Предлагаемое изобретение относится к медицине, а именно к способам получения псевдомонофиламентного хирургического шовного материала с антимикробным действием и может быть применен во время хирургических операций при наличии высокого риска гнойно-воспалительных раневых осложнений. Прототипом предлагаемого изобретения является способ получения хирургического шовного материала с антимикробным действием (авт.св. 1473147 А1, А 61 L 17/00), заключающийся в импрегнации традиционных хирургических нитей спирторастворимым антибиотиком или комбинацией антибиотиков в течение 5-24 ч и закреплением их 10%-ным ацетоновым раствором клея "Сульфакрилат" в структуре нити. За счет этого достигается депонирование в нитях до 28 мас.% антибиотиков, что обеспечивает антимикробную активность в тканях сроком до 6-8 дней. Поставленная цель достигается тем, что при импрегнации увеличивается время экспозиции традиционного шовного материала в насыщенном спиртовом растворе антибиотика или смеси антибиотиков до 48 ч, что обеспечивает проникновение антимикробного препарата в структуру нити на всю ее глубину, а закрепление антибиотика клеем "Сульфакрилат" производится в 15%-ном ацетоновом растворе, за счет чего создается монолитное полимерное покрытие, обеспечивающее в первые часы и сутки свойства монофиламентности и продляющее антимикробный эффект в тканях до 11-14 суток. Способ осуществляется следующим образом. Традиционные хирургические нити (капрон, лавсан, шелк, кетгут) после стерилизации любым принятым методом погружаются в насыщенный спиртовой раствор (96%-ного этилового спирта) спирторастворимого антибиотика или смеси таких антибиотиков. Уровнем насыщения является для эритромицина фосфата - 40%; левомицетина сукцината - 7%; тетрациклина - 6%. Максимальный уровень насыщения достигается уже к 22 часам и составляет: 1) для эритромицина в капроне - 52,3; в лавсане - 36,2; в шелке - 53,5; в кетгуте - 23,1 мас.%; 2) для левомицетина - 20,8; 6,1; 26,8 и 21,4 мас. %; 3) для тетрациклина - 35,1; 17,4; 36,7 и 23,2 мас.% соответственно. Однако при увеличении времени экспозиции, как установлено нашими исследованиями микроскопией поперечных срезов нитей, методикой отмывания импрегнированных нитей в проточной воде с последующим их взвешиванием и проверкой остаточного уровня антимикробиной активности посевом на тест-культуры раневой госпитальной инфекции, происходит проникновение антибиотика в структуру лигатур на всю их глубину, что обеспечивает более надежное депонирование антимикробного агента и более равномерное и длительное выделение его в ткани. Таким критическим сроком экспозиции является период в 48 ч, свыше которого существенных изменений динамики процесса импрегнации уже не отмечается. The method is as follows. Извлеченные из клеевого раствора нити просушиваются при комнатной температуре до полимеризации клеевого покрытия, которая наступает через 2-3 мин. Исследования с имплантацией полученного данным способом шовного материала в эксперименте на лабораторных животных показали, что высокий уровень антибактериальной активности в тканях организма сохраняется до 11-12 суток с прослеживающимся эффектом до 14 суток (см. чертеж). Обработанный по заявляемому способу хирургический шовный материал применен в клинике на 68 операциях при экстренной патологии органов брюшной полости, сопровождающейся разлитым перитонитом. Отмечено уменьшение гнойно-воспалительных раневых осложнений по сравнению с контрольной группой с 14,8 до 6,2%. В исследуемой группе отсутствовали такие осложнения, как эвентрация в гнойную рану и лигатурные свищи в отдаленном периоде. Полученный по заявляемому способу шовный материал позволяет снизить количество гнойно-воспалительных раневых осложнений при операциях в заведомо инфицированных тканях до уровня 6%.
