Распространение специфического воспаления на мочеточник с формированием стриктур осложняет течение нефротуберкулеза в 30-60% случаев [1, 2, 3]. Среди всех стриктур мочеточника доля специфических достигает 9% [4]. Верифицировать диагноз бактериологически удается редко из-за полного или частичного блока пораженной почки. Уретроскопия с биопсией мочеточника должна предприниматься в случаях, когда этиология стриктуры не является очевидной. Технический результат достигается тем, что в способе диагностики и хирургического лечения стриктур мочеточника туберкулезного и неспецифического генеза, включающем уретероскопию и многофокусную биопсию нижней трети мочеточника, дополнительно биоптаты берут из средней и верхней третей мочеточника. При выявлении фиброза очагового характера проводят эндоскопическую коррекцию рубцово измененного участка; в случае диагностики тотального фиброза осуществляют заместительную уретеропластику, а при обнаружении активного туберкулеза выполняют противотуберкулезную терапию.The technical result is achieved by the fact that in the method for the diagnosis and surgical treatment of ureteral strictures of tuberculosis and non-specific genesis, including ureteroscopy and multifocal biopsy of the lower third of the ureter, additional biopsy samples are taken from the middle and upper third of the ureter. Способ осуществляется следующим образом. Под спинальной анестезией выполняют цистоскопию гибким инструментом, через рабочий канал которого проводится одна из струн-проводников. Последнюю вводят через устье в пораженный мочеточник. Цистоскоп извлекают без струны-проводника, по которому в мочевой пузырь, а затем в мочеточник вводят гибкий уретероскоп. Выполняют осмотр всех его отделов с одновременным взятием биоптатов из нижней (тазовой), средней и верхней третей мочеточника с помощью гибких щипцов. Биоптаты помещают в 10% раствор формальдегида и окрашивают гематоксилин-эозином. Операцию завершают внутренним дренированием почки самоудерживающимся стентом.The method is as follows. Многократные обследования в различных урологических стационарах выявили стриктуру левого мочеточника, но не позволили установить этиологию заболевания. 02.02.2010 г. выполнена уретроскопия с многофокусной биопсией мочеточника по описанной методике. Одновременно произведено рассечение стриктуры гольмиевым лазером. Послеоперационный период - без осложнений. Морфологическое исследование биоптатов выявило туберкулезный характер воспаления. На фоне установленного стента начата интенсивная противотуберкулезная терапия. Стент удален через 1 мес. При ежегодном обследовании в течение 2 лет уродинамика верхних мочевых путей не нарушена, функция почек удовлетворительная.Example 1. Поступил в отделение в мае 2011 г. с диагнозом туберкулез мочевой системы: поликавернозный туберкулез правой почки, туберкулезный папиллит левой почки, туберкулез мочеточников; стриктуры нижних отделов обоих мочеточников; уретерогидронефроз с обеих сторон; нефросклероз; нефункционирующая правая почка; ХПН, компенсированная стадия. Длительная противотуберкулезная терапия к регрессу нарушений оттока мочи из почек не привела. 20.01.12 выполнена уретроскопия с многофокусной биопсией мочеточника слева по описанной методике. По данным морфологического исследования обнаружен тотальный фиброз стенки органа во всех биоптатах, специфических изменений не выявлено. Принято решение заместительной пластике мочеточника. 14.03.12. - произведена илеоуретропластика слева. Послеоперационный период - без осложнений. Мочеточниковый дренаж удален на 12 сутки. Наблюдение в течение года не выявило изменений со стороны верхних мочевых путей и нарушения функции правой единственной функционирующей почки.Example 2. Patient K., 48 years old. Observation during the year did not reveal changes in the upper urinary tract and impaired function of the only functioning kidney.
В 2008-2011 гг. в урологическом отделении ФГБУ «Уральский НИИ фтизиопульмонологии Минздравсоцразвития РФ» уретеропиелоскопия с многофокусной биопсией стенки ВМП выполнена 11 больным туберкулезом почки и мочеточника (7 женщинам и 4 мужчинам). В ряде других случаев исследование осуществить не удалось из-за выраженного стеноза устья мочеточника. В ходе процедуры стриктуры, служившие препятствием для продвижения уретероскопа по мочеточнику, рассекались холодным ножом. При морфологическом исследовании биоптатов (световой микроскопии) у 27,2% больных просвет сосудов микроциркуляторного русла оказался расширенным. Степень выраженности клеточной инфильтрации варьировала. Воспалительный инфильтрат у 90,9% больных состоял из лимфоцитов, плазматических клеток, макрофагов, фибробластов. Sites of atrophy and hyperplasia of the epithelium were detected in 54.5% of cases, submersible growth of urothelium (acanthosis) - in 9.1%, proliferative activity with elements of dysplasia - in 63.6%.
Морфологическое исследование биоптатов мочевых путей выявило полнокровие сосудов, отек разной степени выраженности, элементы склероза, скопления фибробластов, диффузную лимфоидно-гистиоцитарную инфильтрацию, характерные для склерозирующего процесса (фиг.2), у 8 (72,7%) пациентов, гранулезное воспаление с наличием гигантских многоядерных клеток Лангханса - у 2 (18,2%) из них (фиг.3). У 3 (27,3%) больных отмечена лимфоидно-гистиоцитарная инфильтрация и скопления фибробластов очагового характера. Следует отметить, что уретероскопия с или без уретеротомии в 3 (почти у ⅓) случаях, соответствующих морфологической картине очагового фиброза стенки ВМП, способствовала регрессу ретенционных изменений. Очаговый характер патологических изменений стенки ВМП позволяет ограничиться эндоскопическими способами коррекции стриктур. При распространенных фиброзно-воспалительных изменениях стенки мочеточника мы считаем эндоскопическое лечение бесперспективным, в этих случаях абсолютно показана заместительная уретеропластика (операция Боари или кишечная пластика).
