Эта формула является результатом изучения катамнеза 122 детей, находившихся на ИВЛ в раннем неонатальном периоде. Данные по наследственному анамнезу семьи, соматическому и акушерско-гинекологическому анамнезу матерей, получены на основании медицинской документации и собеседования с родителями. Оценка периода новорожденности проведена ретроспективно по данным историй болезни новорожденных. Состояние здоровья детей в постнатальном периоде после выписки из стационара оценивали на основании собеседования с родителями, данных амбулаторных карт развития и других медицинских документов. При осмотре ребенка проводили оценку особенностей физического развития, наличия малых аномалий развития, признаков соединительнотканных дисплазии (классификация Абакумовой Л.Н., 2006). Лабораторно-инструментальное обследование проводилось врачами-специалистами на базе СПбГБУЗ КДЦД. Затем методом логистической регрессии была построена математическая модель для возможности прогнозирования формирования бронхолегочной патологии у детей, находившихся на ИВЛ в неонатальном периоде. Такая модель является экспресс-прогнозом, т.к. строится на основании минимально достаточного числа известных признаков. В качестве прогнозируемого показателя-отклика определен диагноз в возрасте 8 (11) лет (благоприятный - ребенок без патологии бронхолегочной системы - 0 и неблагоприятный - ребенок с патологией легких - 1). В качестве признаков, включаемых в модель как независимые факторы-причины, определена совокупность данных анамнеза, достоверно связанных с исходами. В исходную обучающую матрицу было включено 11 признаков. После логического анализа и оценки связей исходных данных с помощью корреляционного анализа для дальнейшего исследования в обучающей матрице осталось 7 признаков, которые имели сильную (r>0,7) или умеренную (0,27<r<0,7) и статистически значимую (p<0,05) корреляционную связь с исходом. Решение задачи логистического регрессионного анализа было реализовано с помощью процедуры Logistic Regression из пакетов прикладных программ по статистической обработке данных Statistica. По итогам расчетов с помощью модуля Logistic Regression, в модель включены 7 признаков, обладающих статистической надежностью не менее 70%. В результате получилась статистически значимая (p<0,001) и классификационно способная на 76,2% математическая модель. В подгруппе детей без патологии легких исследуемая модель обеспечивает совпадение прогнозируемого результата с реальным в 77,4% случаев (у 65 из 84); в подгруппе детей с патологией бронхолегочной системы совпадение прогнозируемых и реальных исходов отмечено в 73,6% наблюдений (в 28 из 38 случаев); информационная способность модели в целом составляет 76,2% (совпадение диагноза ребенка с прогнозируемым у 93 из 122).This formula is the result of studying the follow-up history of 122 children who were on mechanical ventilation in the early neonatal period. The assessment of the neonatal period was carried out retrospectively according to the data of the newborn’s case histories. The health status of children in the postnatal period after discharge from the hospital was evaluated on the basis of an interview with parents, data from outpatient development cards and other medical documents. When examining a child, an assessment was made of the features of physical development, the presence of small developmental abnormalities, signs of connective tissue dysplasia (classification by Abakumova L.N., 2006). Laboratory and instrumental examination was carried out by medical specialists on the basis of SPbGBUZ KDTSD. Then, using a logistic regression method, a mathematical model was built to predict the formation of bronchopulmonary pathology in children who were on mechanical ventilation in the neonatal period. Such a model is an express forecast, because built on the basis of a minimum sufficient number of known features. As a predicted response indicator, a diagnosis was defined at the age of 8 (11) years (favorable - a child without pathology of the bronchopulmonary system - 0 and adverse - a child with lung pathology - 1). As signs included in the model as independent factors-causes, a set of anamnesis data reliably associated with outcomes is determined. 11 features were included in the original training matrix. After a logical analysis and assessment of the relationships of the source data using the correlation analysis for further research in the training matrix, there are 7 signs that were strong (r> 0.7) or moderate (0.27 <r <0.7) and statistically significant (p <0.05) correlation with outcome. The solution to the problem of logistic regression analysis was implemented using the Logistic Regression procedure from Statistica statistical software packages. According to the results of calculations using the Logistic Regression module, 7 features with a statistical reliability of at least 70% are included in the model. The result was a statistically significant (p <0.001) and 76.2% classifiable mathematical model. In the subgroup of children without lung pathology, the studied model ensures the coincidence of the predicted result with the real one in 77.4% of cases (65 out of 84); in the subgroup of children with pathology of the bronchopulmonary system, a coincidence of predicted and real outcomes was noted in 73.6% of cases (in 28 of 38 cases); the informational ability of the model as a whole is 76.2% (the diagnosis of the child coincides with the predicted in 93 out of 122).
