Известны способы лечения урогенитального хламидиоза (УГХ), включающие введение антибиотика как перорально, так и парентерально (Скрипкин Ю.К., Шарапова Г. Я. , Кубанова А.А., Селисский Г.Д. Инфекции, передаваемые половым путем, Москва, МЕДпресс, 1999, с. 190-191). Выбор лечебного препарата в настоящее время осложняется расширением спектра антибиотикоустойчивости хламидий. Кроме того, назначение препарата больному на основании рекомендаций по применению или по сравнительной эффективности разных антибактериальных средств без исследования чувствительности возбудителя к препаратам приводит к неэффективному лечению и, как следствие, к большому числу рецидивов и осложнений. Поставленная задача решается тем, что в известном способе лечения УГХ, включающем предварительное определение эффективности антибиотиков по исследованию чувствительности возбудителя к ним и введение антибиотиков, согласно изобретению, выбирают наиболее эффективный препарат и менее эффективный по сравнению с ним препарат из другой фармакологической группы и вводят их последовательно с интервалом 1-2 дня, начиная с менее эффективного препарата. Поэтому введение на следующем этапе терапии препарата наиболее эффективного из другой фармакологической группы вызывает быстрое, устойчивое и полное подавление размножения хламидий. Последовательное введение выбранных антибиотиков с интервалом 1-2 дня обусловлено продолжительностью цикла развития хламидий: от момента инвазии клеток до их разрушения и выхода нового поколения готовых к инвазии элементарных телец проходит 48-72 часа. Определяют эффективность антибиотиков путем исследования чувствительности возбудителя к основным препаратам, применяющимся в настоящее время. Для этого выделенные у больного штаммы хламидий культивируют в присутствии антибиотиков и выявляют наличие цитоплазматических включений (ЦПВ) в % и элементарных телец (ЭТ) в контроле и в присутствии антибиотиков различных фармакологических групп стандартным методом. Препарат считают эффективным по индивидуальной чувствительности, если он вызывает полное подавление размножения хламидий (наименьший% ЦПВ). Больной О., 44 лет, обратился с жалобами на наличие скудных, прозрачных выделений из наружного отверстия мочеиспускательного канала, чувства неполного опорожнения мочевого пузыря, учащенного мочеиспускания. Жалобы беспокоят в течение последних 10 дней. Неоднократно в течение последних 2 лет половые контакты без презерватива с разными партнершами. Объективно: состояние удовлетворительное, кожные покровы и видимые слизистые чистые, обычной окраски. Периферические лимфоузлы не увеличены. При пальпации со стороны органов брюшной полости и малого таза - без особенностей. Половые органы - развиты, без патологии. Отмечается наличие скудных, прозрачных выделений из уретры. RW(-)отр., Ф-50(-)отр., прямая иммунофлюоресценция (ПИФ) соскоба из уретры на ВПГ (+), посевы на микоплазмы, уреаплазмы и трихомонады (-) отрицательные. При бактериоскопическом исследовании сока предстательной железы определялось увеличение числа лейкоцитов до 40 в поле зрения. Культуральным методом выявлены хламидии в соскобе из уретры и в соке простаты. Пациенту выполнена антибиотикограмма на определение чувствительности хламидии к антибиотикам. При этом исследовании было подтверждено наличие хламидий (в контроле - до 10% ЦПВ+ЭТ), в присутствии антибиотиков: сумамед - 2% ЦПВ+ЭТ, доксициклин+бисептол - 4% ЦПВ+ЭТ, таривид - 6%, рифампицин - 3% ЦПВ+ЭТ, макропен - 0% ЦПВ+ЭТ, ломефлоксацин - 0% ЦПВ+ЭТ, абактал - 4% ЦПВ+ЭТ, рулид - 3% ЦПВ+ЭТ, клацид - 4% ЦПВ+ЭТ, ровамицин - 5% ЦПВ+ЭТ, ципрофлоксацин - 5-6% ЦПВ+ЭТ. Выбрано сочетание антибактериальных препаратов из разных фармакологических групп: наиболее эффективного - макропена и менее эффективного по сравнению с ним - доксициклина+бисептол. Назначено лечение: первые 5 дней доксициклин в/в капельно по 200 мг ежедневно и бисептол 480 по 2 таблетки 2 раза в день, а через 1 день после окончания приема доксициклина - макропен в таблетках по 400 мг 4 раза в день в течение 10 дней. Кроме антибактериальных препаратов больной получал комплексное лечение, включающее также и иммунокорригирующие препараты, протеолитические ферменты, витамины, местную терапию с использованием средств, непосредственно воздействующих на очаг инфекции. В результате проведенного лечения пациент отметил значительное улучшение самочувствия, нормализацию частоты мочеиспускания, исчезли выделения. При контрольных культуральных исследованиях через 2 недели и через 1 месяц - хламидии не обнаружены. 