Согласно операции по Кистнеру разрезом по верхней трети бедра выделяют бедренные вены и находят клапан, обычно находящийся в устье бедренной вены. После наложения зажима кровь вытесняют проксимально. При возобновлении возможности ретроградного тока несостоятельный клапан пропускает кровь в подклапанный сегмент вены. При восстановлении несостоятельного клапана накладывают зажим и рассекают вену продольно на 3 см ниже клапана в проксимальном направлении через комиссуру створок и продолжают далее разрез еще на 3 см, предупреждают о возможности повреждения створок при рассечении в области комиссуры. Находят перерастянутые опущенные створки клапана, позволяющие обратный ток крови. Восстановление производят, пришивая избыток листка створки к стенке вены у места крепления к стенке вены 4-6 узловыми швами, для чего в просвет вены через прокол в ее стенке снаружи вовнутрь вводят атравматическую иглу с нитью 6/0 - 7/0, захватывают створки на расстоянии 2 мм от края крепления, затем иглу с нитью через прокол изнутри выводят наружу. Узлы завязывают на внешней стенке вены. Аналогично уменьшают створки со стороны рассеченной комиссуры. Вопрос о степени уменьшения створки считают дискутабельным. Флеботомию ушивают узловыми швами, предупреждают о возможности при этом деформации створок. После закрытия флеботомии проверяют компетентность клапана. Если клапан несостоятелен, операцию повторяют или лигируют вену. Способ осуществляется следующим образом (Фиг.1 - 4). Рассекают латеральнее линии Кена мягкие ткани верхней трети бедра на длину 8 - 10 см, выделяют общую бедренную, бедренную и глубокую бедренные вены. Обычно в устье бедренной вены находят клапан. Дистальнее клапана накладывают сосудистый окклюзатор. Кровь выдавливают в проксимальном направлении. При ретроградном токе крови несостоятельный клапан пропускает ее в подклапанный сегмент. Далее накладывают окклюзатор проксимальнее клапана, при необходимости и на глубокую вену. Вену в области клапанного синуса рассекают продольно строго по середине между комиссурами на протяжении 8 - 10 мм, причем две трети разреза выполняют проксимальнее уровня комиссур. Измеряют длину свободного края створок, откладывают на нем у каждой комиссуры половину разницы между длиной свободного края створки и длиной полуокружности вены. Далее создают неокомиссуры. Путем прокола снаружи вовнутрь выше уровня комиссур в просвет вены вводят атравматическую иглу с нерассасывающейся нитью 8/0 - 9/0, подхватывают свободный край створок на расстоянии отложенной от комиссуры длины избытка свободного края, через прокол изнутри наружу выводят нить из просвета вены (Фиг.1) и завязывают узел на внешней стороне стенки вены, нить не срезают. Аналогично создают вторую неокомиссуру с другой стороны. Разрез вены ушивают непрерывно - узловым швом (Фиг.2). Снимают проксимальный окклюзатор. Поднимают давление в надклапанном сегменте вены. По ретроградному заполнению кровью клапанного синуса и отсутствию ее в подклапанном сегменте судят о восстановленной замыкательной функции клапана. Снимают остальные окклюзаторы. Нерассасывающейся атравматической нитью 7/0 - 8/0 накладывают верхний и нижний внутристеночные циркулярные кисетные швы в адвентиции вены в надклапанном и подклапанном сегментах без сужения просвета вены (Фиг.3). Следующим этапом выполняют фиксацию неокомиссур к циркулярному кисетному шву. Нитями, оставленными после формирования неокомиссур, накладывают продольные внутристеночные швы в адвентиции от неокомиссур в проксимальном направлении с фиксацией к верхнему циркулярному кисетному шву без деформации венозной стенки (Фиг.4). Это является профилактикой рецидива несостоятельности клапана. Операцию завершают послойным ушиванием раны. The method is as follows (Fig.1 - 4). The threads left after the formation of the neocomissure impose longitudinal intra-wall sutures in adventitia from the neocomissure in the proximal direction with fixation to the upper circular purse string suture without deformation of the venous wall (Figure 4). This is to prevent recurrence of valve failure. The operation is completed by layering the wound.

