К известным способам определения плотности костной ткани относят двойную рентгеновскую абсорбциометрию, количественную компьютерную томографию, метод ядерного магнитного резонанса. Существующие современные методы рентгеновской диагностики основаны на четкой связи между степенью поглощения рентгеновских лучей и костной массой, однако в очень небольшой степени отражают структурные изменения, влияющие на прочность кости (Л.А, Щеплягина, Т.Ю.Моисеева, М.В.Коваленко и др. Недостатком данных методов является невозможность определения прочности кости, использования их для скрининга у здоровых детей раннего возраста и мониторинга динамики проводимых коррекционных мероприятий, ввиду лучевой нагрузки. Для диагностики нарушения снижения прочности кости у детей используют количественную ультрасонометрию (КУС). Процедура ультразвуковой диагностики непродолжительна, безболезненна, может многократно повторяться, не имеет противопоказаний к назначению. Приборы для количественного ультразвукового исследования занимают мало места и не требуют радиационной защиты. Ультразвуковой метод обеспечивает регистрацию даже незначительных изменений плотности биологических сред. В силу своей простоты, безвредности и эффективности он может применяться для ранней диагностики снижения костной прочности у новорожденных и детей раннего возраста. Все методы КУС являются не визуализирующими, основываются на взаимодействии ультразвука с костной тканью, через которую он проходит, в результате чего получаем количественный результат, характеризующий такие свойства кости, как масса, структура или качество. Взаимодействие между ультразвуковыми волнами и костной тканью ведет к изменению быстроты звука и к снижению мощности проведения звука. Применительно к методам КУС быстротой звука обычно называют «скорость звука» (Speed of Sound - SOS) и она измеряется в единицах - метр в секунду (м/с). Костная прочность оценивается методом количественной ультрасонометрии с использованием прибора Omnisense 7000S ("Sunlight Medical Ltd", Израиль). Костную прочность у детей грудного возраста диагностируют в середине диафиза большеберцовых костей, используют специальный датчик и педиатрическую программу. Продолжительность измерения одного участка скелета составляет не менее 1 мин. Полученные данные сравнивают с перцентильными таблицами для данного прибора. Снижение костной прочности диагностируют при скорости звука ниже 10 перцентиля (Z-score ниже -1 стандартное отклонение (SD)). Снижение скорости звука менее 5 перцентиля (Z-score ниже -2 SD), расценивают как патологию (Щеплягина Л.А., Круглова И.В. In relation to the KUS methods, the speed of sound is usually called the “speed of sound” (Speed of Sound - SOS) and it is measured in units - meter per second (m / s). Bone strength is assessed by quantitative ultrasonometry using an Omnisense 7000S instrument (Sunlight Medical Ltd, Israel). Bone strength in infants is diagnosed in the middle of the diaphysis of the tibia, using a special sensor and a pediatric program. The duration of the measurement of one section of the skeleton is at least 1 min. The data obtained are compared with percentile tables for this device. Decrease in bone strength is diagnosed at a sound speed below 10 percentile (Z-score below -1 standard deviation (SD)). A decrease in the speed of sound of less than 5 percentile (Z-score below -2 SD) is regarded as a pathology (Scheplyagina L.A., Kruglova I.V. Задачей изобретения является раннее экспресс-прогнозирование патологии развития костной системы у детей трудного и раннего возраста в амбулаторных условиях путем измерения прочности кости в физиологические критические периоды ее формирования. Ожидаемым техническим результатом заявленного изобретения будет выделение диспансерной труппы наблюдения детей в условиях детской поликлиники для проведения лечебно-профилактических мероприятий по снижению прочности кости.The objective of the invention is the early rapid forecasting of the pathology of the development of the skeletal system in children of difficult and young age on an outpatient basis by measuring bone strength in physiological critical periods of its formation. Сущность предложенного способа состоит в определении скорости ультразвуковой волны в кортикальном слое трубчатых костей у детей грудного и раннего возраста в критические возрастные периоды формирования кости: 4,8,18,24 месяцев, при снижении скорости звука менее 2776 м/с в 4 месяца, менее 2970 м/с - в 8 месяцев, менее 3223 м/с - в 18 месяцев и менее 3235 м/с - в 24 месяца прогнозируют неблагоприятное формирование кости.The essence of the proposed method consists in determining the speed of the ultrasonic wave in the cortical layer of tubular bones in infants and young children in critical age periods of bone formation: 4,8,18,24 months, with a decrease in the speed of sound less than 2776 m / s in 4 months, less 2970 m / s - at 8 months, less than 3223 m / s - at 18 months and less than 3235 m / s - at 24 months, adverse bone formation is predicted.
