Результат изобретения достигается тем, что в качестве донорского лоскута используют предварительно подготовленный тканевой комплекс на основе лучевого кожно-фасциального лоскута, который пересаживают на стопу, оборачивая фасциальной частью лоскута выбранные суставы пальца стопы (межфаланговые и (или) плюснефаланговый) с наложением анастомозов «конец в конец» на лучевую артерию лоскута и тыльную артерию стопы, а также на соответствующие комитантные вены. 1. Подготовка стопы. Перед операцией определяют пульсацию тыльной артерии стопы и маркируют ход артерии. Затем на конечность накладывают жгут. Фигурным разрезом по тыльной поверхности стопы послойно выделяют тыльный сосудистый пучок стопы и подкожную вену. Разрез продлевают дистально на 2 палец, кожу на пальце отсепаровывают циркулярно в области межфаланговых и (или) плюснефалангового сустава.1. Preparation of the foot. Выкраивают Т-образный фасциальный лоскут в нижней трети предплечья с включенными в него лучевыми сосудами, кожным «буйковым» лоскутом размером 2×4 см. После поднятия кожно-фасциального лоскута на проксимальной сосудистой ножке, выделенной на достаточном протяжении, и перфузии его выполняют перевязку и пересечение сосудистой ножки. Необходимую длину ножки измеряют на реципиентной кисти от поврежденного сустава до реципиентных сосудов (лучевой артерии на уровне запястья). Затем лоскут переносят на стопу. 3. Формирование сосудистой ножки донорского сустава. Лоскут укладывают на стопе так, чтобы дистальная фасциальная часть лоскута была проведена под кожей на уровне межфаланговых и (или) плюснефалангового суставов 2-го пальца в ранее сформированном туннеле, обернута вокруг сустава и подшита на себя (Фиг.1). При этом пальцевые сосуды по медиальной стороне пальца включают в лоскут и сохраняют их суставные ветви. Латеральные пальцевые сосуды сохраняют для кровоснабжения пальца. «Буйковый» кожный лоскут укладывают в рану на пальце и подшивают к краям раны. Сосудистую ножку лоскута выводят к месту реципиентных сосудов тыльного пучка стопы. Затем с использованием микрохирургической техники накладывают анастомозы между лучевой артерией лоскута и реципиентной тыльной артерией стопы «конец в конец». Вены лоскута анастомозируют «конец в конец» с венами на стопе. Рану ушивают и дренируют. Иммобилизацию осуществляют гипсовой лонгетной повязкой в течение 3 недель.3. 2. Выделение суставов пальца стопы на сформированной сосудистой ножке. На тыльной поверхности стопы определяют по пульсации и отмечают ход сформированной сосудистой ножки, накладывают жгут. Разрез выполняют по старому рубцу и окаймляющий разрез в основании 2 пальца с треугольным лоскутом на подошвенной поверхности. Мобилизуют сосудистый пучок ранее пересаженного комплекса тканей от основания пальца на протяжении. Выполняют остеотомию второй плюсневой кости, поднимают палец на сформированной сосудистой ножке. Снимают жгут и оценивают его кровоснабжение. Сосудистую ножку необходимой длины перевязывают и пересекают. Палец переносят на кисть. Рану на стопе ушивают. Выделенный палец со стопы подготавливают к пересадке: кожу по подошвенной поверхности рассекают и отсепаровывают до боковых поверхностей пальца, удаляют ногтевую фалангу, экзартикулируя в дистальном межфаланговом суставе, суставные поверхности резецируют. Выполняют остеотомию основной фаланги с целью разделения межфаланговых и (или) плюснефалангового суставов, не разобщая их кровоснабжения за счет сформированной сосудистой ножки.2. Сосудистую ножку выводят к месту реципиентных сосудов. Затем с использованием микрохирургической техники выполняют шов сосудов. Рану ушивают и дренируют. Иммобилизацию осуществляют гипсовой лонгетной повязкой в течение 5 недель.3. Transplantation of prepared joints. Пациент Т., 39 лет, ист.болезни №1689, травма фрезой за 4 месяца до госпитализации. В результате у пациента дефект 3, 4 пястных костей и пястно-фаланговых суставов левой кисти, застарелое повреждение сухожилий разгибателей 1-5 пальцев, дефект мягких тканей тыльной поверхности кисти, замещенный ротационным тыльным лоскутом предплечья. Дефект 3, 4 пястно-фаланговых суставов заместили предварительно подготовленными суставами 2 пальца стопы (проксимальным межфаланговым и плюснефаланговым). Также была выполнена транспозиция короткого лучевого разгибателя запястья в позицию длинного разгибателя 1 пальца. Через 6 месяцев была выполнена аллопластика сухожилий разгибателей 2-5 пальцев.Patient T., 39 years old, source of illness No. 1689, injury with a milling cutter 4 months before hospitalization.
