Abstract
Description
The invention relates to the field of ophthalmology medicine.
Polymer hydrogels are widely used as drug carriers for the treatment of various eye diseases. Hydrogel binding of the drug provides not only immobilization, but also controlled release during use. A typical example of therapeutic hydrogels is soft contact lenses, impregnated with a medicine and used to treat eye diseases. Often, antibiotics of various nature are used as therapeutic agents. The form of administering the drug to the eye, unlike eye drops introduced by the pipette, allows the antibiotic to be delivered to the treatment area with virtually no loss, and a prolonged release of the antibiotic from the polymer hydrogel can be arranged. The criterion for the effectiveness of the use of a drug in a soft contact lens is the absorption (sorption) ability of the lens based on the use of different polymer materials, its weight and the amount of time for the drug to be extracted (desorption) from it. To obtain the maximum sorption values of the lens polymers, it is necessary to use soft contact lenses with the largest mass and use solutions of medicinal substances introduced into the lens with the highest concentration.
In ophthalmology, it is known to use soft contact lenses impregnated with a drug; as an injection of a drug into the eye, ophthalmic films are used, consisting of acrylamide polymers or copolymers thereof with certain acrylic and vinyl monomers impregnated with a drug, see M. M. Krasnov "Ophthalmic medicinal films for first aid and specialized care for eye damage, damage to the organ of vision." Proceedings of the Military Medical University named after Kirov, Leningrad, 1984, vol. 214 p. 42-49. An ophthalmic drug film is placed in a conjunctival sac, in which it is wetted with tear fluid, and the polymer is completely dissolved. The resulting solution containing the drug is distributed in the conjunctival cavity and creates a thin transparent film on the surface of the eyeball. Since the dissolution of the ophthalmic drug film occurs gradually, the drug enters the ophthalmic medium over a period of time, acting on the damaged cornea.
The disadvantages of this solution are as follows: when an ophthalmic drug film is inserted into the lower conjunctival arch, a small amount of the drug enters the cornea, dissolving the film with tear fluid is ineffective, the film itself contains a small amount of the drug, i.e. the absorption capacity of the eye film is small, the coefficient of specific binding of the drug to the polymer film is low, there is no bandage effect of the lens, a large loss of a significant amount of the drug with tear fluid. A large amount of medication is required, which is inconvenient for the patient and can have local and general toxic effects.
The closest solution to the proposed one is the design of a soft contact lens consisting of a polymer material and a drug introduced into it, see the book by A. A. Kivaev, E. I. Shapiro “Contact vision correction”, M., publishing house “LDM” Service ”, 200, p. 64-66. The property of hydrophilic polymers allowed the use of soft ophthalmic contact lenses pre-saturated with the drug as a device for administering the drug to the eye. The material of such a lens makes it possible to achieve high adsorption ability with respect to various substances in solutions, to set quite accurately and continuously maintain the most effective concentration of the preparation, for example, polycarpin, hydrochloride. The use of a soft ophthalmic contact lens is effective as protection against mechanical damage to the eye in case of injuries, after surgery, if necessary, the introduction of drugs with severe side effects, or with the introduction of toxic drugs.
The disadvantages of certain soft eye contact lenses are as follows: the selection of polymeric materials with low sorption, the coefficient of specific binding of the drug to the polymer material of the lens is not effective, there is no quantitative ratio of the mass of the drug to the mass of the lens material, which leads to an ineffective therapeutic effect of eye diseases.
The technical result of the proposed solution is to increase the coefficient of specific binding of the drug to the lens material (hydrogel) and to increase the sorption and desorption of the lens material soaked in the selected drug for the treatment of infectious diseases.
This result is achieved in that in a soft contact lens made of a polymer and impregnated with a drug made of a polymer material 2-hydroxyethyl methacrylate with a mass of 25.5 mg at a water content of 38%, a drug from the fluoroquinolone series - levofloxacin with a mass of 0.035 is used mg per 1 mg of dry polymer, with a concentration of 0.5% in an aqueous solution and desorption of levofloxacin from the lens, comprising 40-80 minutes.
The invention is expressed in the aggregate of essential features sufficient to achieve the technical result provided by the invention.
