Изобретение относится к медицине, а именно к функциональному исследованию нервной системы, и может быть использовано в клинической физиологии, неврологии, нейрореаниматологии, нейроанестезиологии. В настоящее время известно около 200 способов автоматического частотно-амплитудного анализа фонового ЭЭГ сигнала, применяемых для оценки функционального состояния мозга (см. Обзор О.Б. Сазоновой "Мониторинг спонтанной биоэлектрической активности мозга в нейроанестезиологии и нейрохирургии" // Российский журнал анестезиологии и интенсивной терапии, 1999, 1, с.63-70.)
Недостатком известных способов является то, что в целом с их помощью оценивается только наличие или отсутствие (в регистрируемых с различных отделов головы ЭЭГ сигналах) основных ЭЭГ ритмов (альфа, бета, тета, дельта), амплитуда этих ритмов и их соотношение по доминированию в спектре ЭЭГ сигнала. При этом количественно не определяются колебания (модуляции) амплитуды ЭЭГ ритмов во времени, являющиеся отражением происходящих изменений функциональной активности мозга. Кроме того, традиционная оценка фоновой ЭЭГ не дает информации о сохранности регуляторных механизмов мозга в условиях нейроанестезии, нейровегетативной "защиты", а также у больных, находящихся в коматозных состояниях, т. к. в этих условиях регистрируются только патологические ритмы фоновой ЭЭГ.Currently, there are about 200 methods of automatic frequency-amplitude analysis of the background EEG signal used to assess the functional state of the brain (see the review by O.B. A disadvantage of the known methods is that in general, with their help, only the presence or absence (in the EEG signals recorded from different parts of the head) of the main EEG rhythms (alpha, beta, theta, delta), the amplitude of these rhythms and their ratio by dominance in the spectrum is estimated EEG signal. Moreover, fluctuations (modulations) of the amplitude of the EEG rhythms in time are not quantified, which are a reflection of the changes in the functional activity of the brain. In addition, the traditional assessment of background EEG does not provide information about the preservation of the regulatory mechanisms of the brain under conditions of neuroanesthesia, neurovegetative "protection", as well as in patients in coma, because in these conditions only pathological rhythms of background EEG are recorded.
Наиболее близким к заявленному способу по совокупности существенных признаков, достигаемому результату и выбранным в качестве прототипа, является мониторинг функций мозга по П.Ф. Прайор (см. П.Ф. Прайор. Мониторный контроль функций мозга. М.: Медицина, 1982, 328 с. с ил.). Недостаток известного способа заключается в том, что степени вариабельности амплитуды ЭЭГ дается лишь качественная характеристика по ширине полосы записи (изменение значений амплитуды сигнала ЭЭГ во времени регистрируется в виде полосы записи, где верхний край соответствует максимальным, а нижний минимальным уровням регистрируемой активности мозга): чем шире полоса, тем больше вариабельность. Количественный анализ вариабельности с оценкой гемодинамической составляющей ЭЭГ сигнала данный метод не предусматривает. Поставленная задача достигается тем, что регистрация средней амплитуды сигнала ЭЭГ от удара к удару сердца согласно изобретению фиксируется в виде тренда с последующим Фурье-анализом и позволяет оценить характер функциональной активности мозга, связанной с основными афферентными влияниями (барорефлекс, метаболорефлекс и др.). Способ осуществляется следующим образом. The method is as follows.
С помощью двух отводящих электродов, установленных на коже головы над правой и левой теменными областями, регистрируется ЭЭГ. (Третий электрод устанавливается на коже кпереди от первых двух с целью защиты от мощных электрических помех). На основании анализа абсолютных и относительных величин мощности колебаний амплитуды ЭЭГ делается вывод о наличии и доминировании определенных регуляторных механизмов мозга, сохранности реакции мозга на афферентацию (барорефлекс, метаболорефлекс и др.). Анализ вариабельности амплитуды ЭЭГ отражает функциональное состояние мозга независимо от того, какие основные ритмы регистрируются в этот период (альфа, бета, тета, дельта). На фоновой ЭЭГ регистрируется альфа-ритм (фиг.1). Изменение амплитуды зарегистрированы в виде тренда (фиг.2). Средняя амплитуда ритма - 15 мВ. Оценка вариабельности проведена с использованием метода Фурье (фиг.3). Мощность колебаний амплитуды ритма - 20 мВ2. На фоновой ЭЭГ регистрируются патологические дельта- и тета-волны (фиг. 5). Изменение амплитуды зарегистрированы в виде тренда (фиг.6). Средняя амплитуда (10 мВ) и мощность колебаний ритмов (14 мВ2) снижены (фиг.8). На фиг.8 представлена мощность колебаний в абсолютных и относительных единицах. Дыхательные и барорефлекторные модуляции сохранены и доминируют (фиг.7 и 8). Несмотря на коматозное состояние больного и патологические ритмы на фоновой ЭЭГ, можно говорить о хорошем прогнозе заболевания. (Через 16 суток больной был переведен из реанимационного отделения в неврологическое в удовлетворительном состоянии.)
Пример 3. Больной Гажиу В.А., 23 года. Диагноз: черепно-мозговая травма. Ушиб мозга тяжелой степени. Кома (4 балла по шкале Глазго). 6 сутки болезни. Проводится нейро-вегетативная защита мозга (повторные в течение суток внутривенные введения реланиума и ГОМК).Pathological delta and theta waves are recorded on the background EEG (Fig. 5). Средняя амплитуда (8 мВ) и мощность колебаний ритмов (11 мВ2) снижены. На фиг.12 представлена мощность колебаний в абсолютных и относительных единицах. Дыхательные и барорефлекторные модуляции резко снижены не только по абсолютной величине, но и по представительству в общем спектре колебаний. Преобладают волны гуморально-метаболической регуляции (фиг.11 и 12). По данным анализа можно предполагать плохой прогноз болезни. Коматозное состояние у данного больного трансформировалось в хроническое вегетативное состояние.Pathological delta and theta waves are recorded on the background EEG (Fig. 9).
