Известен метод определения расположения лоханки при выполнении реконструктивных операций на трансплантате с помощью гидравлического расширения коллекторной системы почки путем предварительного блокирования нефростомического дренажа за 1-2 часа до операции. При этом расширенная лоханка определяется пальпаторно (Трапезникова М.Ф., Казимиров В.Г., Перлин Д.В., Уренков С.Б. Ж. Урология и нефрология, 1997, № 6, с.20-24). Этот метод имеет следующие существенные недостатки: во-первых, сложность пальпаторного определения лоханки через толстый слой грубой фиброзно-измененной ткани и большая вероятность ошибочного определения интимно прилегающих к ней магистральных сосудов, по плотности напоминающих лоханку и мочеточник. Во-вторых, метод неприменим при выполнении операции по поводу некроза мочеточника, когда имеется наружный мочевой свищ, из-за невозможности гидравлического расширения коллекторной системы почки. В-третьих, даже относительно непродолжительное нарушение оттока мочи из трансплантата может привести к возникновению пиеловенозного рефлюкса, а как следствие - к серьезным осложнениям.A known method for determining the location of the pelvis during reconstructive operations on the transplant using hydraulic expansion of the collector system of the kidney by preliminary blocking the nephrostomy drainage 1-2 hours before surgery. Thirdly, even a relatively short violation of the outflow of urine from the transplant can lead to pyelovenous reflux, and as a result to serious complications.

Задачей настоящего изобретения является устранение указанных недостатков за счет создания надежного способа визуализации лоханки, необходимого для ее поиска и выделения, при выполнении реконструктивных операций на мочевом тракте трансплантата, таких как пиелоуретеростомия, уретероуретеростомия, пиеловезикостомия и др., в условиях выраженного фиброзного уплотнения окружающих тканей и отсутствия постоянных анатомических ориентиров. С этой целью предлагается визуализировать лоханку путем локального освещения с помощью световода, проведенного через нефростомический дренаж. Положение рабочей части световода корригируется под контролем флюороскопа. В качестве источника видимого излучения предлагается использовать гелий-неоновый лазер длиной волны 0,63 мкм и мощностью на конце световода 20 мВт. Излучение с указанными характеристиками обладает высокой проникающей способностью через плотные фиброзно-измененные ткани. Дополнительным преимуществом метода является бактериостатический эффект излучения, что также повышает безопасность операции.The objective of the present invention is to remedy these disadvantages by creating a reliable method for visualizing the pelvis, necessary for its search and isolation, when performing reconstructive operations on the urinary tract of the transplant, such as pyeloureterostomy, ureteroureterostomy, pyelovecicostomy, etc., under conditions of pronounced fibrous compaction of surrounding tissues and lack of permanent anatomical landmarks. For this purpose, it is proposed to visualize the pelvis by local illumination using a fiber through a nephrostomy drainage. The position of the working part of the fiber is corrected under the control of a fluoroscope. It is proposed to use a helium-neon laser with a wavelength of 0.63 μm and a power at the end of the fiber of 20 mW as a source of visible radiation. Radiation with the indicated characteristics has high penetration through dense fibro-altered tissues. An additional advantage of the method is the bacteriostatic effect of radiation, which also increases the safety of the operation.

Экстренно госпитализирован 25.09.1991 с жалобами на высокую лихорадку. При обследовании обратило на себя внимание существенное снижение функции почки (креатинин плазмы 0,90 ммоль/л), выраженная лейкоцитурия (до 50-60 в поле зрения). При УЗИ обнаружен выраженный гидронефроз трансплантата. Выполнена пункционная нефростомия. Повторная трансплантация выполнена 16.08.1999 года от трупного донора с ретроперитонеальным фиброзом. Обработка трансплантата сопровождалась техническими трудностями вследствие выраженного рубцового процесса в воротах почки. Функция трансплантата отсроченная с восстановлением диуреза на 27 сутки. При очередном УЗИ 22.09.1999 года обнаружено объемное жидкостное образование у нижнего полюса трансплантата, размерами 5×6 см. Под ультразвуковым контролем образование пунктировано. Биохимический состав полученной жидкости соответствовал моче. Выполнена чрескожная нефростомия, дренирование мочевого затека. При антеградной пиелоуретерографии отмечалась массивная экстравазация контраста в области средней трети мочеточника.
