Изобретение относится к медицине, в частности к клинической лабораторной диагностике, и может быть использовано в различных областях экспериментальной и клинической медицины для оценки состояния тромбоцитов при различных формах патологии и воздействии различных физических и медикаментозных факторов. Предлагаемый метод позволяет оценить чувствительность тромбоцитов к изменяющейся скорости сдвига, обеспечиваемой вязким взаимодействием элементов движущейся среды.The invention relates to medicine, in particular to clinical laboratory diagnostics, and can be used in various fields of experimental and clinical medicine to assess the state of platelets in various forms of pathology and the effects of various physical and medical factors. The proposed method allows to evaluate the sensitivity of platelets to a changing shear rate, provided by the viscous interaction of elements of a moving medium.
Оценка функции тромбоцитов при различных патологических состояниях является важным звеном диагностики и служит основой для разработки эффективных методов патогенетической терапии многих заболеваний. Известно, что одним из регуляторных контуров, обеспечивающих оптимальную адгезивную и агрегационную функции тромбоцитов, является так называемая "сдвиговая регуляция", осуществляемая за счет возникновения мембранного клеточного сигнала при изменении скорости смещения вязких элементов среды при движении форменных элементов в потоке крови [1, 3, 5]. В различных участках сосудистого русла за счет локальных особенностей вазоархитектоники, различий в диаметре сосудов и величине перфузионного давления возникают реальные колебания скорости потока крови и, следовательно, имеют место различные значения скорости сдвига. Важным фактором в восприятии и реализации сдвигового сигнала является состояние самих кровяных пластинок [3, 5, 6].Evaluation of platelet function in various pathological conditions is an important link in the diagnosis and serves as the basis for the development of effective methods of pathogenetic therapy of many diseases. It is known that one of the regulatory circuits that provide the optimal adhesive and aggregation function of platelets is the so-called “shear regulation”, which is carried out due to the emergence of a membrane cell signal with a change in the rate of displacement of viscous elements of the medium during the movement of uniform elements in the blood stream [1, 3, 5]. Due to the local features of vasoarchitectonics, differences in the diameter of blood vessels and the value of perfusion pressure, real fluctuations in the blood flow velocity occur in different parts of the vascular bed and, therefore, different values of the shear rate occur. An important factor in the perception and implementation of a shear signal is the state of the blood platelets themselves [3, 5, 6].
В условиях патологии, например при развитии атеросклеротического процесса, изменяются реологические свойства крови, возникают нарушения ламинарности тока крови вследствие формирования бляшек в просвете сосуда, происходит изменение скоростных режимов кровотока, что естественно отражается на характере сдвиговой регуляции функции кровяных пластинок [4, 7]. Нами впервые предлагается способ количественной оценки эффективности сдвиговой регуляции функции тромбоцитов, включающий определение среднего размера агрегатов и числа объектов, адгезированных на жесткой подложке, в пробе крови одного пациента при низкой (200 с-1) и высокой (1300 с-1) скоростях сдвига и расчет индекса сдвиговой регуляции агрегации (ИСРагр) как отношения значения среднего размера агрегатов при высокой скорости сдвига к его значению при низкой скорости сдвига, и индекса сдвиговой регуляции адгезии (ИСРадг) как отношения среднего числа адгезированных объектов при высокой скорости сдвига к значению данного показателя при низкой скорости сдвига. У пациента по стандартной методике забирается проба цитратной крови (соотношение кровь-цитрат 9:1). Кровь выдерживается при комнатной температуре в течение 40 мин и делится на 2 аликвоты (для исследования при низкой и высокой скорости сдвига). Затем проводится исследование поведения тромбоцитов на жесткой подложке - экстраклеточном матриксе (ЭКМ), представляющем собой комплекс продуктов, вырабатываемых эпителием роговицы быка, и содержащих различные типы коллагена, фибриноген, фибронектин, витронектин, фактор Виллебранда, тромбоспондин, ламинин и гликопротеины [2], или на полистирене. Используются стандартные 4-ячейные