Abstract
Description
Примером подобных заболеваний могут служить кандидомикозы, развивающиеся как следствие длительного применения антибиотиков широкого спектра действия, что ведет к нарушению соотношений в нормальной микрофлоре человеческого организма и развитию дисбактериоза, а также при снижении защитных сил организма вследствие длительного назначения некоторых препаратов (кортикостероидов, иммунодепрессантов).The invention relates to medicine and the pharmaceutical industry and for the creation of dosage forms containing interferon preparations, as well as antimycotic drugs of the imidazole group (ketoconazole or fluconazole), and can be used to treat combined fungal infections. Candidomycoses that develop as a result of prolonged use of broad-spectrum antibiotics can serve as an example of such diseases, which leads to a violation of the ratios in the normal microflora of the human body and the development of dysbiosis, as well as to a decrease in the body's defenses due to the long-term use of certain drugs (corticosteroids, immunosuppressants).
Логическим продолжением данного препарата стала мазь "Виферон", в которой так же, как и в свечах, кроме основного действующего вещества - генноинженерного человеческого ИФ α-2 - имеются витамины Е и С /6/.Known domestic genetically engineered human interferon alpha-2 (IF α-2), which has a pronounced biological activity in the culture of human cells / 1 /. The resulting drug for the treatment of combined fungal infections is in the form of a soft or liquid form.
The essential features of the proposal are the use of a combination of alpha-recombinant interferon with an antimycotic drug to prevent candidiasis of various etiologies.
According to the literature, it is known that preparations of alpha-recombinant interferon are an effective tool for the correction of intestinal dysbiosis / 3 /. Against the background of the appointment of such drugs, regardless of the activity of the disease, fungi of the genus Candida practically disappear from the intestinal contents, the seeding in feces of Staphylococcus aureus, Klebsiella, E. coli with altered properties is significantly reduced. This property of alpha-recombinant interferon preparations is significantly enhanced by the introduction of ketoconazole (or fluconazole) into the therapeutic agent, which have a direct antimycotic effect.
Recently, attention is increasingly being paid to the fact that the use of antifungal drugs of the imidazole series is recommended in combination with immunostimulants and immunomodulators (due to the damaging effect on the immune system). Thus, their introduction into the dosage forms of alpha-recombinant interferon allows you to get a new result - a decrease in the side effects of antimycotics, an increase in antiviral and immunomodulating activity. The antioxidants that make up the combined therapeutic agent stabilize the membranes of body cells, increasing their resistance to various damaging agents of fungi.
Thus, the creation of a combined therapeutic agent based on dosage forms of alpha-recombinant interferon, which also includes the antimycotic ketoconazole (or fluconazole), allows us to obtain a new result, namely:
1) расширить спектр терапевтического применения, используя комбинированное лечебное средство в тех случаях, когда применение препаратов без антигрибковых агентов мало- или неэффективно;1) to expand the range of therapeutic applications using a combined therapeutic agent in cases where the use of drugs without antifungal agents is little or ineffective;
2) достигать того же уровня противогрибковой активности, как у препаратов-прототипов при уменьшенной дозе антимикотиков (либо получать более высокую эффективность при той же дозе антимикотика);2) achieve the same level of antifungal activity as that of the prototype drugs with a reduced dose of antimycotics (or get higher efficiency with the same dose of antimycotics);
3) to avoid undesirable side effects inherent in the imidazole series drugs due to correction by alpha-recombinant interferon and antioxidants.
In specially conducted studies, it was shown that going beyond the declared values of the components of the therapeutic agent in the direction of decrease causes a decrease in specific activity, and upward leads to a deterioration in the organoleptic properties of the drug without enhancing specific activity. When studying the effect of the components of the therapeutic agent on the antifungal effect of ketoconazole and fluconazole, it was shown that the above components in the declared concentrations did not affect the manifestation of their fungicidal activity by ketoconazole and fluconazole. The possible effect of antimycotics, as well as other components of the drug in selected concentrations on the antiviral activity of human recombinant α-2 interferon in experiments in vitro was also not revealed. When studying the toxic effect of the components of the therapeutic agent on the culture of human fibroblast diploid cells, strain M-27, it was shown that higher concentrations of the components of the therapeutic agent cause toxic effects on the cells.
The effectiveness of a therapeutic agent (for example, rectal and vaginal suppositories) was studied clinically in two groups of patients: candidal bowel dysbiosis (20 people) and vaginal candidiasis (16 people). Clinical anamnestic data were analyzed, microbiological and immunological studies were performed.
Candles were prescribed 1-2 times a day. The harmlessness and reactogenicity of the drug was determined by deviations in the general condition of patients (blood and urine tests, temperature, general well-being). The therapeutic effect was evaluated by the timing of the disappearance of the culture of the fungus from feces and vaginal discharge and the intensity of the clinical manifestations of relapse. Conclusions about the immunomodulating activity of the claimed therapeutic agent were made on the basis of studying the immune status of patients before and after treatment. The timing of treatment was different depending on the type and severity of the pathology.
Example 1. Patient V., 25 years old, complained of pulling pains in the lower abdomen and pathological discharge from the genital tract (whitened curd harkter); low-grade fever. Clinidoid colpitis has been clinically identified. In a laboratory study of a smear of vaginal secretion, individual colonies of Candida albicans were found. Vaginal candidiasis with ureoplasmosis is diagnosed. Treatment with ketoconazole in tablets at the same time as antibiotic therapy was ineffective. The patient recommended vaginal suppositories containing:
Suppositories were administered vaginally 2 times a day for 7 days. On the 2nd day, the fading of pain began, the temperature decreased, by the 5th day of treatment, pathological discharge from the genital tract disappeared. Conducted 7 days after the start of the drug, a culture study on the presence of vaginal secretion of C. albicans in the smear gave a negative result. The absence of clinical symptoms was combined with the normalization of the immune system. The patient was tracked throughout the year. There are no relapses.
Example 2. Patient K., 28 years old. After prolonged use of broad-spectrum antibiotics, there are complaints of nausea, flatulence, and diarrhea. During a cultural study of feces, the fact of the presence of C.albicans fungi in the intestinal microflora in association with staphylococcus and Klebsiella strains with high diagnostic titers was established. Diagnosed with candidal bowel dysbiosis. A rectal suppository containing:
Suppositories were administered rectally 1 time per day for 5 days. The disappearance of subjective sensations in the patient occurred already on the first day of the use of the therapeutic agent. A cultural study carried out on the fifth day from the start of treatment showed the absence of C. albicans fungi in the feces of the patient in association with Staphylococcus aureus. Diagnostic titer of the microorganism Klebsiella sp. after treatment, it corresponded to the normal balance of the microflora that populates the human intestines.
Thus, the data presented indicate the significance of the qualitative and quantitative composition of the claimed therapeutic agent.
Compared with the prototype, the claimed therapeutic agent has higher efficiency and a wider spectrum of action, since the alpha-recombinant interferon included in the composition can reduce the side effect of antimycotics, has antiviral and antibacterial activity. The distinctive properties of the therapeutic agent allow it to be used simultaneously for the purpose of antifungal and immunoprophylaxis in patients with reduced body defenses due to the long-term administration of certain drugs (broad-spectrum antibiotics, corticosteroids, immunosuppressants).
The use of the claimed therapeutic agent in the complex therapy of combined fungal infections makes it highly economical since it allows to achieve the desired therapeutic effect when using one drug (instead of several drugs) containing fewer antimycotics than in the case of the prototype.
