Abstract
Description
The invention relates to medicine, namely to otorhinolaryngology.
The problem of chronic tonsillitis is the most urgent in otorhinolaryngology not only in its prevalence - 15% of the total population and 22.1-40.1% - among children, but in the absence of objective diagnostic criteria for the compensated form with immunological compensation and the decompensated form of chronic tonsillitis with immunological decompensation, which positively resolves the issue of surgical treatment - tonsillectomy.
The state of immunological activity of the lymphoid tissue of the tonsils does not always coincide with the clinical diagnosis of a compensated or decompensated form of chronic tonsillitis. The criteria used in the clinical diagnosis of chronic tonsillitis (size, the presence of adhesions, scars, purulent plugs, symptoms of Giese, Zach, Preobrazhensky, submandibular lymphadenitis, etc.) are not associated with the immunological function of tonsils and cannot indicate compensation or decompensation.
У больных компенсированной формой хронического тонзиллита при исходном количестве лимфоцитов 73±4 после приема преднизолона - увеличение до 207±10, увеличение миграции лимфоцитов в просвет крипты до 304±8% (р<0,01).The prototype of the proposed method, the authors propose a laboratory method for diagnosing the state of immunological activity of the tissue of the tonsils by determining the average number of cells of the lymphoid row (small lymphocytes and lymphocytes in the blast transformation stage) in the contents of the mouth of the crypt tonsils in one large square of the Goryaev’s chamber by counting cells in 10 squares (according to 5 in the upper and lower rows of the grid) using a prednisolone sample, in which the migration of shaped elements from the tonsil tissue to the surface increases awn, in the gaps of gaps and crypts; which characterizes the functional activity of the tonsils. Prednisolone stimulates the formation in the cells of a regulatory peptide, vasocarotene, which blocks the enzyme hyaluronidase, resulting in increased barrier function of the histohematological barriers, in particular the permeability of the capillary bed. In patients with a compensated form of chronic tonsillitis with an initial lymphocyte count of 73 ± 4 after taking prednisolone, an increase to 207 ± 10, an increase in the migration of lymphocytes into the crypt lumen up to 304 ± 8% (p <0.01).
У здоровых лиц исходное количество клеток лимфоидного ряда в содержимом крипты 22±2, после приема преднизолона -38±3, увеличение миграции лимфоцитов в просвет крипты - 187±6% (р<0,05).In patients with a decompensated form of chronic tonsillitis without concomitant pathology, before the administration of prednisone, the number of lymphocytes is 5 ± 1, and after - 6 ± 1; increased lymphocyte migration into the crypt lumen up to 117 ± 5% (p <0.05). In healthy individuals, the initial number of lymphoid cells in the crypt content is 22 ± 2, after taking prednisolone -38 ± 3, the increase in lymphocyte migration into the crypt lumen is 187 ± 6% (p <0.05).
Ж. - №2 (9). - С.114-116).With the decompensation of the functions of the tonsils, the number of cells of the lymphoid row in the lumen of the crypts is significantly reduced compared with patients with the compensated form of chronic tonsillitis - more than 12 times and with healthy ones - 4 times, which indicates the absence of any functional reserve in the tonsils (Chernysh A. V., Vikulov VV, Bolotov PN Morphological and functional state of the tonsils. J. Russian otorhinolaryngology ". 2004. - No. 2 (9). - P.114-116).
In the authors' opinion, the method has some drawbacks - the difficulty in carrying it out and interpretation, and hence a low degree of reliability.
The authors propose a method for differential diagnosis of compensated and decompensated forms of chronic tonsillitis by the study of saliva using infrared spectroscopy. Infrared spectroscopy was carried out using an IKAR infrared analyzer on 9 channels for 1 min, after 1 sec. As the test material, saliva in a volume of 5 ml was used. 56 people were examined, including 22 patients with a compensated form of chronic tonsillitis, 24 patients with a decompensated form of chronic tonsillitis, and a control group of healthy individuals -10 people.
The most informative were 1, 2, 3, 4, 5, 6 channels corresponding to the ranges of 3085-2832 cm -1 , 2120-1880 cm -1 , 1600-1535 cm -1 , 1543-1425 cm -1 , 1430-1210 cm -1 and 1127-1057 cm -1 , which determined the average values of the transmittance of infrared radiation.
- 448 с./.For a sample of the patient's saliva, 10 cycles of measuring the transmittance of infrared radiation were carried out in each range for 30 seconds. Thus, a training multivariate sample was formed with 6 variables (ranges) and volume (rows) 560, where each row had a label of one of the states: compensated form of chronic tonsillitis, decompensated form of chronic tonsillitis and control. The sample was analyzed by a PC using the hierarchical decision tree method (classification trees). / Potemkin V.G. The decision trees algorithm is a sequential hierarchical structure consisting of nodes that contain rules, i.e. logical constructions of the form "If ... then ...". The final nodes of the tree are the “leaves” corresponding to the solutions found and combining a certain number of lines (states) of the training sample.
The goal of constructing “decision trees” is to identify significant patterns or systematic relationships between the infrared transmittance (dimensionless value) and the patient’s conditions in the training sample, which can then be applied to new measurements of the saliva sample, thereby diagnosing the patient’s condition. This method combines an effective analytical apparatus for generating solutions with the simplicity of its use and intuitive and easily interpreted end results.
