Известно, что ремантадин обладает противовирусной активностью в отношении широкого ряда штаммов вируса гриппа типа А, особенно сероподтипа H3N2. Механизм его действия достаточно изучен. Он относится к мембранотропным препаратам и блокирует протонный канал, образованный М2 белком вируса гриппа, что приводит к ингибиции инфекционной активности вируса на стадии декапсидации. Следует отметить, что ремантадин в общепринятой лекарственной форме в высоких дозах обладает рядом существенных недостатков: 1) препарат активен только в отношении вируса гриппа типа А; 2) при приеме препарата регистрируются побочные действия в отношении ЦНС, клеток печени и почек, желудочно-кишечного тракта (чаще у лиц пожилого возраста); 3) быстрое формирование резистентных штаммов вируса. В пределах 3-5 дней терапии у 30% детей и взрослых были выделены резистентные к ремантадину вирусы. У пациентов, от которых были выделены резистентные к препарату вирусы, не отмечалось клинического ухудшения, но заболевание протекало длительнее [5]. Эффективность использования двух препаратов в ингибиции репродукции вируса гриппа была подтверждена в экспериментах на мышах (табл.2). Препарат но-шпа и ремантадин (в концентрации 20 мг/кг каждого) вводили внутрибрюшинно через 1, 24, 48 и 72 ч после инфицирования мышей. Установлено, что во всех очагах, где принимали одновременно но-шпу (в возрастных концентрациях) и ремантадин (в концентрации 0,025мг), к концу первых суток после начала приема препаратов снижалась температурная реакция с последующим значительным уменьшением клинических проявлений гриппа, снижалась общая продолжительность заболевания, отсутствовали осложнения, не наблюдалось побочных симптомов. Кроме того, больные-аллергики (2 человека) легче переносили гриппозную инфекцию, что выражалось в уменьшении их срока выздоровления.The effectiveness of the treatment of influenza with the simultaneous administration of no-shpa and remantadine was confirmed in family foci.
