Способ осуществляют следующим образом.The method is as follows.
После забора цельного раневого отделяемого к нему добавляют 3-5 мл физиологического раствора, гомогенизируют, центрифугируют 5-7 минут при 1500 об/мин на центрифуге ОПн - 3 для осаждения крупных частиц раневого детрита. Затем супернатант засевают шпателем в количестве 0,1-0,2 мл на плотную питательную среду, например кровяной агар. Спустя 10 минут на среду наносят 0,01-0,05 мл жидкого комбинированного бактериофага, например «Секстафаг», выпускаемый ФГУП НПО «Микроген» МЗ РФ филиал «Пермское НПО «Биомед». Посевы инкубируют при температуре 37°С в течение 18-24 часов. Пример 1. Больной Б-ов, 25 лет, поступил в клинику с диагнозом: острый панкреатит, тяжелое течение, инфицированный панкреонекроз. Забрюшинная флегмона слева. До поступления был оперирован в районной больнице, где выполнена лапаротомия, санация и дренирование сальниковой сумки, сформирована бурсооментостома. В послеоперационном периоде отмечалось обильное гнойное отделяемое из бурсооментостомы. К взятому цельному раневому отделяемому из бурсооментостомы (0,2 мл) добавили 3 мл физиологического раствора, произвели гомогенизацию и центрифугирование при 1500 об/мин в течение 5 минут для осаждения крупных частиц детрита. Затем супернатант засеяли шпателем в количестве 0,1 мл на плотную питательную среду. Спустя 10 минут на среду нанесли 0,05 мл жидкого комбинированного бактериофага. Посев инкубировали при температуре 37°С в течение 18 часов. При оценке результатов отметили отсутствие роста в зоне нанесения бактериофага, что свидетельствовало о чувствительности микрофлоры к жидкому комбинированному бактериофагу. После этого в комплекс лечения сразу был включен жидкий комбинированный бактериофаг в виде ежедневного промывания бурсооментостомы и наложения повязок с ним. На 5-е сутки отмечалась нормализация температуры тела и снижение лейкоцитоза. При бактериологическом исследовании раневого отделяемого выделены культуры Pseudomonas aeruginosa и Klebsiella spp., чувствительные к соответствующим видовым бактериофагам и к жидкому комбинированному бактериофагу. Микробиологический мониторинг (7-е сутки) свидетельствовал о снижении уровня микробной обсемененности бурсооментостомы в 10 раз. К 15-м суткам появились признаки очищения бурсооментостомы и отмечалось дальнейшее снижение концентрации микроорганизмов в гнойном отделяемом.Example 1. When evaluating the results, there was a lack of growth in the area of application of the bacteriophage, which indicated the sensitivity of microflora to the liquid combined bacteriophage. After that, the liquid combined bacteriophage was immediately included in the treatment complex in the form of daily washing of the burso-omentostomy and application of dressings with it. On the 5th day, normalization of body temperature and a decrease in leukocytosis were noted. During bacteriological examination of the wound discharge, cultures of Pseudomonas aeruginosa and Klebsiella spp., Sensitive to the corresponding species bacteriophages and to a liquid combined bacteriophage, were isolated. Microbiological monitoring (day 7) indicated a 10-fold decrease in the level of microbial contamination of the bursomentostomy. By the 15th day, signs of purification of the burso-omentostomy appeared and a further decrease in the concentration of microorganisms in the purulent discharge was noted.

Проведена гомогенизация и центрифугирование (7 минут, 1500 об/мин) для осаждения крупных частиц детрита. В дальнейшем супернатант засеяли шпателем в количестве 0,2 мл на плотную питательную среду. Через 10 минут на среду нанесли 0,01 мл жидкого комбинированного бактериофага. Посев инкубировали 24 часа при температуре 37°С. В зоне нанесения жидкого комбинированного бактериофага отметили отсутствие роста бактерий (наличие стерильного пятна). Сразу в комплекс лечения включили жидкий комбинированный бактериофаг в виде ежедневной обработки раны и наложения повязок с ним. По результатам проведенного классического бактериологического исследования из раневого отделяемого выделена культура Staphylococcus epidermidis, чувствительная к стафилококковому бактериофагу и жидкому комбинированному бактериофагу. К 3 суткам согласно клиническим и бактериологическим критериям рана очистилась. Отмечался рост грануляций и краевая эпителизация.Example 2. By 3 days, according to clinical and bacteriological criteria, the wound cleared. Granulation growth and marginal epithelization were noted.
Положительный эффект: предлагаемый способ экономичен, сокращает расход питательных сред в 2-6 раз, не требует сложного подготовительного этапа, менее трудоемок. Способ может применяться как экспресс-метод в любых лечебных учреждениях. Применение способа дает выигрыш во времени для начала целенаправленной бактериофаготерапии, так как исследование занимает не более 18-24 часов по сравнению с 72-96 часами в способе-прототипе.Positive effect: the proposed method is economical, reduces the consumption of nutrient media by 2-6 times, does not require a difficult preparatory stage, less time-consuming.
