Технический результат достигается тем, что в способе лечения больных с гнойно-воспалительными заболеваниями нижних конечностей на фоне сахарного диабета путем введения инсулина, антиагрегантов и антибактериальной терапии, в качестве которой осуществляют клеточно-ассоциированное введение препаратов, для чего выделенные форменные элементы аутокрови в количестве 1-1,45 мл на 1 кг массы тела больного смешивают с 5 мл 20%-ного раствора цефтриаксона и 0,3 мл диметилсульфоксида, инкубируют в течение 30 мин, добавляют 1 мл дистиллированной воды на 5 мл выделенных форменных элементов аутокрови, выдерживают в течение 10-15 мин. Ввиду прогрессирования флегмоны стопы больной предложена ампутация конечности, от которой она категорически отказалась. Направлена в МРКБ. При поступлении в области 1-го межпальцевого промежутка рана 4×4 см с гнойным отделяемым, некрозом мягких тканей и сухожилий. На основании анализа результатов лечения 45 пациентов с гнойно-некротическими осложнениями сахарного диабета установлено, что применение метода оказывает более выраженное антибактериальное и ранозаживляющее действие, что позволяет быстрее и эффективнее купировать клинические проявления заболевания по сравнению с традиционными схемами антибиотикотерапии. У пациентов 3-й группы зарегистрировано более раннее (по сравнению больными с 1- и 2-й групп) появление грануляций, ускорение краевой эпителизации ран. Отмечено увеличение частоты встречаемости воспалительно-регенераторного типа цитограмм раневого отделяемого и снижение воспалительно-дегенеративного.A study of the dynamics of the wound process (based on cytograms from the surface of the wounds) and a number of clinical indicators.
