В патогенезе острого гематогенного остеомиелита большое значение имеют нарушения системы антиинфекционной резистентности организма. При этом важную роль играет возникновение аутоиммунных процессов в организме, что в значительной степени ухудшает прогноз заболевания, повышает вероятность его хронизации. В соответствии с этим необходимо на ранней стадии заболевания выявить наличие или отсутствие нарушенных звеньев иммунной системы, своевременно провести дифференцированную иммунокорригирующую терапию с целью подавления аутоагрессии, что очень важно при таком тяжелом заболевании, как остеомиелит, особенно у детей.In the pathogenesis of acute hematogenous osteomyelitis, disorders of the body's anti-infectious resistance system are of great importance. При обследовании 75 больных обнаружено, что у больных с острым гематогенным остеомиелитом уже в начале заболевания выявляются антитела к двуспиральной ДНК (АТдДНК) и антинейтрофильные антитела. Причем среди антинейтрофильных антител чаще всего определяются антитела к усиливающему бактерицидное действие белку (BPI), к эластазе и реже к катепсину G, что отрицательно сказывается на таком важном компоненте защитных сил организма, как фагоцитоз.When examining 75 patients, it was found that in patients with acute hematogenous osteomyelitis, antibodies to double-stranded DNA (ATdDNA) and antineutrophil antibodies were detected already at the beginning of the disease. Р 0,05). Пример 1. Больной Романов Д.А., 1 год, и/б №4293, 2006 г. Диагноз: острый гематогенный остеомиелит правой бедренной кости. При исследовании сыворотки крови больного с помощью иммуноферментного анализа установлено, что в остром периоде уровень АТдДНК составил 16,7 ед/мл и отмечалось наличие антинейтрофильных тел. Через 1 месяц содержание АТдДНК снизилось до 10 ед/мл, антинейтрофильные антитела не определялись. Все это свидетельствует о благоприятном течении заболевания и отсутствии хронизации.Example 1. При исследовании сыворотки крови в острой стадии заболевания выявлен повышенный уровень АТдДНК до 30 ед/мл, а также наличие антинейтрофильных антител. Через месяц уровень АТдДНК возрос до 90 ед/мл и сохранялось наличие антинейтрофильных антител. Эти данные свидетельствуют о присоединении аутоиммунного компонента воспалительного процесса и о большой вероятности хронизации остеомиелита. У этого ребенка через три месяца открылся гнойный свищ в области послеоперационного рубца и произошел патологический перелом правой бедренной кости, что потребовало прибегнуть к наложению наружного стержневого аппарата для остеосинтеза.Example 2.
