Предлагаемое изобретение относится к медицине, в частности к способам обработки консервированной крови перед трансфузией. The present invention relates to medicine, in particular to methods for processing canned blood before transfusion.
Одним из компонентов крови, наиболее часто используемых в трансфузионной терапии, является эритроцитарная масса. Однако уже со второго дня хранения в эритроцитах происходят существенные изменения: ухудшаются их реологические свойства, снижается активность каталазы, происходит диск-сферическая трансформация эритроцитов, снижается их связывающая способность и т.д. Это приводит к тому, что переливания эритроцитарной массы даже небольших сроков хранения (4-7 дней) сопровождаются значительными посттрансфузионными осложнениями: развитием ДВС-синдрома, микроциркуляторными изменениями в легких, печени, почках и других органах. Все это приводит к снижению эффективности трансфузий. One of the blood components most commonly used in transfusion therapy is red blood cell mass. However, from the second day of storage, significant changes occur in red blood cells: their rheological properties deteriorate, catalase activity decreases, disk-spherical transformation of red blood cells occurs, their binding ability decreases, etc. This leads to the fact that red blood cell transfusions of even small storage periods (4-7 days) are accompanied by significant post-transfusion complications: the development of DIC, microcirculatory changes in the lungs, liver, kidneys and other organs. All this leads to a decrease in the effectiveness of transfusions.

И.Н.Пиксин и соавт./Каталазная активность и перекисное окисление липидов донорской крови при ультрафиолетовом облучении//Вестн. хирургии им. И.И. Грекова. - 1994. - N 1-2. - С. 119-120). В качестве прототипа нами выбран способ предтрансфузионной обработки консервированной эритроцитарной массы, заключающийся в перфузии консервированных эритроцитов 15-20 суток хранения через колонку с углеродными волокнистыми адсорбентами (см. К.Я.Гуревич, О.А.Портной. Перспективы применения активированных углеродных волокон для улучшения характеристик консервированной крови// Гематол. и трансфузиол. - 1987. - N 11. - С. 58-61. Задачей предлагаемого изобретения является предупреждение посттранфузионных осложнений за счет повышения эффективности восстановления функциональных свойств эритроцитов общепринятых сроков хранения. Поставленная задача решается за счет того, что в способе, включающем перфузию эритроцитов через колонку с сорбентом, во время трансфузии дополнительно эритроциты общепринятых сроков хранения облучают светом гелий-неонового лазера. Способ предтрансфузионной обработки консервированной эритроцитарной массы осуществляют следующим образом: непосредственно перед переливанием эритроцитарной донорской массы к трансфузионному контуру подсоединяют гемосорбционную колонку, содержащую 0,8 г углеволокнистого сорбента. Один виток силиконовой трубки системы помещают в камеру для экстракорпорального лазерного облучения ("ПЭЛОК"), куда подсоединен световод гелий-неонового лазера. В камере суммарная мощность рассеянного пучка света составляет 0,2 мВт. Скорость прокапывания эритроцитарной массы для трансфузий 30 мл/час. Время обработки равно продолжительности прокапывания всего объема эритроцитарной массы в стандартном пластиковом мешке - 300 мл. Через колонку можно пропускать до 1,5 л консервированной эритроцитарной массы. Если после обработки эритроцитарной массы по способу прототипа большинство важнейших эритроцитов: метаболическая активность (оцениваемая по поглощению глюкозы), концентрация токсичных продуктов метаболизма (молекулы средней массы и малоновый диальдегид), ионный баланс (внутриклеточная концентрация Na+), - хотя и несколько улучшаются, однако остаются достоверно отличными от аналогичных показателей в свежезаготовленной крови. После обработки консервированных эритроцитов предложенным способом эти показатели не отличаются от значений для свежезаготовленных эритроцитов. Кроме того, после комплексной обработки в значительно большей степени улучшаются такие показатели как диеновые коньюгаты (достоверность отличия двух различных способов обработки p<0,05); степень диск-сферической трансформации (% эхиноцитов, p<0,02); внутриэритроцитарное содержание K+ (p<0,05).If, after processing the erythrocyte mass by the prototype method, the most important erythrocytes: metabolic activity (measured by glucose uptake), concentration of toxic metabolic products (medium-weight molecules and malondialdehyde), ionic balance (intracellular concentration of Na + ), although they improve somewhat, however remain significantly different from those in freshly prepared blood. Следует отметить, что после предтрансфузионной обработки консервированных эритроцитов существенно улучшается важнейший реологический параметр - их деформируемость и восстанавливается барьерная функция мембран (причем после обработки нашим способом проницаемость к мочевине снижается достоверно, чего нет при использовании способа прототипа).