An essential feature of the invention, which coincides with the features of the prototype, is: A - the lens material is made of a polymer impregnated with a drug.
The salient features of the proposed device are: B - in a lens made of a polymer material 2-hydroxyethyl methacrylate with a mass of 25.5 mg at a water content of 38%, a drug from the fluoroquinolone series is used - levofloxacin with a mass of 0.035 mg per 1 mg of dry polymer, with its concentration in an aqueous solution of 0.5% and with the desorption of levofloxacin from the lens, component 40-80 minutes.
The soft contact lens is made of a synthetic polymer, the material of which is used 2-hydroxyethyl methacrylate 1, impregnated with the drug 2 from the fluoroquinolone series of levofloxacin. To obtain maximum sorption values, it is necessary to use a soft contact lens with the largest mass and use antibiotic solutions with the highest concentration. To quantify the binding of an antibiotic to a hydrogel due to specific interactions and, therefore, one of the criteria for the high efficiency of producing a polymer hydrogel is the coefficient “K” of specific binding of the antibiotic to a hydrogel. When searching for the optimal polymer hydrogel-antibiotic combination, systems that are characterized by specific interactions, such as the formation of hydrogen bonds, ionic bonding, and the appearance of donor-acceptor complexes, should be selected. This coefficient "K" is equal to K = C / C l - , where C is the experimentally determined amount of antibiotics (mg) per 1 mg of dry polymer; With l - the concentration of the antibiotic in the solution.
Сл=CwА/(100-Cw) 100, где Cw - влагосодержание гидрогеля в растворе, А - сорбция антибиотика в расчете на 1 мг гидрогеля.With l = C w A / (100-C w ) 100, where C w is the moisture content of the hydrogel in solution, A is the sorption of the antibiotic per 1 mg of hydrogel.
К=С(100-Сw)А-1Сw -1100 - коэффициент специфического связывания лекарственного препарата материалом линзы (гидрогелем).K = C (100-C w ) A -1 C w -1 100 is the coefficient of specific binding of the drug to the lens material (hydrogel).
The weight of the polymer of 2-hydroxyethyl methacrylate was chosen 25.5 mg, with a water content of 38%, the drug levofloxacin with a mass of 0.035 mg per 1 mg of dry polymer with a concentration in aqueous solution of 0.5% and s was used as an antibiotic from the fluoroquinolone series desorption of levofloxacin from the lens, component 40-80 minutes.
So, it was noted that the hydrogel polymer 2-hydroxyethyl methacrylate is active in complexation with fluoroquinolones, the coefficient of specific binding of the drug to the lens material “K” is the largest and is 11.42, high sorption, which is 0.90. These results make it possible to reasonably choose the optimal type of hydrogel for a particular antibiotic. A larger amount of the drug allows you to increase the time of its exposure to the cornea susceptible to the disease, in particular, infectious diseases with keratitis. The release time of half the amount of levofloxacin bound by the lens in 100 ml of isotonic sodium chloride solution at 37 ° C is 40 '. Thus, a uniform and prolonged release of the antibiotic is achieved within an hour.
Using the device “Soft contact lens” in comparison with the prototype allows to increase the effectiveness of eye treatment by increasing the sorption and desorption of the lens material, as well as the specific binding coefficient of the drug with hydrogel due to the fact that in the lens made of the material polymer 2-hydroxyethylmethacrylate with a mass of 25, 5 mg with a water content of 38% in it, a drug from the fluoroquinolone series — levofloxacin with a mass of 0.035 mg per 1 mg of dry polymer, with its concentration in water was used solution of 0.5% and with desorption of levofloxacin in the lens, component 40-80 minutes. In addition, such a soft eye contact lens is a good bandage for the eye. Using such a lens, it is possible to administer a sufficient amount of the drug into the eye and to carry out a sufficient amount of time for gradual exposure. So it is possible to treat epithelial-endothelial dystrophy of the cornea, kerato-conjunctivitis of various origins, non-healing corneal ulcers. The manufacturing technology of such a polymer lens with the introduction of a drug into it is well developed. Such an ophthalmic lens, made of the proposed material and saturated with levofloxacin, has been tested in medical practice. The proposed soft contact lens can be recommended for use in the treatment of eye diseases, in particular keratitis.
