Изобретение относится к медицине, а именно к способам структурной стабилизации биотканей для протезирования и пластики, преимущественно при изготовлении ксеногенных биологических протезов клапанов сердца, используемых в клинической практике для коррекции клапанных пороков сердца.The invention relates to medicine, namely to methods for structural stabilization of biological tissues for prosthetics and plastics, mainly in the manufacture of xenogenic biological prosthetic heart valves, used in clinical practice for the correction of valvular heart defects.
Структурная стабилизация биопротезов производится с целью получения биологической инертности их ткани (устранения иммуногенности и обеспечения устойчивости к ферментативному лизису). Это достигается путем химической обработки биоткани веществами, способными образовывать прочные поперечные связи с молекулами коллагена.Structural stabilization of bioprostheses is carried out with the aim of obtaining biological inertness of their tissue (eliminating immunogenicity and ensuring resistance to enzymatic lysis). Известен способ структурной стабилизации биотканей, включающий обработку глутаральдегидом (CarpentierA., Lemaigre G., Robert L. et al. Biological factors affecting long-term results of valvular heterografts // J. Thorac. Cardiovasc. Surg. -1969. - Vol. 58, N 4. - P. 467-483). Данный химический агент обеспечивает стерильность, существенно повышает механическую прочность ткани биопротеза и устойчивость к действию протеолитических ферментов. Однако глутаральдегид изменяет физико-химические и механические свойства биотканей. Потеря гидрофильных свойств и девитализация клеток ксеноткани провоцируют образование центров дистрофической кальцификации. Изменение вязкоупругих свойств ткани ксенографта под воздействием глутаральдегида приводит к ухудшению в целом макробиомеханики биопротеза и образованию стресс-деформаций преимущественно в области комиссур (особенно при наличии стента), что также провоцирует развитие локусов тканевой дегенерации и кальцификации. Свободные остатки глутаральдегида, обладающие цитотоксическим воздействием на окружающие ткани больного, усугубляют дистрофическую кальцификацию.A known method of structural stabilization of biological tissues, including treatment with glutaraldehyde (Carpentier A., Lemaigre G., Robert L. et al. However, glutaraldehyde alters the physicochemical and mechanical properties of biological tissues. Loss of hydrophilic properties and devitalization of xenotissue cells provoke the formation of dystrophic calcification centers. Changes in the viscoelastic properties of xenograft tissue under the influence of glutaraldehyde lead to a general deterioration in the macrobiomechanics of the bioprosthesis and the formation of stress deformations mainly in the area of commissures (especially in the presence of a stent), which also provokes the development of tissue degeneration and calcification loci. Free glutaraldehyde residues, which have a cytotoxic effect on the surrounding tissues of the patient, exacerbate dystrophic calcification.
Известен способ консервирования биоткани для протезирования клапанов сердца и сосудов 2-5% раствором диглицидилового эфира этиленгликоля в течение 21 суток (патент РФ №2008767) с целью подавления процессов кальцификации и дополнительной фиксации гепарина с целью повышения резистентности к тромбообразованию. Биоткани, стабилизированные эпоксидами, обладают лучшими вязкоупругими свойствами по сравнению с тканями, обработанными глутаральдегидом. Соотношение массы биоткани к массе раствора должно составлять 1:15-30. По предложенному способу была проведена обработка 65 образцов: 15 корней аорты и 50 створок (5 групп). Для определения степени структурной стабилизации ксеноаортальной ткани использовали весовой анализ. Для этого образцы ткани отмывали, лиофилизировали и взвешивали дважды, до и после обработки коллагеназой. По разнице потерь массы нативных (Δ mnativ) и стабилизированных (Δ mstab) образцов рассчитывали индекс стабилизации (81) по формуле: SI=(Δ mnativ-Δ mstab)/Δ mnativ× 100%. При анализе влияния рН раствора на скорость структурирования ткани (фиг.2 и 3) было выявлено, что в нейтральном растворе (рН 7,4) фиксация и стабилизация материала происходит с одинаковой скоростью и достигает максимальных значений на 4-5 сутки. В щелочном растворе (рН 8,6-9,0) максимальная фиксация и стабилизация наступает раньше - в 1-2 сутки. Обработка биоткани в растворе с рН 10,2 приводила к максимальной стабилизации коллагена в среднем через 12 часов, однако в дальнейшем было отмечено снижение степени стабилизации ткани, объясняемое частичной денатурацией белка под воздействием высокощелочной среды.When analyzing the effect of the pH of the solution on the rate of tissue structuring (Figs. 2 and 3), it was revealed that in a neutral solution (pH 7.4), the fixation and stabilization of the material occurs at the same speed and reaches maximum values for 4-5 days.