Возрастные периоды, в которые проводится исследование прочности кости детей раннего возраста, выбраны на основании проведенного нами КУС исследования и полученных результатов: физиологическое критическое снижение прочности кости у грудных детей в процессе роста и минерализации в 4 и 8 месяцев и быстрый набор прочности кости на втором году жизни (фиг.1, 2). На фиг.1 представлены средние показатели SOS большеберцовой кости у мальчиков в зависимости от возраста. На фиг.2. - средние показатели SOS большеберцовой кости у девочек в зависимости от возраста. Способ осуществляется следующим образом. Измерение скорости звука проводят в соответствии с инструкцией, прилагаемой к прибору Omnisense 7000S ("Sunlight Medical Ltd", Израиль). Специальный педиатрический датчик прибора устанавливают в середине диафиза большеберцовой кости у грудного ребенка. Датчик перемещают поперек кости с отклонением в 45° от продольной оси. Продолжительность измерения одного участка скелета составляет от 30 сек до 1 мин. Всего таких исследований проводится от 3 до 5 раз. Если скорость звука менее 2776 м/с в возрасте 4 месяцев, в возрасте 8 месяцев - менее 2970 м/с, в возрасте 18 месяцев - менее 3223 м/с, а в 24 месяца - менее 3235 м/с, то прогнозируют неблагоприятное формирование кости. И данные дети формируют группу диспансерного наблюдения с проведением лечебно-профилактических мероприятий и углубленного обследования.The method is as follows. При проведении количественной ультрасонометрии - показатели SOS 2689 м/с (ниже нормы), проведены коррекционные мероприятия (увеличена доза витамина D и в питание добавлен витамино-минеральный комплекс «Friso» - обогатитель трудного молока, содержащий дополнительно три вида солей кальция, фосфор и другие эссенциальные микроэлементы и витамины, сроком на 2 месяца). При проведении обследования через 2 месяца, показатели скорости звука в кости были в пределах возрастной нормы - 3060 м/с. Пациент П., 18 месяцев. На первом году жизни клинически диагноз рахита не ставился. При проведении КУС установлено снижение прочности кости ниже порогового значения - 3180 м/с. Коррекционных мероприятий не проводилось. В динамике ребенок осмотрен повторно, ребенок не набрал нормальную прочность кости. Всего предложенным способом (КУС) было обследовано 2316 детей от 0 до 3 лет. Декретированным сроком проведения исследования от 0 до 1 года явился 1 месяц, от 1 года до 2 лет - 3 месяца, от 2 до 3 лет - 6 месяцев. В ходе исследования установлены критические сроки формирования кости (4, 8, 18, 24 месяцев). При анализе данных средневозрастных показателей скорости звука кости установлено, что физиологическое снижение показателей скорости звука в кости голени происходит от 1 месяца к 4 месяцам максимально, а в дальнейшем увеличивается к году, как у мальчиков, так и у девочек. Следовательно, возраст 4 месяца является «критическим моментом» формирования кости и является «отрезной» точкой диагностики прочности коечной ткани на первом году жизни. Второй физиологический пик снижения костной массы регистрировался в 8 месяцев, что взято нами за вторую «отрезную» точку для контроля прочности кости.In total, the proposed method (CLC) examined 2316 children from 0 to 3 years old. The second physiological peak of bone mass reduction was recorded at 8 months, which we took as the second “cut-off” point for monitoring bone strength.
В возрасте 4 месяцев обследовано 109 детей (средние показатели скорости звука 2873±79 м/с). У 14 из них (12,8%) диагностированы низкие показатели скорости ультразвуковой волны (ниже 2776 м/с). Группу контроля (n=40) составили дети с нормальными показателями прочности кости в 4 месяца (средние показатели скорости звука большеберцовой кости 2892±134 м/с).
