Abstract
Description
Изобретение относится к хирургии, в частности к травматологии и ортопедии для лечения повреждений локтевого сустава у детей и подростков. The invention relates to surgery, in particular to traumatology and orthopedics for the treatment of injuries of the elbow joint in children and adolescents.
При травме локтевого сустава у детей и подростков требуется точное сопоставление отломков, обусловленное наличием зон роста. In injuries of the elbow joint in children and adolescents, an accurate comparison of fragments is required due to the presence of growth zones.
У пациентов детского и подросткового возраста при выполнении доступа к локтевому суставу с депериостированием локтевого отростка (Бойчев Б., Конфорти Б. , Чоканов К. Оперативная ортопедия и травматология. - София. 1961. - С. 248-249) наблюдаются следующие осложнения: вследствие механического повреждения проксимальной зоны роста локтевой кости и нарушения кровоснабжения проксимального отдела локтевой кости возникает деформация и нарушение конгруэнтности в плечелоктевом суставе, нарушается объем движений в локтевом суставе, ограничивается функциональная активность больного. Задача - полное восстановление конгруэнтности и объема движений в локтевом суставе при травматических повреждениях путем зигзагообразного оперативного доступа. The task is the full restoration of congruence and range of motion in the elbow joint in case of traumatic injuries by means of a zigzag surgical access.
Задача реализуется следующим образом. Выполняют зигзагообразный разрез по задней поверхности локтевого сустава: разрез начинают над основанием локтевого отростка локтевой кости и направляют в сторону вершины медиального надмыщелка плечевой кости. Над медиальным надмыщелком плечевой кости разрез направляют под углом 80-90o (формируют лоскуты с некритическим кровоснабжением) по задней поверхности плеча к его задненаружной поверхности, где делают поворот на 80-90o и продолжают до задневнутренией поверхности плеча (фиг.1, поз. 1). Треугольные кожно-подкожные лоскуты отслаивают и отводят в стороны (фиг. 2, поз.1). Апоневроз трехглавой мышцы плеча (фиг.2, поз. 3) рассекают продольно, строго по средней линии, не доходя на 1 см до вершины локтевого отростка локтевой кости (фиг.2, поз. 2). Из дистального угла разреза апоневроза трицепса продолжают его рассечение в двух направлениях: медиально и латерально от локтевого отростка под углом не более 80-90o, оба разреза направляют к основаниям надмыщелков плечевой кости (фиг.2, поз. 4). Трехглавую мышцу плеча расслаивают продольно по ходу мышечных волокон до надкостницы плечевой кости. Капсулу локтевого сустава (фиг.3, поз. 4) рассекают скальпелем от верхнего края локтевой ямки до ее середины, затем в полость сустава вводят лезвие скальпеля, располагают его режущей поверхностью к капсуле. Капсулу локтевого сустава рассекают в медиальном и латеральном направлениях параллельно краям локтевого отростка (фиг.3, поз. 5). Преимущества доступа:
- зигзагообразный разрез создает максимальную мобильность кожной раны, возможность расширения разреза в любом направлении и обеспечивает большой угол операционного действия;
- расслоение трехглавой мышцы плеча, а не рассечение ее, позволяет сохранить анатомическую целостность функционально активных мышечных волокон, что не способствует формированию грубого мышечного рубца;
- форма разреза апоневроза трехглавой мышцы плеча позволяет увеличить угол операционного действия, сохранить в целостности надкостницу на проксимальном отделе локтевой кости, сохранить кровоснабжение эпифиза и проксимальной зоны роста локтевой кости;
- углообразная форма разреза апоневроза трехглавой мышцы плеча позволяет точно сопоставить его раневые края при ушивании, что не нарушает синхронность мышечного сокращения;
- выполнение артротомии локтевого сустава путем направления лезвия скальпеля режущей поверхностью к капсуле сустава исключает случайное повреждение суставного хряща, что не создает дополнительных условий, помимо полученной травмы, для развития деформирующего артроза.The task is implemented as follows. From the distal angle of the incision of the triceps aponeurosis, its dissection is continued in two directions: medially and laterally from the ulnar process at an angle of no more than 80-90 o , both sections are directed to the bases of the epicondyles of the humerus (Fig. 2, item 4). The triceps muscle of the shoulder is stratified longitudinally along the muscle fibers to the periosteum of the humerus. The capsule of the elbow joint (Fig. 3, item 4) is dissected with a scalpel from the upper edge of the ulnar fossa to its middle, then a scalpel blade is inserted into the joint cavity, and its cutting surface is placed to the capsule. The capsule of the elbow joint is dissected in the medial and lateral directions parallel to the edges of the ulnar process (Fig. 3, item 5). Access Benefits:

- the incision form of the aponeurosis of the triceps of the shoulder muscle allows you to increase the angle of the surgical action, to maintain the integrity of the periosteum at the proximal ulna, to maintain blood supply to the pineal gland and proximal ulna growth zone;
- the angular shape of the aponeurosis of the triceps of the shoulder muscle allows you to accurately match its wound edges when suturing, which does not violate the synchronism of muscle contraction;
- performing arthrotomy of the elbow joint by directing the scalpel blade with the cutting surface to the joint capsule eliminates accidental damage to the articular cartilage, which does not create additional conditions, in addition to the injury, for the development of deforming arthrosis.
