Наиболее близким к заявленному решению является способ фармакологической коррекции ишемии скелетной мышцы ресвератролом (патент RU 2518965 С1 от 10.06.2014). В данном экспериментальном исследовании ишемию мышц голени моделировали оперативным удалением участка магистральных сосудов, включающего бедренную, подколенную, переднюю и заднюю большеберцовые артерии на вторые сутки эксперимента; лабораторному животному внутрижелудочно вводили ресвератрол в суточной дозе 2,0 мг/кг каждые 46 часов первые 7 суток эксперимента; уровень микроциркуляции в мышцах оценивали на 28 сутки ишемии посредством лазерной допплеровской флоуметрии. Однако в данном исследовании ишемию мышц голени моделируют на вторые сутки эксперимента оперативным путем после применения эффекта ишемического прекондиционирования, который состоит, как известно, из двух периодов: раннего (длится до 2 часов и защищает клетки от повреждения) и позднего («второе защитное окно» развивается через 24 часа после начала действия причинного фактора и длится около 48 часов). Кроме этого, недостатком способа является то, что авторами препарат ресвератрол, обладающий доказанным цитопротекторным эффектом, применяется как раз в фазу позднего периода ишемического прекондиционирования, а именно каждые 46 часов, что способствует тому, что уровень микроциркуляции становится выше, чем даже у интактных животных. Основным преимуществом предлагаемого способа является то, что на фоне применения препарата «Актовегин» восстановление микроциркуляции достигается лишь за счет стимуляции неоангиогенеза и активации развития коллатерального кровотока в ишемизированной конечности и улучшение, таким образом, артериального притока крови из проксимальных отделов конечности в дистальные. Опыты проводят на 160 крысах-самцах линии Вистар массой тела 300-350 г. Крысы-самцы были разделены на четыре серии: интактную, контрольную, опытную и сравнения. В каждую из серий было включено по 40 крыс-самцов, которые были разделены на четыре группы в зависимости от срока выведения, в каждую из групп вошло по 10 крыс-самцов. Оценка уровня микроциркуляции в мышцах голени крыс-самцов выполнялась методом лазерной допплеровской флоуметрии (ЛДФ). Исследование осуществляли при помощи аппарата - лазер-допплеровского флоуметра «Biopac-systems MP-100» и датчика «TSD-144». Запись и обработка данных производилась при помощи программы AcqKnowledge 38. Исследование выполняли под наркозом хлоралгидратом в дозе 250-300 мг/кг, который вводили внутрибрюшинно в виде водного раствора. После наступления наркотического сна иссекали участок кожи на переднелатеральной поверхности правой голени задней конечности. Крысу-самца укладывали на противоположную сторону. Датчик вплотную приставляли к мышце и проводили флоуметрию. Уровень микроциркуляции регистрировали в пяти точках: 1) середина длины мышцы; точки на 3-4 мм 2) выше, 3) ниже, 4) латеральнее и 5) медиальнее от первой (патент №2353977 от 27.04.2009). Запись кривой уровня микроциркуляции проводили в течение 30 сек в каждой точке. Из полученных пяти значений выводили среднее, которое вносили в протокол и принимали за уровень микроциркуляции в мышцах голени у данной крысы-самца. Из 10 полученных значений выводили среднее, которое принимали за уровень микроциркуляции в данной группе крыс-самцов на данном сроке исследования.Assessment of the level of microcirculation in the muscles of the tibia of male rats was performed by laser Doppler flowmetry (LDF). The microcirculation level curve was recorded for 30 seconds at each point. From the five values obtained, the average was derived, which was entered into the protocol and was taken as the level of microcirculation in the calf muscles in this male rat. From the 10 obtained values, the average was deduced, which was taken as the microcirculation level in this group of male rats at this study period.
У интактных крыс-самцов оценивали уровень микроциркуляции и мышцах голени правой задней конечности методом ЛДФ. Полученное среднее значение уровня микроциркуляции в этой группе было принято за «норму» и составило 535,22±37,1 перфузионных единиц (п.е.). Как видно из таблицы 1, в контрольной серии крыс-самцов после операции моделирования ишемии отмечается резкое и быстрое снижение уровня микроциркуляции по сравнению с интактной: на 5 сутки - на 71,5%; на 10 сутки - на 61,3%; на 21 сутки - на 42,9%; на 28 сутки - на 33,4%. Прослеживается медленное восстановление уровня микроциркуляции, что свидетельствует о включении имеющихся компенсаторных механизмов коллатерального кровообращения.