Больная О. 32 лет, служащая, поступила в стационар 04.02.1996 г. с жалобами на отеки левой голени и области голеностопного сустава, появляющиеся в первой половине дня, не исчезающие полностью после отдыха, ортостатическую круралгию, наличие варикозных вен, трофических расстройств кожи левой голени. Больна с 19 лет, заболевание постепенно прогрессировало, резкое ухудшение после первой беременности. При осмотре: на передне - медиальной поверхности левого бедра и левой голени в бассейне большой подкожной вены варикозные узлы, при пальпации болезненные, мягко - эластической консистенции. Отек области голеностопного сустава и нижней трети голени, при надавливании остаются ямочки. В нижней трети голени по медиальной поверхности зона индурации до 6х5 см. При ретроградной флебографии выявлена несостоятельность клапанов бедренной вены (сброс контрастного вещества до подколенной вены). При ультразвуковой доплерографии - несостоятельность клапана бедренной вены. Учитывая наличие несостоятельности клапанного аппарата бедренной вены, больной предложено оперативное лечение - микрохирургическая реконструкция клапана бедренной вены с комбинированной флебэктомией в системе большой подкожной вены. 07.02.1996 г. под эпидуральной анестезией произведена операция - микрохирургическая реконструкция клапана бедренной вены с профилактикой рецидива несостоятельности, комбинированная флебэктомия в системе большой подкожной вены левой нижней конечности. Разрезом длиной 10 см на передней поверхности левого бедра, начинающимся под паховой складкой и идущим латеральнее линии Кена вниз, выделены общая (ОБВ), бедренная (БВ) и глубокая (ГБВ) бедренные вены. В устье БВ выявлен несостоятельный клапан. После тестирования клапана и подтверждения его несостоятельности на ОБВ, БВ, ГБВ наложены окклюзаторы. Венотомия между комиссурами строго посередине на протяжении 8 мм. При ревизии клапан двухстворчатый, свободные края створок провисают до уровня дна клапана. Измерена длина полуокружности вены и величина избытка свободного края створок. Созданы неокомиссуры путем ушивания половины избыточной длины свободного края створок с прошиванием стенки вены проксимальнее уровня комиссур на половину избыточной длины свободного края створок нитью 8/0 с обеих сторон. Разрез вены ушит нитью 8/0. После снятия окклюзаторов с ОБВ и ГБВ при тестировании клапана поступления крови в подклапанный сегмент нет - клапан состоятелен, снят окклюзатор с БВ. Нитью 7/0 наложены внутристеночные циркулярные кисетные швы в адвентиции вены в надклапанном и подклапанном сегментах. Нитями, оставленными после формирования неокомиссур, наложены продольные внутристеночные швы в адвентиции вены от неокомиссур в проксимальном направлении с фиксацией к верхнему циркулярному кисетному шву. Контроль гемостаза - сухо. Послойное ушивание раны. Выполнена комбинированная флебэктомия в системе большой подкожной вены. Швы на раны. Эластическое бинтование. Example. Given the failure of the valve apparatus of the femoral vein, the patient was offered surgical treatment - microsurgical reconstruction of the femoral vein valve with combined phlebectomy in the saphenous vein system. 02/07/1996, an operation was performed under epidural anesthesia - microsurgical reconstruction of the femoral vein valve with the prevention of relapse, combined phlebectomy in the saphenous vein of the left lower limb. A section of 10 cm in length on the front surface of the left thigh, starting under the inguinal fold and extending laterally down the Ken line, highlights the total (OBV), femoral (BV) and deep (HBV) femoral veins. Insolvent valve revealed at the mouth of the BV. After testing the valve and confirming its failure on OBV, BV, GBV superimposed occluders. The vein incision is sutured with 8/0 thread. After removing the occluders from the OBV and the HBV, when testing the valve, there is no blood flow to the subvalvular segment - the valve is sound, the occlusor is removed from the BV. Thread 7/0 imposes intra-wall circular purse string sutures in adventitia of the vein in the supravalvular and subvalvular segments. The threads left after the formation of the neocomissure imposed longitudinal intra-wall sutures in the adventitia of the vein from the neocomissure in the proximal direction with fixation to the upper circular purse string suture. Hemostasis control is dry. Layer wound closure. A combined phlebectomy was performed in the saphenous vein system. Stitches for wounds. Elastic bandage.
Больная осмотрена через три месяца после операции. Отмечено исчезновение отека, ортостатической круралгии, регрессия трофических расстройств кожи голени.
