Несмотря на значительные успехи консервативной терапии язвенной болезни двенадцатиперстной кишки (ДПК), почти у половины больных с течением времени развиваются опасные для жизни осложнения. Грозным и опасным для жизни осложнением является перфорация язвы. По данным литературы язвенная болезнь двенадцатиперстной кишки встречается у 7-15% трудоспособного населения, а частота перфораций гастродуоденальных язв составляет около 1,5-2 человека на 10 тыс. трудоспособного населения. В структуре острой хирургической патологии органов брюшной полости это осложнение составляет 1,9-2,6%. Летальность при перфоративных язвах остается высокой, не имеет тенденции к снижению особенно у пожилых людей и составляет 5-15% (Белов И.И. 2004; W.T.Siu, H.T.Leong, Law et al. 2002).Despite the significant success of conservative treatment of duodenal ulcer (duodenal ulcer), almost half of the patients develop life-threatening complications over time. Одним из методов, применяемых при лечении перфоративной дуоденальной язвы, является операция, состоящая из ушивания перфоративного отверстия в сочетании с лапароскопической суперселективной ваготомией (С.Grassi), (прототип заявленного способа - А.А.Курыгин, В.В.Румянцев. Ваготомия в хирургической гастроэнтерологии. Санкт-Петербург, 1992. - 152 с.). После ушивания перфоративного отверстия выполняется селективная проксимальная ваготомия, при которой выполняется пересечение проксимальных ветвей блуждающих нервов, иннервирующих тело и свод желудка, непосредственно у их вхождения в стенку желудка. Данная операция обеспечивает денервацию всех кислото-пепсинопродуцирующих полей желудка при сохранении иннервации пилорического жома и его функции за счет нервов Латарже. Однако выполнение данной операции в ее открытой модификации приводит к развитию послеоперационных осложнений, связанных как с особенностями операции, так и с травматичностью хирургического доступа (нагноение послеоперационной раны, внутрибрюшные инфильтраты и абсцессы, несостоятельность швов в области перфоративного отверстия, ранняя спаечная кишечная непроходимость, легочные осложнения). However, the performance of this operation in its open modification leads to the development of postoperative complications associated with both the features of the operation and the trauma of surgical access (suppuration of the postoperative wound, intraperitoneal infiltrates and abscesses, inconsistency of sutures in the perforated opening, early adhesive intestinal obstruction, pulmonary complications ) The total frequency of complications, according to the author of the prototype of the claimed method, reaches 4.6%.

Способ осуществляется следующим образом.The method is as follows.
Больным с целью уточнения диагноза и выбора оперативной тактики после дообследования выполняется диагностическая лапароскопия. При верификации диагноза и отсутствии противопоказаний выполняется ушивание перфоративного отверстия и санация брюшной полости. Для чего выполняют лапароскопию, во время которой измеряют диаметр перфоративного отверстия и протяженность воспалительного инфильтрата от краев перфоративного отверстия. В том случае, если имеет место перфоративная язва 12-перстной кишки класса IIIC, IVA, IVC, выполняется открытая операция и селективная ваготомия не проводится. Далее визуализируется конечная ветвь левого (переднего) ствола блуждающего нерва (ветвь Латарже), иннервирующая кисло-репродуцирующую зону желудка. От проксимальной ветви Латарже по направлению к пищеводу по малой кривизне с помощью ультразвуковых ножниц (например, Ultracision, фирма Ethicon, США) выполняется селективная проксимальная ваготомия. Больному выполнена диагностическая лапароскопия, при которой выявлена перфоративная язва передней стенки 12-перстной кишки размером до 5 мм и воспалительным валом протяженностью 1 см (IIB). Больному выполнено лапароскопическое ушивание перфоративной язвы 12-перстной кишки отдельными однорядными швами рассасывающейся нитью, селективная проксимальная ваготомия, санация, дренирование брюшной полости.The patient underwent diagnostic laparoscopy, in which a perforated ulcer of the anterior wall of the duodenum 12 up to 5 mm in size and an inflammatory shaft 1 cm long (IIB) were revealed. Послеоперационный период протекал гладко. Дренажные трубки удалены на 4-5 сутки после операции. Швы сняты на 6 сутки. Раны зажили первичным натяжением. В послеоперационном периоде выполнены обзорный рентгеновский снимок грудной клетки и брюшной полости, УЗИ органов брюшной полости, рH-метрия и рентгенологическое исследование желудка с контрастным веществом.The postoperative period was uneventful.
