Изобретение относится к медицине, а именно к нейрохирургии, восстановительной хирургии, и конкретно касается замещения дефектов периферических нервных стволов. Известен способ замещения дефектов периферических нервов с помощью свободной (неваскуляризированной) аутонервной пластики [1] , при которой трансплантаты кожных нервов (чаще - икроножного) вшиваются в дефект поврежденного нервного ствола периневральными швами. Недостатками данного способа являются опасность развития некроза нервных трансплантатов до того, как восстановится их адекватное кровоснабжение за счет новообразованных сосудов, а наличие двух линий швов затрудняет прорастание регенерирующих аксонов в дистальный отрезок нерва. Кроме того, некоторые больные в течение длительного времени испытывают неприятные ощущения в зоне иннервации нерва-донора. There is a method of replacing defects of peripheral nerves using free (non-vascularized) autologous plastic [1], in which grafts of skin nerves (usually gastrocnemius) are sutured into the defect of the damaged nerve trunk with perineural sutures. The disadvantages of this method are the danger of developing necrosis of nerve grafts before their adequate blood supply is restored due to the newly formed vessels, and the presence of two suture lines makes it difficult for the regenerating axons to grow into the distal segment of the nerve. In addition, some patients for a long time experience discomfort in the area of innervation of the donor nerve.
Наиболее близким по своему техническому решению к предлагаемому является способ, принятый за прототип и заключающийся в замещении дефекта лучевого нерва на плече с помощью трансплантации или транспозиции его поверхностной ветви в составе лучевого лоскута предплечья на лучевых питающих сосудах [2]. Однако при наличии дефекта нерва более 10 см данный способ позволяет получить только один кровоснабжаемый трансплантат, что существенно снижает шансы на получение полезной реиннервации мышц разгибательной группы предплечья, так как удается заместить не более 1/3 площади поперечного сечения лучевого нерва. Сущность предлагаемого изобретения заключается в совокупности существенных признаков, достаточных для достижения искомого технического результата, а именно - улучшения функциональных результатов у больных с обширными дефектами лучевого нерва на плече. Эта сущность заключается в том, что одним блоком с трансплантатом поверхностной ветви лучевого нерва на лучевых питающих сосудах забирают и пересаживают в область дефекта трансплантат латерального кожного нерва предплечья. The essence of the invention lies in the combination of essential features sufficient to achieve the desired technical result, namely, improving functional results in patients with extensive defects of the radial nerve on the shoulder. Замещение дефекта лучевого нерва на плече двумя параллельно вшитыми васкуляризированными трансплантатами позволяет заместить около 2/3 площади сечения лучевого нерва. Способ осуществляется следующим образом. The method is as follows.
Производят выделение проксимального и дистального фрагментов поврежденного лучевого нерва, измеряют величину диастаза между ними. В случае наличия дефекта более 10 см выделяют лучевой лоскут на лучевых питающих сосудах с предплечья, в который, кроме поверхностной ветви лучевого нерва, включают латеральный кожный нерв предплечья. После выделения лоскута проводят гемостаз и оценивают жизнеспособность лоскута на непересеченной сосудистой ножке. Убедившись в хорошем кровоснабжении лоскута, производят его трансплантацию в область верхней трети плеча. При дефекте лучевого нерва в средней или нижней трети плеча возможна транспозиция лоскута. При пересадке лучевого лоскута его включают в кровоток, накладывая микрососудистые анастомозы между лучевыми сосудами (артерией и двумя венами) и плечевой артерией и комитантными венами. При этом кровоснабжение латерального кожного нерва предплечья, идущего через лучевой лоскут транзитом, восстанавливается автоматически с возобновлением кровотока в самом лоскуте. После восстановления кровоснабжения производят аутонервную пластику дефекта лучевого нерва двумя васкуляризированными трансплантатами (поверхностная ветвь лучевого нерва и латеральный кожный нерв предплечья) строго в соответствии со схемами внутриствольной топографии. When a radiation flap is transplanted, it is included in the bloodstream, superimposing microvascular anastomoses between the radiation vessels (an artery and two veins) and the brachial artery and commitinous veins. In this case, the blood supply to the lateral cutaneous nerve of the forearm, which passes through the radial flap in transit, is automatically restored with the resumption of blood flow in the flap itself. After the restoration of blood supply, autologous plastic surgery of the radial nerve defect is performed with two vascularized grafts (the superficial branch of the radial nerve and the lateral cutaneous nerve of the forearm) strictly in accordance with the schemes of intra-stem topography. A skin defect in the donor area is closed using autodermoplasty.

