Abstract
Description
The invention relates to medicine, namely to devices used in surgery. Designed for instrumental dosed sphincter apparatus of the gastrointestinal tract, in particular, pyloric pulp with simultaneous aspiration of the contents of hollow organs.
An analogue of this device can be considered a retractor, for example, a thoracic, vascular with a cremallera, used in surgical practice [Operative surgery and topographic anatomy. // Ed. V.V. Kovanova. M.: Medicine, 1978, 415 pp.]. Common is the principle of action. Namely, by axial rotation of the lever acting on the toothed surface of the base of one of the branches, there is a mechanical movement of the second branch of the expander in one plane. The result is a dilatation of the edges of the wound at a certain distance. The disadvantages of this device can be considered, firstly, sufficiently large sizes, limiting the manipulation of the surgical wound; secondly, the shape of the retractor jaw does not allow their introduction through the sphincter apparatus into the lumen of a hollow organ having a rounded cross section, and therefore a uniform effect on the sphincter muscles or walls of the hollow organ over their entire surface cannot be achieved, which can lead to muscle fiber breaks.
The prototype of the proposed device can be considered an existing rectal mirror [Operative surgery and topographic anatomy. // Ed. V.V. Kovanova. M .: Medicine, 1978, 415 pp.], The principle of action of which is that by rotating the thrust helical mechanism, dilution of the expander branches by a certain distance is achieved. In addition, the rounded shape of the branch allows you to act on the maximum possible surface of the walls of a hollow organ or its sphincter apparatus. Significant disadvantages of the existing prototype is, firstly, the lack of the possibility of a metered divulsion by a given value; secondly, in the process of dilatation of the lumen of the hollow organ on the surgical field, its stagnant contents inevitably pour out, which greatly complicates further manipulations and requires additional time and effort to drain the surgical wound.
The purpose of the invention is to reduce the morbidity of manipulation, the implementation of portioned instrumental divulsion of the sphincter apparatus of a hollow organ with simultaneous aspiration of its contents.
The invention is illustrated by drawings, where figure 1 and 2 shows a General view of the expander-aspirator.
The essence of the proposal lies in the fact that the expander-aspirator additionally contains a suction tube (7) fixed on one of the branches of the device (4) for aspiration of the contents of the hollow organ in the process of sorption. Rows of holes (9) are applied to the branches (4) and a fitting (8) is placed at the end, the opposite end of the suction tube (7) having the shape of a truncated cone with a base facing between the branches (4), which are made with additional holes (5) ) to facilitate aspiration of stagnant contents from the lumen of a hollow organ. In addition, a persistent screw mechanism is mounted on one of the device handles, calibrated in such a way that each turn of the screw (10) ensures that the jaw opens to a predetermined distance, and the presence of a convex risk on the screw handle (11) (12) allows both visual and and tactile speed control.
The use of this device is as follows. When preparing the expander-aspirator for operation, the vinyl chloride hose of the standard suction of a centralized vacuum system is connected to the fitting of the suction tube (8). The device is introduced into the sphincter and the stagnant contents are evacuated from the hollow organ by turning on the suction. Then, without turning off the vacuum suction, the jaws (4) of the device are gradually bred to the required distance by smoothly rotating the thrust screw (10) for a certain number of revolutions. At the same time, the rotation of the screw (10) is controlled according to the convex risk (12) by means of visual or finger control.
The aspirator expander consists of two handles (1) with two branches (4). The axial mobility of the levers is due to the presence of a fixing screw (3). One of the levers has a return leaf spring (2). A thrust screw mechanism is located on the second lever, including an internal threaded channel with a screw (10). The screw handle (11) has a convex risk (12). A suction tube (7) with rows of holes (9) is attached to the inner surface of one of the expander branches using a removable fixation sleeve (6). The fitting (8) at the end of the suction tube, facing the base of the jaw (4), is used to fasten the standard vinyl chloride hose of a centralized vacuum system. Additional holes (5) are applied to the expander branches (4) to facilitate aspiration of stagnant contents from the lumen of the hollow organ.
The use of the device was carried out during surgical operations associated with the need for intraoperative divisions of the pyloric pulp. The use of an expander-aspirator helps to reduce the morbidity of manipulation, provides a portioned sulphonic apparatus divulsion of a hollow organ with simultaneous aspiration of its contents.
