closed abdominal injury. Rupture of the recto-sigmoid colon. Common fibrinous-purulent peritonitis. Анализ баллов шкалы SOFA был проведен на первые, вторые и третьи сутки от операции. На первые сутки по шкале SOFA определено 4 балла, на вторые сутки - 5, на третьи сутки - 7 баллов. С первых суток у больного имелась первая стадия нарушения функции органов.
