Предлагаемое изобретение относится к медицине, а именно к челюстно-лицевой хирургии, и может быть использовано для лечения привычных вывихов нижней челюсти, дистрофических заболеваний височно-нижнечелюстого сустава (ВНЧС), сопровождающихся вывихом нижней челюсти.The present invention relates to medicine, namely to maxillofacial surgery, and can be used to treat the usual dislocation of the lower jaw, degenerative diseases of the temporomandibular joint (TMJ), accompanied by a dislocation of the lower jaw.
Суставной бугорок является структурным элементом ВНЧС вместе с суставной ямкой, головкой нижней челюсти, внутрисуставным диском, суставной капсулой. Для фиксации трансплантата использовал проволоку или винт. А.А.Ильин проводил остеотомию по основанию суставного бугорка со стороны переднего его ската с последующим смещением костного фрагмента вниз и введением в образовавшийся диастаз соответствующего размера аллотрансплантата. Трансплантат фиксировал за счет выпиливания паза трапециевидной формы в месте остеотомии кости. Способ осуществляют следующим образом.The method is as follows.
Перед проведением операции больному проводят линейную и/или компьютерную томографию и измеряют высоту и ширину суставного бугорка. Операцию начинают с обнажения суставного бугорка 1, выделения наружной и передней части суставной капсулы предушным доступом. Отслоив надкостницу, на вершине суставного бугорка 1 фрезой или цилиндрическим бором создают плоскую площадку 2. Фрезой моделируют костный аллотрансплантат 3 округлой формы с плоским основанием. Диаметр основания аллотрансплантата 3 должен соответствовать диаметру подготовленной на вершине суставного бугорка 1 плоской площадки 2. Высота моделируемого аллотрансплантата не должна быть менее 5 мм и более 10 мм с учетом полученных рентгенологических данных. Подготовленный трансплантат 3 устанавливают плоским основанием на вершине суставного бугорка 1. Бором формируют вертикальный костный канал через трансплантат и прилежащую к нему часть суставного бугорка, трансплантат 3 фиксируют винтом 4, установленным по вертикальной оси суставного бугорка. При введении винта 4 его «шляпку» погружают в кость. Наличие привычного вывиха нижней челюсти у данной пациентки обусловлено небольшой высотой суставных бугорков. Больной было проведено хирургическое лечение, заключающееся в увеличении высоты правого суставного бугорка костным аллотрансплантатом округлой формы с плоским основанием и высотой 6 мм с фиксацией его винтом по вышеописанному способу. После создания плоской площадки на вершине суставного бугорка его высота составила 2 мм. В послеоперационном периоде иммобилизацию нижней челюсти не проводили. Рана зажила первичным натяжением.
