Abstract
Description
The invention relates to psychology, namely to medical psychology, can be used in special psychology, neurology and psychiatry.
Г. В. В. Л. All researchers recognize the role of perinatal dysfunction in mental abnormalities and impaired adaptation of children in the future (K.A.Semenova, I.A. Skvortsov, O.V. Khaletskaya, Yu.I. Barashnev and A.E. Litsev, etc.) . However, in children with non-severe perinatal lesions of the nervous system in the subsequent stages of life, the role of early brain dysfunctions is underestimated, there is no objective anamnestic information about them, and the severity of clinical symptoms is leveled. The establishment of cause-effect relationships of problems of mental development with early brain damage is often not carried out, and violations are associated with constitutional features, development conditions, etc., thus, adequate medical treatment and correctional measures are not carried out in a timely manner. All this makes it relevant to develop appropriate clinical and psychological diagnostic methods (E.G. Simernitskaya, T.V.Akhutina, A.V. Semenovich, M.L. Dunaikin).
A known method for diagnosing deviations of the neuropsychic development of children with the determination of functional insufficiency of certain areas of the brain by recording indicators of higher brain functions (acoustic, visual and tactile gnosis, memory, attention, speech, writing, reading, praxis, intellectual activity, emotional-volitional sphere) , including the presentation of test material from a set of neuropsychological samples. The method uses bulk diagnostic material (cubes, pictures, toys, etc.). After testing, a qualitative and quantitative assessment of the results of the tests is carried out (L. S. Tsvetkova. Methods of diagnostic neuropsychological examination of children. M., 1997. - p.24-74). The disadvantages of this method are the inability to determine the specifics of early (perinatal) brain damage, the duration of the examination (at least 1.5 hours per child), the interpretation of the data requires a high special (neuropsychological) qualification.
A known method for determining the functional immaturity of the brain in children 5-7 years old, including testing and analysis of its results, characterized in that they determine the profile of the lateral organization of the brain and with the profile of the lateral organization "pure right-handed" or "pure left-handed" determine the functional immaturity of the parietal -occipital departments of the brain, with the profile of the lateral organization "right-handed" determine the functional immaturity of the fronto-anteroposterior departments, with the profile of the lateral organization "le oruky "- immaturity of the parietal, and / or upper temporal, and / or parietal-temporal parts of the brain, with the profile of the lateral organization" ambidextr ", additionally take into account whether the child had a history of perinatal encephalopathy and in the presence of transferred perinatal encephalopathy determine the functional immaturity of the posterior frontal parts of the brain (Dzyatkovskaya E.N., Pulyaevskaya O.V. A method for determining the functional immaturity of the brain in children 5-7 years old. Patent RU 2225162 C1). The disadvantage of this method is the lack of analysis of the psychological characteristics of the course of mental processes; the method does not reveal the fact of early brain damage.
A known method for the diagnosis of mental state (S.Ya. Rubinshtein. Experimental methods of pathopsychology and the experience of their use in the clinic (Practical Guide) - M .: Medicine, 1970). According to this method, the subject is presented with a plot picture and asked to tell what is drawn on it. According to his story, ingenuity, the ability to distinguish the essential from the details, as well as the emotional response caused by the plot of the picture (p. 130-131) are evaluated. In this method of interpretation of plot pictures, there is no comprehensive assessment of mental processes; the structure and nature of their disturbances are not determined.
- 2005. - с.119-122).There is a method of complex assessment of mental state, including the interpretation of the subject plot picture with the assessment of mental abilities and emotional response, characterized in that the subject’s interpretation of the plot picture is evaluated in five areas of mental response: attention, visual gnosis, speech, thinking, emotional and personal sphere, symptoms their disorders are entered into a specially developed protocol and, based on the results of the study, make a conclusion about the structure and nature of the violation of mental functions th (Dunaikin ML, Brin IL Comprehensive assessment of mental processes in the interpretation of the plot picture. "Structural, functional and neurochemical laws of asymmetry and plasticity of the brain" (Collection of articles of the State Research Institute of the Brain RAMS. - 2005. - p.119- 122).
