Изобретение относится к области медицины, в частности к ортопедии и травматологии, а именно к послеоперационному восстановлению, и предназначено для лечения и профилактики некротических изменений кожи в области послеоперационной раны после тотального цементного эндопротезирования голеностопного сустава с последующим применением гирудотерапии.The invention relates to medicine, in particular to orthopedics and traumatology, namely to postoperative recovery, and is intended for the treatment and prevention of necrotic skin changes in the area of a postoperative wound after total cemented ankle replacement followed by hirudotherapy.
Учитывая объем оперативного вмешательства, вполне естественно, что операция эндопротезирования вызывает существенные физиологические и метаболические изменения. В послеоперационном периоде наблюдаются выраженные воспалительно-травматологические процессы: венозный стаз, микротромбы, асептическое местное воспаление и подобные процессы, которые ведут к циркуляторной гипоксии и оксидативному стрессу, как следствие, к местным репаративным нарушениям. Голеностопный сустав является самым дистальным крупным суставом, у которого отсутствует богатый кровеносными сосудами мышечный массив, но имеются плотные соединительнотканные фасции с небольшим адаптационным резервом кровоснабжения.Given the volume of surgical intervention, it is quite natural that the endoprosthetics operation causes significant physiological and metabolic changes. Способ осуществляют следующим образом.The method is as follows.
Пациентам по стандартной методике проводят эндопротезирование голеностопного сустава. В послеоперационном периоде назначают соответствующее медикаментозное лечение, направленное на профилактику гнойно-септических и тромботических осложнений, а также общеукрепляющее, спазмолитическое и обезболивающее.Patients according to the standard method perform ankle replacement. Для объективной оценки изменений, происходящих в области шва послеоперационной раны и в прилегающих мягких тканях, проводили исследования с помощью лазерного доплеровского флоуметра (ЛДФ), позволяющего измерять скорость капиллярного кровотока (КК).