планшеты из полистирена, на дно ячеек которых нанесен ЭКМ, либо ячейки без ЭКМ. В ячейку помещаются 200 мкл цельной цитратной крови. Исследования проводятся с использованием прибора "Cone and Plate Analyzer" (CPA), позволяющего исследовать поведение тромбоцитов при различных скоростях сдвига. В приборе используется принцип ротационной вискозиметрии. В ячейки с биоматериалом опускается цилиндр с концевой конусной конфигурацией, благодаря которой между ним и дном ячейки образуется зазор. Вращение цилиндра с заданной скоростью создает необходимые параметры скорости сдвига. Используются две скорости сдвига: низкая - 200 с-1 (являющаяся подпороговой для запуска механизмов сдвиговой активации) и высокая - 1300 с-1 (активирующая функции тромбоцитов). Исследование на приборе каждой аликвоты занимает 2 минуты.A sample of citrated blood is taken from a patient by a standard technique (blood-citrate ratio 9: 1). Standard 4-cell polystyrene plates are used with ECM applied to the bottom of cells, or cells without ECM. 200 μl of whole citrated blood are placed in the well. Studies are performed using the Cone and Plate Analyzer (CPA) instrument, which allows the study of platelet behavior at various shear rates. The device uses the principle of rotational viscometry. A cylinder with an end conical configuration is lowered into the cells with biomaterial, due to which a gap is formed between it and the bottom of the cell. The rotation of the cylinder at a given speed creates the necessary shear rate parameters. Two shear rates are used: low - 200 s -1 (which is the subthreshold for triggering shear activation mechanisms) and high - 1300 s -1 (activating platelet functions). The study on the device of each aliquot takes 2 minutes.

Однако в предлагаемом методе концентрация фактора Виллебранда в двух сравниваемых пробах крови, а также на ЭКМ, является постоянной и, следовательно, изменение ИСР отражает изменение реактивности кровяных пластинок в плане их чувствительности к сдвиговой регуляции (активации).Platelet activation at a high shear rate depends on the reactivity of the blood platelets themselves, as well as on the content of von Willebrand factor in the blood plasma and ECM [3, 5]. However, in the proposed method, the concentration of von Willebrand factor in the two compared blood samples, as well as on an ECM, is constant and, therefore, the change in SRI reflects a change in the reactivity of blood plates in terms of their sensitivity to shear regulation (activation).
Для верификации данного метода нами проведены исследования сдвиговой регуляции функции тромбоцитов в группе здоровых доноров (n=15) мужского пола. Возраст обследуемых - от 18 до 34 лет. Результаты исследований представлены в таблице.To verify this method, we conducted studies of the shear regulation of platelet function in a group of healthy male donors (n = 15). Пример. Донор М. 28 лет. Из локтевой вены получены 5 мл цитратной крови. После 40 минутного выдерживания при комнатной температуре 200 мкл крови помещены в ячейку с ЭКМ, которая установлена в прибор "Cone and Plate Analyser" и проведено исследование при малой (200 с-1) скорости сдвига. Далее препарат окрашен краской май-грюнвальд 2 мин. Затем в другую ячейку с ЭКМ помещена следующая порция (200 мкл) крови и проведено ее исследование при высокой (1300 с-1) скорости сдвига. Препарат окрашен краской май-грюнвальд 2 мин. После подсыхания оба препарата анализировались на имидж-анализаторе. Подсчитывались количество адгезированных объектов (КАО) и средний размер адгезированных объектов (СРО). Получены следующие результаты:Example. Данный тест, благодаря его простоте и быстроте выполнения, может явиться важным дополнительным критерием в оценке функционального состояния тромбоцитов при различных заболеваниях. Кроме того, он может быть использован при исследовании влияния различных фармакологических препаратов и физических факторов на состояние сдвиговой регуляции функции тромбоцитов. При увеличении ИСР делается заключение об усилении (повышении эффективности) сдвиговой регуляции, что является основанием для применения средств, ингибирующих функцию тромбоцитов. Уменьшение ИСР свидетельствует о гипореактивности (понижении эффективности регуляции) кровяных пластинок, что требует применения препаратов, активирующих их функцию.This test, due to its simplicity and speed of execution, can be an important additional criterion in assessing the functional state of platelets in various diseases.
