Abstract
Description
The invention relates to medicine, namely to diagnostic transabdominal methods, and can be used in gynecology when examining the pelvic organs, in urology, in the process of accelerated screening ultrasonography.
Known x-ray method of retrograde cystography (Lopatkin N. A. "Guide to urology. - M., 1998, S. 218-219), which produce transurethral contrast of the bladder. However, this method is used in isolation in urology to diagnose diseases of the bladder.
A known method of transurethral catheterization of the bladder for instillation, rehabilitation, as well as emptying the bladder in violation of the urodynamics of the lower urinary tract of various origins.
The well-known "Method of ultrasound examination of the uterus and appendages (Strizhakov A.N., Davydov A.I." Obstetrics and gynecology "- 1990, No. 1, p. 3) with tight filling of the bladder through the anterior abdominal wall. This method is chosen by us as a prototype.
Using this technique can significantly improve the visualization of the uterus and its appendages, as the filled bladder performs a number of functions necessary for ultrasound examination: 1) it is an ideal acoustic window with a high transmission of ultrasound; 2) contributes to the displacement of intestinal loops from the pelvic cavity and, therefore, the elimination of interference arising from them; 3) displays the uterus in the middle position and thereby facilitates its visualization; 4) serves as a standard in determining the internal structure of volumetric formations of the small pelvis. The need to fill the bladder with transabdominal scanning causes difficulties in examining patients with urgent pathology or diseases of the urinary tract, and also causes unpleasant sensations in women; acute intoxication with water is also possible during ultrasound examination of gynecological patients. In addition, the process of filling the bladder requires a certain time, which significantly slows down the time of the diagnostic process. Using only transvaginal visualization access has several limitations. In particular, when the size of the uterus is more than 9 weeks of pregnancy, it is impossible to examine the bottom and cavity of the uterus, the state of the bladder, and the condition of the ovaries, especially in the presence of syntopy, as well as extensive adhesions.
Thus, it is advisable to conduct a comprehensive ultrasound, and transabdominal ultrasound imaging is an integral first stage of a qualitative diagnosis.
The aim of the invention is to accelerate the results of transabdominal sonography and dynamic individualization of the retrograde process of transurethral filling of the bladder.
This goal is achieved by the fact that a gynecological patient in an empty urine state undergoes transabdominal ultrasound imaging through the anterior abdominal wall, on the basis of the anatomical orientation, the size of the uterus and the appendages are judged, then a transurethral sterile catheter of a droplet system with 100-500 ml of physiological saline and 250 ml of uroseptic standard solution of furatsilin, produce retrograde filling of the bladder under dynamic ultrasonic control, regulate the rate of jet injection of fluid into the cavity of the bladder within 1-5 minutes and the individual volume of injected physiological solution until the intestinal loops are completely crowded out, the bladder contrasts and the uterus and appendages are visualized, the mobility and relative position of the bladder relative to the uterus and appendages, the state of the internal the surface of the bladder mucosa and external contours.
The invention "The method of transabdominal ultrasound examination" is new, since it is not known from the level of medicine in the field of screening ultrasonography.
The novelty of the invention lies in the fact that accelerated intensification of the transabdominal stage of ultrasound diagnostics is achieved by means of retrograde transurethral filling of the bladder under dynamic ultrasonic control, which allows to individualize the speed and volume of injected saline with uroseptic, thereby reducing the time of filling the bladder up to 1-5 minutes from the moment the beginning of ultrasound, avoids intoxication with the natural filling of the bladder.
The invention is industrially applicable, as it can be used in healthcare, in medical institutions equipped with ultrasound equipment, in cancer research institutes, oncology dispensaries and other clinical institutions.
The method of transabdominal ultrasound" is as follows.
A transabdominal ultrasound scan is performed through the anterior abdominal wall in a gynecological patient in an unfilled bladder for anatomical orientation on the size of the uterine body and examination of the appendage zone. Next, a transurethral sterile catheter is located in the distal droplet system. The drip system is mounted on a standard tripod and contains 250 ml of physiological saline with 250 ml of uroseptic (standard solution of furatsilin). Then, opening the shut-off valve of the droplet system, retrograde filling of the bladder is performed under dynamic ultrasonic control, thereby regulating the speed of jet injection of fluid into the cavity of the bladder, as well as the individual volume of injected saline solution (from 100 to 500 ml) with uroseptic until the loops are completely crowded out intestines by contrasting the bladder and visualizing the body of the uterus and appendages, as well as the mobility and relative position of the bladder relative to the body of the uterus and idatkov, the state of the inner surface of the bladder mucosa and external circuits.
The technical and economic efficiency of the “Transabdominal ultrasound examination method” consists in the fact that accelerated intensification of the transabdominal stage of ultrasound diagnostics is achieved, dynamic ultrasound control allows you to individualize the speed and volume of injected physiological saline with uroseptic, thereby reducing the bladder filling time to 1-5 minutes with the onset of ultrasound, avoids acute intoxication that occurs with the natural filling of the bladder. In addition, this method can be used for diseases of the urinary system to assess the dynamic mobility of the bladder, its relative position relative to the body of the uterus, which makes it possible to evaluate adhesive-infiltrative changes in paravesical tissue in case of chronic urological, gynecological diseases.
