Сущность изобретения заключается в том, что в способе непрямой магнитно-резонансной лимфографии, включающем интерстициальное введение в зону исследования вещества для магнитно-резонансного контрастирования и выполнение магнитно-резонансной томографии, предлагается предварительно в зону исследования вводить вещество, обладающее гиалуронидазной активностью. При этом в качестве вещества для магнитно-резонансного контрастирования может быть использован парамагнетик, содержащий гадолиний, а в качестве вещества, обладающего гиалуронидазной активностью, может быть использована лидаза. Лидаза может вводиться в количестве 16-32 ед. Применение лидазы широко известно в терапии в качестве лимфотропного средства. (Левицкий А.П., Марусич В.А. Применение препаратов гиалуронидазы для лечения резаных инфицированных ран в эксперименте. Хирургическая и ортопедическая стоматология. Вып.2. 1981. С.55-57.) Фермент гиалуронидаза, входящий в состав лидазы, катализирует процесс расщепления гиалуроновой кислоты - кислого мукополисахарида, являющегося цементирующим веществом соединительно-тканных элементов организма, расширяя поры в стенке лимфатических капилляров и разрыхляя структуру соединительной ткани, что ведет к увеличению проницаемости соединительной ткани. Лимфатическая терапия в хирургии. Изд-во им. Ибн-Сины, 1991. - 238 с.) Вышеописанные свойства лидазы до сих пор использовались только в терапевтических целях. Основными показаниями к применению лидазы в терапии являются контрактуры суставов, рубцы после ожогов и операций, анкилозирующий спондило артрит, гематомы и др. Раствор лидазы (1 мл) вводят в этих случаях вблизи места поражения под кожу или под рубцово-измененные ткани. Инъекции производят ежедневно или через день; курс лечения состоит из 6-10-15 и более инъекций. Лечебный эффект проявляется размягчением рубцов, появлением подвижности в суставах, устранением или уменьшением контрактур, рассасыванием гематом. Известно также использование лидазы в терапии для ускорения всасывания лекарственных веществ, вводимых под кожу и внутримышечно (местные анестетики, мышечные релаксанты, изотонические растворы и др.) Отличие заявляемого способа заключается в том, что лидазу используют в диагностике путем одноразового введения в зону проведения магнитно-резонансной лимфографии непосредственно перед введением парамагнетика, а не систематически в зону места поражения, как при терапии. При этом известное свойство лидазы - способность увеличивать проницаемость соединительной ткани, в том числе и лимфатических сосудов, - используется в новой совокупности признаков и обеспечивает достижение нового (сверхсуммарного) технического результата - повышение разрешающей способности непрямой магнитно-резонансной лимфографии, повышение точности определения (выявления) здоровых и патологически измененных сосудов и узлов лимфатической системы в их взаимосвязи с другими анатомическими образованиями, а также расширение области применения способа, что свидетельствует о неочевидности изобретения.The essence of the invention lies in the fact that in the method of indirect magnetic resonance lymphography, including interstitial introduction into the study area of a substance for magnetic resonance contrast and magnetic resonance imaging, it is proposed to first introduce a substance having hyaluronidase activity into the study area. В заявляемом изобретении предварительное введение в зону проведения непрямой магнитно-резонансной лимфографии вещества, обладающего гиалуронидазной активностью, обеспечивает повышение резорбции парамагнетика в узлы и сосуды лимфатической системы, за счет чего повышает разрешающую способность магнитно-резонансной лимфографии и точности определения (выявления) здоровых и патологически измененных сосудов и узлов лимфатической системы в их взаимосвязи с другими анатомическими образованиями, что, в конечном итоге, приводит к повышению эффективности планирования и проведения онкохирургических вмешательств. Способ осуществляется следующим образом. В зону исследования, куда будет введено вещество - парамагнетик, обладающее парамагнитными свойствами, предварительно вводится лидаза в дозе 16-32 Ед. Затем в эти же участки вводится вещество-парамагнетик, например "Омнискан", из расчета в 70 micromol/kg веса. Магнитно-резонансная томография (предпочтительно - с полем 0,5-1,0 Тл, возможно - с полем 0,2-0,3 Тл) производится через 1,5-2,0 часа, например, с помощью МР-томографа Tomikon S50 фирмы Bruker (Швейцария - Германия), с полем 0,5 Тл. Лимфатические узлы контрастируемого региона представляются хорошо визуализированными на Т1-взвешенном изображении (режимы - MSSE, MSME, t-RARE). Прослеживается внутренняя структура, в том числе - синусная система лимфатических узлов, что позволяет выявить ранние патологические изменения в них.The method is as follows. Для выяснения состояния регионарного лимфатического аппарата больному была проведена непрямая магнитно-резонансная лимфография по предлагаемой методике. За 2 часа до исследования в подслизистый слой прямой кишки, выше зубчатой линии введена лидаза 32 Ед, а затем 12 мл Омнискана. На фиг.1 приведены результаты исследований больного З. по предлагаемому способу: на томограмме белая стрелка указывает на опухоль прямой кишки, черными стрелками обозначены контрастированные лимфатические узлы мезоректума, видны контрастированные лимфатические узлы мезоректума, а метастатические поражения лимфатических узлов не выявляются.Example 1. Пример 2. Больная П., 51 года. У больной был диагностирован рак среднеампулярного отдела прямой кишки. Для выяснения состояния регионарного лимфатического аппарата больной была проведена непрямая магнитно-резонансная лимфография по предлагаемой методике. В подслизистый слой нижней трети прямой кишки из двух точек было введено 32 Ед лидазы и сразу же после этого 10 мл Омнискана (вес больной - 65 кг). Через 2 часа после введения парамагнетика была выполнена магнитно-резонансная томография, на которой были визуализированы тазовые лимфатические узлы (фиг.2). По задней поверхности прямой кишки визуализируется увеличенный до 1,5 см в диаметре лимфатический узел мезоректума с характерным дефектом наполнения контраста (фиг.3). Отсутствие метастатического поражения лимфатического аппарата вне собственной фасции прямой кишки определило дальнейшую хирургическую тактику. (Больной выполнена мезоректальная эксцизия прямой кишки.) Результаты лимфографии подтвердились - при патогистологическом послеоперационном исследовании в указанном лимфатическом узле выявлен метастаз опухоли, в других 14 узлах препарата мезоректума метастазов не обнаружено.Example 2. The absence of a metastatic lesion of the lymphatic apparatus outside the rectal fascia of its own determined further surgical tactics. (The patient underwent mesorectal excision of the rectum.) The results of lymphography were confirmed - with a histopathological postoperative examination, tumor metastasis was detected in the indicated lymph node, and metastases were not found in the other 14 nodes of the mesorectum preparation.