The disadvantage of this method is the lack of information about the features of mental disorders and the predominant dysfunction of one of the cerebral hemispheres in children 6-10 years old with the consequences of non-severe perinatal lesions of the nervous system.
The objective of the invention is to develop a method of clinical and psychological diagnosis of the effects of non-coarse perinatal lesions of the nervous system and the predominant dysfunction of one of the cerebral hemispheres in children 6-10 years old.
Поставленная задача решается тем, что разработан способ клинико-психологической диагностики последствий негрубых перинатальных поражений нервной системы у детей 6-10 лет, включающий интерпретацию испытуемым сюжетной картинки с оценкой пяти сфер психического реагирования: внимания, зрительного гнозиса, речи, мышления, эмоционально-личностной сферы по симптомам их нарушений, занесенным в специально разработанный протокол, с заключением о структуре и характере нарушения психических функций, причем при нарушении вступления в задание, отвлечении от основной линии сюжета, недоучете деталей; нарушении гнозиса, затруднении построения развернутого рассказа, своеобразии интерпретации сюжета, изменении фона эмоционального реагирования, усилении эмоциональной напряженности в ходе исследования, снижении эмоциональной синтонности по отношению к исследователю; наличии установочных тенденций при интерпретации эмоционального содержания картинки, затруднении интерпретации эмоционального содержания сюжета, наличии утрированных стратегий деятельности диагностируют перинатальное поражение мозга; причем при замедленном включении в задание, склонности к детализации, превалировании нарушений лицевого гнозиса над предметным, лаконичности, недостаточности интонационно-мелодического оформления речи, дефектах звукопроизношения и просодики, ситуационно обусловленном нарушении разборчивости речи, своеобразии интерпретации с резонерскими тенденциями, приглушенности эмоционального реагирования, склонности к протестным реакциям при нарастании напряженности в ходе исследования, выраженном снижении синтонности по отношению к исследователю с игнорированием его эмоциональных реакций или отгороженностью, преобладании агрессивных тенденций в трактовке сюжета и "формальной" стратегии деятельности определяют преимущественную дисфункцию правого полушария мозга;The problem is solved by the fact that a method of clinical and psychological diagnosis of the consequences of non-severe perinatal lesions of the nervous system in children 6-10 years old has been developed, including the subject’s interpretation of the plot picture with an assessment of five areas of mental response: attention, visual gnosis, speech, thinking, emotional and personal sphere according to the symptoms of their disorders, recorded in a specially designed protocol, with a conclusion on the structure and nature of the violation of mental functions, and in case of violation of entry into the assignment, treatment for the main plot line, underestimation of details; violation of gnosis, difficulty in building a detailed story, a peculiar interpretation of the plot, changing the background of the emotional response, increasing emotional tension during the study, reducing emotional syntonism towards the researcher; the presence of installation trends in the interpretation of the emotional content of the picture, difficulty in interpreting the emotional content of the plot, the presence of exaggerated strategies for activity, diagnose perinatal brain damage; moreover, with slow inclusion in the task, a tendency to detail, the prevalence of facial gnosis violations over the subject, laconicism, lack of intonational-melodic speech design, defects in sound pronunciation and prosody, situationally caused by a violation of speech intelligibility, a peculiar interpretation with resonant tendencies, muffled emotional response, and a tendency to protest reactions with increasing tension during the study, a pronounced decrease in syntonity with respect to the teacher with ignoring his emotional reactions or fencing off, the prevalence of aggressive tendencies in the interpretation of the plot and the “formal” strategy of activity determine the predominant dysfunction of the right hemisphere of the brain;
при нарушении внимания с импульсивным включением в задание, врабатываемости, повышенной отвлекаемости, выраженном недоучете деталей и соскальзывании на побочные ассоциации, истощаемости, преобладании предметных парагнозий над лицевыми, трудностях планирования высказываний с дефицитом лексических средств, трудностях обобщения и смены гипотезы, склонности к сдерживанию эмоциональных реакций и их лабильности при нарастании напряженности в ходе исследования, превалировании нравоучительных тенденций и сопереживания при интерпретации сюжета, преобладании "неуверенной" стратегии деятельности с "ригидными" и "инсайтными" тенденциями определяют дисфункцию левого полушария мозга.in case of impaired attention with impulsive inclusion in the task, exploitability, increased distractibility, pronounced underestimation of details and slipping to side associations, exhaustion, the prevalence of subject paragnosis over facial, difficulties in planning statements with a deficit of lexical means, difficulties in generalizing and changing the hypothesis, a tendency to restrain emotional reactions and their lability with increasing tension during the study, the prevalence of moralizing tendencies and empathy with interpretation plot, the predominance of an "uncertain" strategy of activity with "rigid" and "insight" trends determine the dysfunction of the left hemisphere of the brain.
