Изобретение относится к медицине, в частности к лабораторной диагностике аутоиммунных процессов в суставах при остеоартрозе и может быть использовано для определения показаний к внутрисуставному введению хондропротекторов на основе гидролизатов гликозаминогликанов суставного хряща. The invention relates to medicine, in particular to laboratory diagnostics of autoimmune processes in joints during osteoarthritis and can be used to determine indications for intra-articular administration of chondroprotectors based on glycosaminoglycans of articular cartilage hydrolysates.
Известно, что в результате дегенеративных изменений, происходящих в хряще, развивается деструкция основного вещества соединительной ткани, становится возможным высвобождение различных ее компонентов (протеогликанов, коллагена и др.) [1, 2]. Освобождающиеся протеогликаны, продукты распада хондроцитов, коллагена, выступая как аутоантигены, вовлекаются в иммунопатологический процесс и способствуют дальнейшему его развитию. Поскольку для лечения остеоартроза в качестве базисной терапии применяются хондропротекторы, содержащие гликозаминогликаны, то искусственное повышение концентрации гликозаминогликанов в организме человека провоцирует развитие иммунологических реакций к ним, а следовательно, и активизацию процесса. Гетерогенностью иммунопатологических реакций, лежащих в основе генеза болезни, можно объяснить необходимость индивидуального подхода к терапии больных с остеоартрозом методом внутрисуставного введения хондропротекторов на основе гидролизатов гликозаминогликанов суставного хряща. Принятый в качестве аналога непрямой вариант твердофазного метода иммуноферментного анализа, описанный в различных литературных источниках, обладает высокой чувствительностью и специфичностью. Since chondroprotectors containing glycosaminoglycans are used as the basic therapy for the treatment of osteoarthritis, an artificial increase in the concentration of glycosaminoglycans in the human body provokes the development of immunological reactions to them, and, consequently, the activation of the process. The heterogeneity of the immunopathological reactions that underlie the genesis of the disease can explain the need for an individual approach to the treatment of patients with osteoarthritis by the method of intra-articular administration of chondroprotectors based on glycosaminoglycans of articular cartilage hydrolysates. The indirect version of the solid-phase enzyme-linked immunosorbent assay adopted as an analogue, described in various literary sources, is highly sensitive and specific.

Задачей настоящего изобретения является разработка способа определения показаний к внутрисуставному введению хондропротекторов на основе гидролизата гликозаминогликанов хряща, позволяющего в условиях in vitro прогнозировать развитие аутоиммунного процесса в суставе и предупреждать появление реакций гиперчувствительности при внутрисуставном введении препаратов-хондропротекторов на основе гидролизата гликозаминогликанов хряща. Поставленная задача решается тем, что в способе определения показаний к внутрисуставному введению хондропротекторов на основе гидролизата гликозаминогликанов хряща в приготовленный раствор Bacto-Agar "Тур USA" вмешивают хондропротектор на основе гидролизата гликозаминогликанов хряща, затем полученный раствор разливают на стеклянную пластину и получают слой геля, после его перфорации в отверстия закапывают синовиальную жидкость, выдерживают в кристализаторе и при его окрашивании идентифицируют образование колец преципитата, наличие которых определяет противопоказания к внутрисуставному применению хондропротекторов на основе гидролизата гликозаминогликанов хряща. Способ осуществляют следующим образом. The method is as follows.
Получает пунктат синовиальной жидкости. Готовят раствор Bacto-Agar "Тур USA". В него вносят гидролизат гликозаминогликанов или хондропротектор. Полученный раствор разливают на стекла 90х120 мм и оставляют до застывания геля. После чего выполняют перфорацию слоя геля и в полученные лунки вносят синовиальную жидкость. Стекла с гелем оставляют во влажной камере на 24 часа. Для визуальной оценки полученных результатов окрашивают гель красителем Амидо шварц. Наличие в геле колец преципитации служит противопоказанием для внутрисуставного введения хондропротекторов. Receives punctate synovial fluid. A Bacto-Agar Tour USA solution is prepared.