Больной Г-ев, 1974 г.р., история болезни 1121, поступил в отделение микрохирургии НИЦТ"ВТО" 31.03.99 с диагнозом: сросшийся перелом левой плечевой кости, травматическое повреждение задней группы мышц левого плеча 8-месячной давности, повреждение лучевого нерва в средней трети левого плеча, нейропатия локтевого нерва, кожнорубцовая деформация задней поверхности плеча. Под общим наркозом произведено иссечение рубцов задней поверхности плеча. При ревизии был выявлен дефект лучевого нерва на плече длиной 13 см, обширный дефект трехглавой мышцы. Локтевой нерв находится в рубцовой ткани по всей длине плеча. После гидропрепаровки 0,25%-ным раствором новокаина произведен невролиз локтевого нерва. С правого предплечья из отдельного доступа забран свободный трансплантат лучевого лоскута с включением плечелучевой мышцы, поверхностной ветви лучевого нерва и латерального кожного нерва предплечья длиной 14 см, участка кожи площадью 30 см2. Данный лоскут забран на сосудистой ножке, состоящей из лучевой артерии диаметром 2 мм, двух вен диаметром 0,8 и 0,6 мм. Трансплантат перенесен на левое плечо, произведены микрососудистые анастомозы его сосудов с плечевыми артерией и веной "конец-в-бок". При пуске кровотока определяется проходимость анастомозов. Васкуляризированная аутонервная пластика лучевого нерва по предлагаемому способу позволила получить два кровоснабжаемых нервных трансплантата длиной 14 см, заместив около 60% площади сечения лучевого нерва. Удалось также заместить дефект трехглавой мышцы плеча, включенной в лоскут плечелучевой мышцей, и произвести кожную пластику зоны дефекта мягких тканей плеча за счет участка кожи лучевого лоскута.Under general anesthesia, scars of the posterior shoulder surface were excised. This flap was taken on a vascular pedicle consisting of a radial artery with a diameter of 2 mm, two veins with a diameter of 0.8 and 0.6 mm. The transplant was transferred to the left shoulder, microvascular anastomoses of its vessels with brachial artery and end-to-side vein were produced. When starting blood flow, patency of the anastomoses is determined. Vascularized autologous plastic surgery of the radial nerve by the proposed method made it possible to obtain two blood-supplied nerve grafts with a length of 14 cm, replacing about 60% of the cross-sectional area of the radial nerve. It was also possible to replace the defect of the triceps muscle of the shoulder, included in the flap of the brachioradialis muscle, and to produce skin plasty of the area of the defect of the soft tissues of the shoulder due to the skin area of the radial flap.
В послеоперационном периоде пациент получал диадинамические токи, магнитотерапию, иглорефлексотерапию, массаж, мануальную терапию, электростимуляцию мышц, витамины, прозерин, трентал, реополиглюкин, антибиотики. Операционные раны зажили первичным натяжением, швы сняты через 2 недели. Пересаженный лучевой лоскут прижил полностью, осложнений не было. In the postoperative period, the patient received diadynamic currents, magnetotherapy, acupuncture, massage, manual therapy, muscle electrical stimulation, vitamins, proserin, trental, reopoliglyukin, antibiotics. Surgical wounds healed by first intention, the sutures were removed after 2 weeks. The transplanted radiation graft fully survived; there were no complications.