1)The method is as follows: the subject is presented with a plot picture, suggesting an ambiguous interpretation of its content, and the subject is asked to tell what is shown in the picture. The methodology for conducting the experiment is traditional, the accepted sets of test patterns are used (S.Ya. Rubinshtein. Experimental methods of pathopsychology and the experience of their use in the clinic (Practical Guide) - M .: Medicine, 1970 - p.130-131). The data obtained are recorded in a specially designed protocol in which the symptoms of violations in the areas of mental response are recorded. (table 1)
Identified violations correlate with the pattern of disorders characteristic of the consequences of perinatal brain lesions, and determine the predominant dysfunction of the right or left hemisphere.
Perinatal brain damage is diagnosed with:
- нарушении вступления в задание (импульсивном или замедленном);- violation of entry into the task (impulsive or slow);
- недостаточной устойчивости деятельности с тенденцией к отвлечению от основной линии сюжета (с повышенной отвлекаемостью или соскальзыванием на побочные ассоциации) и/или детализацией;- insufficient sustainability of the activity with a tendency to distract from the main plot line (with increased distractibility or slipping to side associations) and / or detailing;
- недоучете деталей;- underestimation of details;
- нарушениях предметного и/или лицевого гнозиса;- violations of subject and / or facial gnosis;

Alena M., 8 years old, was consulted about complaints about learning difficulties (memorizing material, missing letters and incomplete letter elements, mastering the multiplication table, etc.), lack of independence, emotional instability. She studies well (without triples in a quarter), but adult help is required. It is known that the girl’s early development was uneventful, learning problems were associated with a family traumatic situation. In the process of clinical and psychological examination using the proposed method revealed the following violations (see Protocol table 2).
The revealed violations are qualified as a consequence of a non-coarse perinatal lesion of the nervous system with predominant dysfunction of the left hemisphere of the brain. Appropriate treatment was prescribed, the workload and daily routine were regulated, recommendations were given on the organization of the child’s activities, taking into account the developmental characteristics of children with perinatal brain damage and left hemisphere dysfunction.
Example 2. Zhenya G., 8.5 years old, was consulted about complaints about fears of the dark, loneliness. The early development of the girl is prosperous, the emergence of fears was associated with the lack of experience of the child being in kindergarten, although in the preschool age she was engaged in music and choreography in studios. Reads from 4 years. He studies in the second grade, manages to “good” and “excellent”, additionally he is engaged in foreign languages, dancing, music and beadwork.
протокол табл.3.).In the process of clinical and psychological examination using the proposed method revealed the following violations (see Protocol table 3.).
The revealed violations are qualified as a consequence of a non-coarse perinatal lesion of the nervous system with predominant dysfunction of the right hemisphere of the brain. Along with psychological recommendations, medication has been prescribed that takes into account the developmental characteristics of children with perinatal brain damage and dysfunction of the right hemisphere.
A significant difference between the proposed solution from the known is that the method allows you to quickly and comprehensively assess the state of mental processes in children with the consequences of non-coarse perinatal lesions of the nervous system, correlate them with the predominant dysfunction of the left or right brain.
The proposed method is compact, short, does not require complex methodological support, special research conditions and can be used to examine people from 6 years of age. The method allows to diagnose disorders of mental processes due to non-coarse perinatal brain damage with the release of predominant dysfunction of one of the hemispheres. The method can be used for rapid diagnosis of mental disorders, adequate assessment of the causes of developmental disorders in children, with dynamic observation in the clinic (for example, to study the medical effects, the effectiveness of correctional and developmental measures, etc.). The users of the proposed method can be psychologists of different specializations, neuropathologists, psychiatrists, defectologists.
