Abstract
Description
Изобретение относится к области медицины, в частности к оперативной хирургии. Эндоскопическая папиллосфинктеротомия является необходимым вмешательством у больных с осложненной желчекаменной болезнью. Однако манипуляции на БСДК и рассечение его ампулы не лишены опасности развития серьезных осложнений - кровотечения, панкреатита, забрюшинной перфорации. Осложнения, связанные с ЭПСТ, возникают в 0,6-23,4% (Поташов Л.В., Кудреватых И.П., Щетинин В.Н., Полиглотов О.В., Христолюбив А.В. Осложнения папиллосфинктеротомии и их профилактика. Забрюшинная перфорация встречается крайне редко, но последствия ее развития порой фатальны для больного (99% умирают на третьи сутки после операции). Некоторые авторы считают, что при ретродуоденальной перфорации достаточно выполнить назобилиарное дренирование общего желчного протока. Другие исследователи для лечения данной категории выполняют лапаротомию, холедохотомию, поиск и ушивание перфоративного отверстия, дренирование холедоха, брюшной полости и забрюшинного пространства(Freemar M.L., Nelson D.B., Sherman S. Haper G.B. et al. Complications or endoscopic biliari sphincterotomy. Ушивание перфорации в условиях развития забрюшинной флегмоны обречено на несостоятельность. Использование назобилиарного дренирования в лечении забрюшинных перфораций (Ретродуоденальные перфорации при эндоскопическом рассечении большого сосочка двенадцатиперстной кишки. Б.С.Брискин, П.В.Эктов, А.Г.Карцев, А.Э.Иванов, Ю.Ф.Клименко. Эндоскопическая хирургия, 1, 2003, С.30-34) не приводит к ограничению воспалительного процесса и способствует дальнейшему развитию флегмоны и перитонита. Решение задачи заключается в том, что при выполнении лапаротомии по поводу забрюшинной перфорации двенадцатиперстной кишки после эндоскопической папиллосфинктеротомии выполняют стандартную холецистэктомию, холедохотомию, холедохолитотомию, мобилизуют двенадцатиперстную кишку по Кохеру. Отличием предложенного способа является то, что оперативное вмешательство заканчивают дренированием холедоха Т-образным дренажем по Керру, который подбирают строго в соответствии с шириной дистального отдела желчного протока. Причем после введения верхней бранши дренажа, нижнюю устанавливают через интрапанкреатическую часть холедоха в созданное при эндоскопической папиллосфинктеротомии холедоходуоденальное соустье в просвет двенадцатиперстной кишки.The solution to the problem is that when performing a laparotomy for retroperitoneal perforation of the duodenum after endoscopic papillosphincterotomy, standard cholecystectomy, choledochotomy, choledocholithotomy are performed, and the duodenum is mobilized according to Kocher. Способ выполняют следующим образом. При выполнении лапаротомии по поводу забрюшинной перфорации 1 двенадцатиперстной кишки после эндоскопической папиллосфинктеротомии выполняют стандартную холецистэктомию, холедохотомию, холедохолитотомию, мобилизуют двенадцатиперстную кишку по Кохеру. Оперативное вмешательство заканчивают дренированием холедоха 2 Т-образным дренажем 3 по Керру (фиг.1, 2), причем дренаж 3 подбирают строго в соответствии с шириной дистального отдела желчного протока 2. При установке дренажа вначале вводят верхнюю браншу дренажа, а нижнюю устанавливают через интрапанкреатическую часть холедоха в созданное при эндоскопической папиллосфинктеротомии холедоходуоденальное соустье в просвет двенадцатиперстной кишки 4, что никогда не делается при классическом способе дренирования холедоха. Такой прием позволяет предотвратить вероятность того, что внутрипротоковые части дренажа могут завернуться. В предложенном способе дренаж кроме выполнения своей основной функции - беспрепятственного отведения желчи, выполняет еще и функцию герметичного закрытия перфорационного отверстия. Правильность и герметичность стояния внутрипротоковых частей проверяют контрольной холеграфией. Ушивание стенки протока осуществляют атравматической иглой и нитью «полисорб» (4/0 или 5/0). Ушивание протока производят до дренажа снизу. Дренаж в протоке удерживают 21 день. В течение этого времени больной с первого дня после операции получает сандостатин.The method is as follows. Пример. Больная С., 57 лет, история болезни №3846/Д, поступила 20.10.2005 с диагнозом ЖКБ. Хронический калькулезный холецистит. Холедохолитиаз. Острый билиарный панкреатит. 21.10.2005 ЭРХПГ + ЭПСТ. Через несколько часов после оперативного лечения появились признаки забрюшинной перфорации: боли в животе и подкожная эмфизема на лице и верхней половине туловища. В экстренном порядке выполнена лапаротомия, холецистэктомия от шейки, дренирование холедоха по предложенному способу. Выписана 14.11.05 в удовлетворительном состоянии. Осмотрена через месяц.
