Abstract
The utility model relates to medicine and ophthalmology, namely to microsurgical instruments used in the massage of the meibomian glands due to clogging of the lumen of the ducts of these glands. The technical result of the utility model is an increase in the secretory activity of the meibomian glands, which, ultimately, will reduce the evaporation of tears from the ocular surface and eliminate visual discomfort. The specified technical result is achieved by the fact that the microsurgical instrument for massaging the meibomian glands in case of their dysfunction contains two working parts located in the same plane. One working part is made in the form of a pointed cone (spear) 5 mm long, the base diameter of which is 0.2 mm. The place of closure of the branches is connected to the base of the spear of the working part by a transitional part, the length of which is 10 mm.
Description
The utility model relates to medicine and ophthalmology, namely to microsurgical instruments used in the massage of the meibomian glands due to clogging of the lumen of the ducts of these glands.
[Nelson JD, Shimazaki J, Benítez del Castillo JM. The international workshop on meibomian gland dysfunction: report of the definition and classification subcommittee. Invest Ophthalmol Vis Sci. 2011; 52: 1930-1937.]. [Mishima S, Maurice DM. The oily layer of the tear film and evaporation from the corneal surface. Exp Eye Res. 1961; 1: 39-45.]. [Yin Y, Gong L. Reversibility of gland dropout and significance of eyelid hygiene treatment in Meibomian gland dysfunction. Cornea. 2017; 36: 332-337.].It is well known that the meibomian glands play an important role in creating the quality of the tear film, since layers of polar and non-polar lipids are formed from their secretion, holding the tear on the ocular surface, preventing its evaporation [Nelson JD, Shimazaki J, Benítez del Castillo JM. The international workshop on meibomian gland dysfunction: report of the definition and classification subcommittee. Invest Ophthalmol Vis Sci. 2011; 52: 1930-1937]. One of the chronic diffuse anomalies of the meibomian glands is meibomian gland dysfunction, usually characterized by obstruction of the excretory ducts or changes in the secretion secreted by them, which can lead to changes in the composition of the tear film, clinically significant inflammation, diseases of the ocular surface and symptoms of eye irritation [Mishima S, Maurice DM . The oily layer of the tear film and evaporation from the corneal surface. Exp Eye Res. 1961; 1: 39-45.]. In this regard, an important aspect in the activation of the meibomian glands are measures aimed at improving their expression. According to the literature data, today there are three main therapeutic directions in the prevention and treatment of meibomian gland dysfunction, namely: eyelid massage using ophthalmic gels for eyelid hygiene, tear replacement therapy based on phospholipid components, systemic therapy using drugs containing Omega-3, Omega-6 fatty acids [Yin Y, Gong L. Reversibility of gland dropout and significance of eyelid hygiene treatment in Meibomian gland dysfunction. Cornea. 2017; 36: 332-337].

The patient is once instilled with 1 drop of local ocular anesthetic and ocular antiseptic in both eyes and seated at the workplace of the slit lamp. Then, using the working part in the form of a spear of a microsurgical instrument, the clogged lumen of the meibomian gland duct is punctured, followed by the capture by the opposite working part in the form of sponges of the inner and outer edges of one of the eyelids of the visible zone, performing several compressive movements, thus performing a massage. Then, with the help of a sterile tupfer, the released secret of the lumen of the duct of the clogged meibomian gland is removed from the surface of the edges of the eyelid. The massage procedure is repeated on another eyelid. Upon completion of the massage of the meibomian glands, the patient is subjected to the installation of an eye antiseptic in both eyes. Massage is carried out daily for a month, with the possibility of repeating the course.
