Известен доступ к таранной кости (трансмаллеолярная остеотомия по Шеврону), когда производят фигурную остеотомию внутренней лодыжки, при которой линии остеотомии расположены в разных плоскостях под углом по отношению друг к другу. Вершина угла расположена по линии, проходящей по центру внутренней лодыжки (на боковой проекции). Дистальный конец линии остеотомии во фронтальной плоскости заканчивается в точке соединения нижней суставной поверхности большеберцовой кости с внутренней лодыжкой (Campbell′s Operative Orthopaedics, 11 edition, S. Terry Canale, James H. После разреза кожи и осуществления доступа к медиальной лодыжке производится остеотомия. Плоскости остеотомии располагаются под углом относительно друг друга. При этом одна из плоскостей может захватывать переднюю либо заднюю часть дистальной суставной поверхности большеберцовой кости, в зависимости от необходимости визуализировать переднемедиальный либо заднемедиальный отдел тела таранной кости. Пациентка К., поступила с жалобами на боли в области правого голеностопного сустава. После клинико-рентгенологического обследования был установлен диагноз «киста тела таранной кости». В соответствии с изобретением было произведено оперативное лечение. После разреза кожи и осуществления доступа к внутренней лодыжке была произведена остеотомия в двух плоскостях, одна из которых проходила через задний край дистальной суставной поверхности большеберцовой кости. На фиг.1 изображена схема остеотомии в двух плоскостях при доступе к таранной кости. Интраоперационно был подтвержден диагноз «киста тела таранной кости». Угол между плоскостями остеотомии составил 90°. При этом был обеспечен широкий обзор заднемедиальных отделов таранной кости, что позволило осуществить доступ к кисте таранной кости, которая была заполнена аутотрансплантатом из крыла подвздошной кости. После завершения основного этапа операции была произведена репозиция отломков и осуществлена фиксация одним винтом. Было произведено послойное ушивание раны. Послеоперационный период протекал без особенностей, рана зажила первичным натяжением, на рентгенограммах стояние отломков удовлетворительное, без смещения.Patient K. was admitted with complaints of pain in the right ankle joint. После клинико-рентгенологического обследования был установлен диагноз «киста тела таранной кости». В соответствии с изобретением было произведено оперативное лечение. Под спинномозговой анестезией после обработки операционного поля произведен продольный разрез кожи в проекции внутренней лодыжки левого голеностопного сустава. Осуществлен доступ к внутренней лодыжке. Произведена двухплоскостная остеотомия, одна плоскость которой проходит через внутреннюю лодыжку, другая - захватывает медиальную часть заднего края большеберцовой кости. На фиг.2 изображена схема остеотомии в двух плоскостях при доступе к таранной кости. Угол между плоскостями остеотомии составил 70°. Получен широкий обзор тела таранной кости. Интраоперационно был подтвержден диагноз «киста тела таранной кости». Произведена обработка полости кисты, заполнение дефекта аутотрансплантатом из крыла подвздошной кости. По завершении основного этапа операции осуществлена репозиция отломков и фиксация одним винтом. Рана послойно ушита. На контрольных рентгенограммах стояние отломков удовлетворительное, без смещения.
