Известно устройство, включающее захват пластинчатый управляемый для удаления камней из желчного протока, которое содержит средство захвата камня, соединенное с толкателем. Толкатель расположен в катетере. Катетер установлен в эндоскопе. Средство захвата камня выполнено в виде упругой пластины. Техническое решение. Заявляемое устройство для литэкстракции представляет собой полую трубку - наружный корпус, которая вводится в холедохотомическое отверстие. В просвете трубки в собранном виде находятся четыре стержня, положение которых регулируется спицами. Последние фиксируются к внутреннему корпусу устройства. Данная трубка сверху оканчивается ручкой. Изменение положения ручки вниз ведет к расправлению стержней по типу «зонтика», а ручки вверх - к их возвращению в исходное положение. Указанные стержни вогнуты в просвет протока, что предупреждает повреждение стенок последнего во время тракции конкремента.Technical solution. Устройство работает следующим образом. При выставлении показаний к операции производят минилапаротомию в правом подреберье и обеспечивают доступ с помощью набора «миниассистент». Поле чего холедох берут на держалки и выполняют холедохотомию. Ориентируясь на данные дооперационного обследования, в просвет холедоха (проксимальную или дистальную его часть) вводят наружный корпус устройства 2 длиной 300 мм и шириной 4,5 мм. Всего в просвете в собранном виде находятся четыре стержня 6, положение которых регулируется спицами 5. Последние фиксируются к внутреннему корпусу устройства 4. Данная трубка (внутренний корпус 4) сверху оканчивается ручкой устройства 1. Изменение положения ручки 1 вниз ведет к расправлению стержней 6 по типу «зонтика». Изменение положение ручки 1 вверх ведет к их возвращению в исходное положение. Указанные стержни 6 вогнуты в просвет протока, что предупреждает повреждение стенок последнего во время тракции конкремента.The device operates as follows. Больной Т., 56 лет, госпитализирован в экстренном порядке с диагнозом ЖКБ. Острый простой холецистит. Холедохолитиаз. Синдром механической желтухи. По данным УЗИ в холедохе визуализировался конкремент до 1,3 см. Начата и проведена инфузионная, спазмолитическая, противовоспалительная терапия. На фоне проводимого лечения острый приступ купирован, билиарная гипертензия разрешилась. Выставлены показания к оперативному лечению. Выполнена минилапаротомия, холецистэктомия. Далее, холедох взят на держалки и выполнена холедохотомия. В холедохотомическое отверстие введено устройство, с помощью которого конкремент удален. При контрольной холедохоскопиипатологии не выявлено. Холедох дренирован по Керу. Рана ушита. Выполнена минилапаротомия с помощью набора «миниассистент». Холедох взят на держалки и выполнена холедохотомия. В холедохотомическое отверстие введено устройство (захват), с помощью которого удалены два конкремента диаметром 0,4 и 0,7 см. При контрольной холедохоскопии патологии не выявлено. Холедох дренирован по Керу. Рана ушита. По данным послеоперационной фистулографии сброс контраста в двенадцатиперстную кишку удовлетворительный; теней, подозрительных на конкременты, не обнаружено. Произведена холедохотомия. При холедохоскопии в терминальном отделе холедоха визуализировался конкремент до 1,0 см в диаметре. Попытки отмыть конкремент, извлечь с помощью корзинки Дормина безуспешны. В холедохотомическое отверстие, ниже зоны обтурации, введено устройство (захват), с помощью которого конкремент удален.
