Abstract
Description
The present invention relates to medicine, namely to neurology, traumatology, neurosurgery. It can be used in the treatment of acute head injuries.
The number of victims of traumatic brain damage is increasing every year. Traumatic brain injury accounts for half of all fatal injuries.
Traumatic brain injury is accompanied by the accumulation of toxic metabolites in the cerebrospinal fluid (CSF) resulting from the destruction of brain tissue and hemorrhage into the subarachnoid space. With the appearance in the CSF of medium molecular oligopeptides, oxyhemoglobin, bilirubin and serotonin, disturbances in the tone of intracerebral vessels and brain ischemia are associated.
Studies on the medical, scientific and patent literature have revealed various methods of treating traumatic brain injury.
A known method of treatment of acute craniocerebral trauma / W. Q. Neurosurgery named after Burdenko. - 1994, No. 4, pp. 18-25 /, which consists in the correction of intracranial pressure, cerebral edema, the prevention of convulsive syndrome under the dynamic observation of neuroimaging methods. Osmodiuretics (mannitol, rheoglumin), barbiturates (sodium thiopental, phenobarbital), glucocorticoids (dexazone) are used as treatment. This method has disadvantages: long treatment periods, low efficiency, high cost of a course of treatment, high risk of death.
РФ №2252023).There is a method of treating acute traumatic brain injury, which consists in the fact that from the first hours of treatment, intravenous drip of 2-ethyl-6-methyl-3-hydroxypyridine succinate is carried out in a dose of 200 mg 2 times a day for severe and 1 time per day 200 mg for moderate to mild injuries (US Pat. RF No. 2252023).
There is a method of treating residual neurological manifestations after brain injuries (ed. St. USSR No. 1560203) by including in the complex treatment of hyperbaric oxygenation. The disadvantage of this method is its low efficiency in terms of restoring the functional activity of brain cells in the early post-traumatic period. The method itself is traumatic, the treatment is long, a large number of complications.
A known method for the treatment of traumatic brain injury by infusion of drugs into the pool of the internal carotid artery. In this case, a catheter is installed in the occipital artery and is carried out below the bifurcation of the carotid artery (US Pat. RF No. 2201261). The known method has a number of significant drawbacks: the difficulty of administering drugs, the possibility of complications associated with damage to the carotid artery and nearby anatomical formations.
Since the basis of mental and neurological disorders in organic brain damage is the defeat of the morphological structures of the latter, it seems promising to use surgical treatment with stem cell transplantation for the treatment of this category of patients, which can migrate to the lesion and directionally differentiate into neurons, astro-oligodendroglia. The basis for such operations is the immunological privilege of the brain, due to which the transplanted embryonic allograft is not rejected.
The closest way to treat a brain contusion is a method consisting in conducting a ventricular puncture with the introduction of a biotransplant of nerve stem cells (100 million cells in a volume of 4 ml) (US Pat. RF No. 2191388). As a biograft use freshly obtained cell culture or after cryopreservation. As a cryoprotectant, dimethyl sulfoxide is used. The operation is very complex and traumatic, the formation of teratoma and teracarcinoma is possible. And besides, a lethal outcome is possible.
The purpose of the invention: improving the effectiveness of treatment by minimizing brain damage and restoring the structure of brain tissue, reducing treatment time, increasing indications for the use of the method, reducing traumatic manipulation.
4-5·106.This goal is achieved through the use of a method of treating brain contusion by introducing stem cells into the human body. In this case, immediately after a brain injury, an intravenous saline solution containing activated macrophages in the amount of 0.5-1 · 10 6 is administered, then 3-4 days after the injury, physiological saline solution containing mesenchymal stem cells in the amount of 5-10 · is administered sequentially 10 6 and intradermal physiological solution containing mesenchymal stem cells in an amount of 4-5 · 10 6 .
The result is achieved through the use of transplantation of activated autologous macrophages to minimize brain damage and re-transplantation of autologous or allogeneic mesenchymal stem cells to restore the structure of brain tissue.
The authors experimentally proved that intravenous administration in the first hours after mechanical damage of their own macrophages, activated by incubation with a peripheral nerve fragment or extract, leads to a significant decrease in the degree of tissue damage, which is reflected in a decrease in the number of cells dying by apoptosis or necrosis. In addition, a rough glial scar does not form that interferes with the restoration of nerve connections. If necessary, repeated intravenous administration of autologous or allogeneic mesenchymal stem cells with an interval of 4 weeks (5-6 injections in total) leads to a gradual restoration of the structure and function of the affected area of the brain. Cells penetrate the affected area, are subjected to homing there, and some of them differentiate into neurons and glia. Intradermal administration of allogeneic mesenchymal stem cells isolated, for example, from the amnion or Warton cord jelly, leads to the activation of regeneration and vasculogenesis and general stimulation of the body.
The treatment method is as follows. Immediately after a brain contusion, an intravenous saline solution containing activated macrophages in an amount of 0.5-1 · 10 6 is administered, then no sooner than 3-4 days after the injury, a physiological saline solution containing 5-10 mesenchymal stem cells is administered sequentially · 10 6 , and intradermal saline containing mesenchymal stem cells in an amount of 4-5 · 10 6 . Intradermal administration of mesenchymal stem cells is performed on the same day as intravenous administration.
