Abstract
Description
Изобретение относится к области медицины, а именно к неврологии, и может быть использовано при восстановлении когнитивных функций (внимания) у больных с цереброваскулярной патологией.The invention relates to medicine, namely to neurology, and can be used to restore cognitive functions (attention) in patients with cerebrovascular pathology.
Существуют способы стандартного медикаментозного лечения пациентов с цереброваскулярной патологией (1).There are methods for standard drug treatment of patients with cerebrovascular pathology (1).

Известен способ тренировки внимания с использованием Таблиц Шульте, когда больному предъявляется 5 матриц из 25 клеток (5×5), в которых в случайном порядке нанесены числа от 1 до 25. Пациент должен отыскивать и называть вслух по порядку эти числа как можно быстрее, при этом фиксируется время выполнения задания (3). На основании вычисления среднего времени выполнения пяти заданий оценивается сохранность возможности концентрации внимания. «Бумажный» вариант таблиц используют только с диагностической целью, в связи с тем, что коррекционный способ требует многочисленное дидактическое оснащение (оформление карточек с различным расположением чисел). Время, выделяемое на поиск одного числа, может варьировать в зависимости от степени нарушений внимания пациента и задается инструктором. По завершении работы выводится время, затраченное на выполнение задания. После чего на экране монитора появляется следующая таблица для дальнейшего выполнения задания. Занятие проводят один раз в день в течение 20 минут. Нами было обследовано 25 пациентов в остром периоде ишемического инсульта, с когнитивными нарушениями разной степени выраженности. Исследуемую группу составили 15 человек, которые кроме медикаментозной терапии прошли курс реабилитации с использованием компьютеризированной таблицы Шульте, в течение 10 дней 1 раз в день, продолжительность одного занятия составляла 20 минут. Для подтверждения эффективности предлагаемого способа использовали общепринятые тесты оценки когнитивных функций: опросник для родственников о когнитивном состоянии пожилого человека (IQCODE), краткая шкала оценки психического статуса (MMSE), батарея тестов лобной дисфункции (FAB), тест рисования часов (часы), «корректурная таблица» (модификация пробы В.Н.Аматуни), с помощью которой определяли индекс утомляемости (ИУ). Одновременно оценивались в динамике двигательные, чувствительные и другие неврологические нарушения по шкале инсульта Национального института здоровья (NIHSS). Пациенты контрольной и исследуемой групп не имели различий в степени выраженности когнитивных нарушений по результатам тестирования с использованием указанных методик.To confirm the effectiveness of the proposed method, generally accepted tests for evaluating cognitive functions were used: a questionnaire for relatives on the cognitive state of an elderly person (IQCODE), a brief scale for assessing mental status (MMSE), a battery of frontal dysfunction tests (FAB), a clock drawing test (clock), "proofreading table ”(modification of the test by V.N. На фоне проводимых занятий с использованием метода биологической обратной связи с помощью компьютеризированной таблицы Шульте был отмечен достоверный регресс выраженности когнитивных и двигательных нарушений. О достоверном улучшении когнитивных функций на фоне проводимых занятий свидетельствовала положительная динамика результатов большинства нейропсихологических тестов, которые использовались в исследовании. Улучшилась память, концентрация и переключение внимания. В сравнении с группой контроля отмечался статистически значимый положительный эффект коррекции когнитивных нарушений (z=3,4 при р=0,0003).Against the background of the classes using the biological feedback method using a computerized Schulte table, a significant regression of the severity of cognitive and motor impairment was noted. Для оценки когнитивных функций использовались следующие методики: краткая шкала оценки психического статуса (MMSE), батарея тестов лобной дисфункции (FAB), тест рисования часов (часы), «корректурная таблица» (модификация пробы В.Н.Аматуни), с помощью которой определяли индекс утомляемости (ИУ).