1. On palpation from the organs of the abdominal cavity and pelvis - without features. The genitals are developed, without pathology. The presence of scarce, transparent discharge from the urethra is noted. RW (-) neg., F-50 (-) neg., Direct immunofluorescence (UIF) of urethral scraping for HSV (+), cultures for mycoplasmas, ureaplasmas and Trichomonas (-) are negative. During bacterioscopic examination of prostate juice, an increase in the number of leukocytes to 40 in the field of vision was determined. The chlamydia in scraping from the urethra and in the juice of the prostate was revealed by the cultural method. An antibioticogram was performed on the patient to determine the sensitivity of chlamydia to antibiotics. In this study, the presence of chlamydia was confirmed (in the control - up to 10% CPV + ET), in the presence of antibiotics: sumamed - 2% CPV + ET, doxycycline + biseptol - 4% CPV + ET, tarid - 6%, rifampicin - 3% CPV + ET, Macropen - 0% CPV + ET, Lomefloxacin - 0% CPV + ET, Abactal - 4% CPV + ET, Rulide - 3% CPV + ET, Clacid - 4% CPV + ET, Rovamycin - 5% CPV + ET, ciprofloxacin - 5-6% CPV + ET. A combination of antibacterial drugs from various pharmacological groups was selected: the most effective is macropene and less effective than doxycycline + biseptol. The treatment was prescribed: the first 5 days of doxycycline IV drip 200 mg daily and Biseptol 480 2 tablets 2 times a day, and 1 day after the end of taking doxycycline - macropen in tablets of 400 mg 4 times a day for 10 days. In addition to antibacterial drugs, the patient received complex treatment, which also includes immunocorrective drugs, proteolytic enzymes, vitamins, local therapy using drugs that directly affect the focus of infection. As a result of the treatment, the patient noted a significant improvement in well-being, normalization of the frequency of urination, discharge disappeared. In control culture studies after 2 weeks and after 1 month, chlamydia was not detected.
2. Больная А., 25 лет, обратилась с жалобами на наличие скудных, прозрачных выделений из влагалища. Также беспокоят боли во влагалище при половом контакте. Нарушений менструального цикла нет. Жалобы беспокоят в течение последних 10 дней. Со слов, у последнего партнера обнаружены хламидии. Объективно: состояние удовлетворительное, кожные покровы и видимые слизистые чистые, обычной окраски. Периферические лимфоузлы не увеличены. При пальпации со стороны органов брюшной полости и малого таза - без особенностей. Per vaginum: значительное количество белесых выделений на стенках влагалища, в области цервикального канала определяются прозрачные выделения. Слизистая влагалища гиперемирована. При лабораторном обследовании: RW(-)отр., Ф-50(-)отр. , ПИФ на хламидии (+), посевы на микоплазмы, уреаплазмы и трихомонады отрицательные. При бактериоскопическом исследовании биоценоза влагалища отмечалось наличие грамм-положительной флоры и увеличение числа лейкоцитов до 40-60 в поле зрения. Больной проведено исследование на определение чувствительности хламидий к антибиотикам. Было подтверждено наличие хламидий (в контроле - 7-8% ЦПВ+ЭТ); в присутствии антибиотиков: сумамед - 3-4% ЦПВ+ЭТ, доксициклин - 4% ЦПВ+ЭТ, таривид - 0%, рифампицин - 6% ЦПВ+ЭТ, макропен - 3% ЦПВ+ЭТ, абактал - 5% ЦПВ+ЭТ. Выбрано сочетание антибактериальных препаратов из разных фармакологических групп: наиболее эффективного - таривида и менее эффективного по сравнению с ним - доксициклина. Назначено лечение: первые 5 дней доксициклин в/в капельно по 200 мг ежедневно, а через 1 день - таривид в таблетках по 400 мг 2 раза в день после еды в течение 10 дней. Больной проведено комплексное лечение с учетом менструального цикла, включавшее в себя антибиотики, противовирусную терапию, иммунокорригирующие препараты, местную и симптоматическую терапию. В результате лечения отмечено значительное улучшение самочувствия, исчезли выделения. При контрольных культуральных исследованиях, выполненных с учетом менструального цикла через 2 недели и через 1 месяц - хламидии не обнаружены. 2. In control culture studies performed taking into account the menstrual cycle after 2 weeks and after 1 month, chlamydia was not detected.
Предложенный способ лечения применяли у 16 больных УГХ в возрасте от 21 до 46 лет. Диагноз поставлен на основании бактериологических исследований. У всех больных предварительно определяли эффективность антибиотиков по исследованию чувствительности возбудителя к ним. В результате лечения полное выздоровление, подтвержденное клинически и бактериологически, отмечено у 15 человек (93,8%). У одного больного полного излечения не наступило (в контрольном анализе - единичные ЦПВ без ЭТ), что обусловлено наличием герпеса в острой форме.
