Проанализирована 31 клиническая история болезни за период с 2000-2002 годы у больных с хронической почечной недостаточностью в терминальной стадии, находившихся на лечении программным гемодиализом в нефрологическом отделении Тюменской областной клинической больницы.We analyzed 31 clinical history of the disease for the period from 2000-2002 in patients with chronic renal failure in the terminal stage who were treated with program hemodialysis in the nephrology department of the Tyumen regional clinical hospital.

Показатели центральной гемодинамики анализировались в двух группах больных (умерших за период с 2000 по 2002 годы и больных с благоприятным исходом за этот же период) - на персональном компьютере с использованием статистической программы EXCEL, а также методом математического компьютерного многофакторного дискриминантного анализа, который реализован в STATGRAPHICS for Windows.Central hemodynamic parameters were analyzed in two groups of patients (who died between 2000 and 2002 and patients with a favorable outcome for the same period) - on a personal computer using the EXCEL statistical program, and also using the mathematical computer multivariate discriminant analysis, which is implemented in STATGRAPHICS for Windows.
Проведенный корреляционный анализ показал, что у диализных больных с терминальной стадией ХПН имеется слабая корреляционная взаимосвязь показателей центральной гемодинамики по неблагоприятному исходу; наиболее значимыми оказались взаимосвязи показателей массы миокарда левого желудочка (mМЛЖ) и диастолического резерва (ДР) с неблагоприятным исходом. Эти показатели и были взяты для расчета дискриминантной функции, позволяющей определить каждого больного к одной из групп: группе живых или к группе с летальным исходом.The conducted correlation analysis showed that in dialysis patients with end-stage chronic renal failure there is a weak correlation between the central hemodynamic parameters for an unfavorable outcome; the most significant were the relationships between the indicators of the left ventricular myocardial mass (m LVM) and diastolic reserve (DR) with an unfavorable outcome. These indicators were taken to calculate the discriminant function, which allows to identify each patient in one of the groups: the living group or the group with a fatal outcome.
Для составления математической модели неблагоприятного прогноза ХПН были взяты два показателя центральной гемодинамики mМЛЖ и ДР в группах больных ХПН живых и с летальным исходом, введены в электронную таблицу STATGRAPHICS. Результаты математического анализа получены с применением линейных дискриминантных функций Фишера, параметры которых определяются в рамках процедуры STATGRAPHICS, они здесь имеют название классифицирующих функций.To compile a mathematical model of an unfavorable prognosis of chronic renal failure, we took two indicators of central hemodynamics, m LVM and DR, in groups of patients with chronic renal failure and living with death, entered into the STATGRAPHICS spreadsheet. Набор дискриминантных функций позволяет отнести любого пациента по значению его показателей mМЛЖ и ДР к той или иной группе. Для этого показатели центральной гемодинамики подставляются в обе формулы. При расчете получаются показатели, принадлежащие конкретному больному. Пациент относится к той группе, для которой найдена наибольшая величина дискриминантной функции.A set of discriminant functions makes it possible to attribute any patient according to the value of his indicators m LVM and DR to a particular group. Пациент относится к группе с летальным исходом, т.к. наибольшая величина дискриминантной функции относится именно к этой группе. Предлагаемое изобретение позволяет отнести конкретного диализного больного с терминальной стадией ХПН к одной из групп: группе живых или группе с возможным летальным исходом в ближайшие 15 месяцев, при этом чувствительность данного способа составляет 75%, специфичность - 80%, прогностичность - 79%.The present invention allows to refer a specific dialysis patient with end-stage chronic renal failure to one of the groups: a living group or a group with a possible fatal outcome in the next 15 months, while the sensitivity of this method is 75%, specificity is 80%, and predictiveness is 79%.