The decision trees method allows predicting whether a particular patient’s saliva sample belongs to one of its conditions: the compensated form of chronic tonsillitis, the decompensated form of chronic tonsillitis and their absence (control). As a result of this procedure, decisive rules are developed:
99,3%.According to these rules, a successful forecast is 99.3%.
Based on the data obtained, the authors concluded that when the average transmittance of infrared radiation in the 3 range is more than 35.9, in the 4 range is more than 57.9, and in the 5 range is less than 54.5, chronic tonsillitis is diagnosed, compensated form; when the value of the average transmittance in the 3 range is more than 32.5, chronic tonsillitis, decompensated form is diagnosed; if the average transmittance in the 3rd range is less than 32.5 and in the 4th range is less than 65.2 - chronic tonsillitis, compensated form is diagnosed; if the average transmittance in the 3rd range is less than 35.9, and in the 1st range is more than 72.8 - chronic tonsillitis, compensated form is diagnosed; if the average transmittance in the 3 range is less than 35.9, and in the 6 range more than 51.7, chronic tonsillitis, compensated form is diagnosed; if the average transmittance in the 3 range is less than 35.9, and in the 2 range more than 79.7, chronic tonsillitis, compensated form is diagnosed; when the value of the average transmittance in the 3 range is less than 35.9, and in the 2 range less than 79.7, chronic tonsillitis, decompensated form is diagnosed. This was used by the authors for the differential diagnosis of compensated and decompensated forms of chronic tonsillitis in patients with a clinically diagnosed chronic tonsillitis.
In addition, the proposed method for the diagnosis of forms of chronic tonsillitis according to the results of a study of the patient’s saliva by infrared spectroscopy is painless fast and can be performed on an outpatient basis.
Clinical examples.
Patient G., examination card No. 29 preliminary clinical diagnosis - compensated form of chronic tonsillitis (complaints of sore throat, perspiration, foreign body feeling, bad breath; history - frequent tonsillitis (1 time per year); tonsils 2 degrees, tuberous , wide gaps, purulent plugs, adhesions with palatine arches, symptoms of Guise, Zach and Preobrazhensky). The study of saliva using the analyzer "IKAR". The average transmittance of infrared radiation was: 1 range of 73.8; 2 range 68.3; 3 range 38.4; 4 range 59.1; 5 range 56.3; 6 range 54.6. We turn to the method of "decision trees", presented graphically in the text above. They always start with the main 3 range, which in our patient is 38.4; we look in the "decision trees" - this corresponds to the 3rd range <= 35.9 then, and from it we go to the 1st range, which for our patient is 73.8, which obliges us to make a graphic image of the "decision tree". This corresponds to the “decision tree” 1 range <= 72.8 then, and this gives us the right to confirm this diagnosis to her patient - chronic tonsillitis compensated form, and recommend conservative treatment. Thus, using the logic of the decision rules developed and the “decision tree” proposed by us, for confirming this diagnosis in a particular patient, it was enough to use the indicators of the average transmittance in only two ranges (3 and 1) out of 6 studied.
Patient A., examination card No. 31, preliminary diagnosis — decompensated form of chronic tonsillitis (complaints of sore throat, tickling, the appearance of purulent “grains” in the tongue, bad breath, weakness, malaise, low-grade fever, joint pain; history - frequent sore throats (1 time per year), frequent acute respiratory infections (up to 4 times a year); enlargement and pain on palpation of the submandibular lymph nodes; tonsils 2 degrees, tuberous, wide lacunae, purulent plugs, subepithelial festering follicles, adhesions with palatine douches symptoms of giza , Zach and Preobrazhensky). The study of saliva using the apparatus analyzer "IKAR". Indicators of the average transmittance of infrared radiation were: 1 range 71.6; 2 range 72.6; 3 range 31.1; 4 range 55.3; 5 range 51.8; 6 range 50.9. We start with the main 3 range, the average transmittance for it in our patient is 31.1. We turn to the “decision tree” proposed by us and see that this corresponds to the 3rd range> 35.9 then, and this dictates us to go to the 1st range, the average transmittance of which in our patient is 71.6, which corresponds to the “decision tree” 1 range> 72.8 then, and this again dictates that we go to range 6, the average transmittance of which in our patient is 50.9, which corresponds to the range of decisions> 51.7 then, according to the decision tree, and this again tells us to go to range 2, the average transmittance of which in our patient is 72.6, which sponds on the "decision tree" 2 range of> 79.7 is, and this gives us the right to confirm the diagnosis in this patient - chronic tonsillitis decompensated form and recommend surgery (tonsillectomy). Thus, for this particular patient, using the logic of the developed decision rules using the “decision tree” that we proposed, indicators of the average transmittance of infrared radiation over four ranges (3, 1, 6, and 2) out of 6 studied were used to confirm this diagnosis. .
Thus, a practical doctor, having diagnosed a chronic tonsillitis to a patient who is clinically ill and additionally examining saliva using an IKAR analyzer, has received data on indicators of average transmittance of infrared radiation over 6 ranges and differentiates the compensated from decompensated form of chronic tonsillitis.
