ОА является одной из наиболее частых причин острого живота, требующего хирургического вмешательства. Диагностика ОА в типичных случаях не представляет сложности. Однако не всегда удается своевременно и правильно поставить диагноз и вовремя установить показания к оперативному вмешательству. В связи с этим диагностика ОА является актуальной проблемой хирургии.OA is one of the most common causes of acute abdomen requiring surgical intervention. Слизистая оболочка кишечника подвергается непрерывному воздействию огромного количества провоспалительных и иммуногенных стимулов, которые исходят из продуктов питания и нормальной микрофлоры кишечника. Динамический баланс микрофлоры желудочно-кишечного тракта (ЖКТ) поддерживается различными механизмами, как иммунными, так и неиммунными. Главную роль при этом играет лимфоидная ткань кишечника. В норме существует регуляция и избирательность механизмов защиты, которые в конечном итоге определяют иммунологическую толерантность относительно индигенной микрофлоры или иммунный ответ на патогены. Дисбиотические отклонения могут изменить фенотип лимфоидной ткани кишечника в сторону превалирования Тh2-клеток, а также способствовать развитию аллергических реакций, аутоиммунных расстройств и воспалительных заболеваний кишечника, в частности развитию острого аппендицита аллергического генеза.The intestinal mucosa is continuously exposed to a huge number of pro-inflammatory and immunogenic stimuli that come from food and normal intestinal microflora. Dysbiotic abnormalities can change the phenotype of intestinal lymphoid tissue towards the prevalence of Th2 cells, and also contribute to the development of allergic reactions, autoimmune disorders and inflammatory bowel diseases, in particular the development of acute appendicitis of allergic genesis.

Проведение лапароскопии может быть затруднено у лиц, перенесших ранее оперативные вмешательства на органах брюшной полости. Кроме того, лапароскопия сама по себе является оперативным вмешательством и может повлечь за собой осложнения: ранения полых органов, бронхоспазм, эмфизему большого сальника и средостения, кровотечение. В связи с этим диагностическая лапароскопия не может выполняться в качестве скрининг-теста у всех больных с подозрением на ОА. При обследовании пациента с подозрением на ОА аллергического генеза на основании беседы и данных лабораторных методов исследования определяют: аллергологический анамнез, соблюдение диеты накануне заболевания, боли в животе ранее, лейкоцитарный индекс интоксикации (ЛИИ), уровень С-реактивного белка, уровень IgG и IgE в периферической крови, уровень циркулирующих иммунных комплексов. Полученные данные сопоставляют с таблицей, где показатели оценены в баллах. Рассчитывают сумму баллов, определяющую величину индекса риска развития острого аппендицита (ИРОА), который может принимать значения от 0 до 15. Величина ИРОА менее 5 баллов свидетельствует о низком риске, а величина ИРОА, равная или более 5 баллов - о высоком риске развития острого аппендицита аллергического генеза. При значении ИРОА, равном или более 5 баллов, требуется выполнение оперативного вмешательства - аппендэктомии. В дальнейшем данным больным рекомендуется проведение специфического аллергологического обследования с целью установления и элиминации причинно-значимых аллергенов для профилактики развития аллергического воспаления на других участках ЖКТ, поскольку аппендэктомия решает проблему только местного характера.When examining a patient with a suspicion of OA of allergic genesis, on the basis of the conversation and the data of laboratory research methods, an allergological history, a diet on the eve of the disease, earlier abdominal pain, leukocyte intoxication index (LII), C-reactive protein level, IgG and IgE levels in peripheral blood, the level of circulating immune complexes. With a value of IROA equal to or more than 5 points, an operative measure is required - appendectomy. In the future, these patients are recommended to conduct a specific allergological examination in order to establish and eliminate causally significant allergens to prevent the development of allergic inflammation in other parts of the gastrointestinal tract, since appendectomy solves the problem only of a local nature.
По предложенному способу в хирургическом отделении ФГУЗ КБ №51 ФМБА РФ г.Железногорска Красноярского края было обследовано 34 пациента, имеющих отягощенный аллергологический анамнез, в возрасте от 8 до 55 лет с клинически верифицированным диагнозом "острый аппендицит".According to the proposed method, in the surgical department of Federal State Health Institution KB No. 51 of the FMBA of the Russian Federation, Zheleznogorsk, Krasnoyarsk Territory, 34 patients with an aggravated allergic history, aged 8 to 55 years, with a clinically verified diagnosis of acute appendicitis, were examined.

Клинический пример 1. Больной А., 19 лет. Поступил с жалобами на боли в нижних отделах живота, тошноту. Болен в течение 5-и часов. При осмотре - умеренная болезненность в нижних отделах живота, аппендикулярные симптомы выражены слабо, симптомов раздражения брюшины нет. Предварительный диагноз: кишечная колика? острый аппендицит? Проведено обследование больного по предлагаемому способу, в результате которого получены следующие данные: положительный аллергологический анамнез - 2 балла; нарушение диеты накануне заболевания - 2 балла; боли в животе ранее (около 6 месяцев назад беспокоили боли в правой подвздошной области, которые купировались самостоятельно) - 1 балл; уровень IgG 18,45 мг/мл - 2 балла; уровень IgE 146 МЕ/мл - 2 балла; уровень циркулирующих иммунных комплексов 115 у.е. - 2 балла. ИРОА составил 11 баллов. Полученное значение ИРОА входит в интервал "равно или больше 5 баллов". Заключение: у пациента высокий риск развития ОА аллергического генеза. Выполнена аппендэктомия. Гистологическая диагностика удаленного 40 подтвердила диагноз ОА, гистологическая картина свидетельствовала в пользу аллергической природы воспаления.Clinical example 1. Patient A., 19 years old. IROA was 11 points. The obtained value of the IROA is included in the interval "equal to or greater than 5 points." Conclusion: the patient has a high risk of developing OA of allergic genesis. Appendectomy performed. The histological diagnosis of the removed 40 confirmed the diagnosis of OA, the histological picture testified in favor of the allergic nature of the inflammation.
Клинический пример 2. Больной М., 25 лет. Поступил с жалобами на боли в нижних отделах живота, тошноту. Болен в течение 8-и часов. При осмотре - умеренная болезненность в нижних отделах живота, аппендикулярные симптомы выражены слабо, симптомов раздражения брюшины нет. Предварительный диагноз: кишечная колика? острый аппендицит? Проведено обследование больного по предлагаемому способу, в результате которого получены следующие данные: положительный аллергологический анамнез - 2 балла; ЛИИ 1,2-2 балла. ИРОА составил 4 балла. Полученное значение ИРОА входит в интервал "менее 5 баллов". Заключение: у пациента низкий риск развития ОА. Поставлен диагноз: кишечная колика.
